Nine Ways to Improve Your Patient Communications - Part 3
 
Research has demonstrated the connection between better communication scores and improved patient adherence, lower re-admission rates, improved mortality rates, lower malpractice risk, and reduced cost per case. No other area of medical knowledge or technical skill has a greater impact on our patients. Try some of these suggestions.
 
1. Choose Positive Words
 
To communicate intent, use the words “for you” and “with you” as often as possible. Consider the difference: “I will call your family to update them” or “I will call your family for you to update them.” Similarly, see the difference between “I am here to make sure your care is coordinated” versus “I am here with you to make sure your care is coordinated.” Also make a point to show appreciation. Say thank you, pay a compliment, or express your thanks to your patients while they are going through a potentially difficult and challenging experience. For example, “Thank you for coming in today,” or “I appreciate all the effort you are investing in your recovery.” Patients often do not expect gratitude, and that makes expressing it particularly heartwarming.

When served with a subpoena to produce documents for a patient suing a thirdparty (like an insurance company or an employer) can you charge for the cost of production of these documents? If so, then how much?

Being in practice a long time, I have many years worth of paper charts. Has anyone come up with a good system to go paperless and scan the information in the old charts? We have an EMR since 2005, but I need the information prior to that year.

Thank you Dr. Ross for mentioning Viagra as something to be concerned with regarding a patient's blood pressure as it is a vasodilator and pressure can be affected, as it had slipped my mind. He reminded me that I also have suggested both Viagra and Cialis to both male and female patients as a potential first-line defense against symptoms associated with Raynaud’s due to the very fact that they are vasodilators. You will get some raised eyebrows, but once you explain its mechanism of action, it is taken seriously and these patients will do almost anything to address their pain and reduce their chances of potential serious complications.

I would add the use of a high-quality thin wool sock. A vascular interventionalist I know uses Viagra in very low doses, but I have not tried this for patients yet and agree with a cautionary advice about using dual treatments that can cause hypotension.

Here is a treatment I’ve used successfully for many years... 200mg of coenzyme Q10 given QD. It works in just several days and is very safe.

I've mentioned before that Raynaud's phenomenon/chilblains often responds well to L-methyl folate. It doesn't interact with any medications. It is commonly used for depression, diabetic neuropathy, Alzheimer's disease, and even as a prenatal vitamin. The only thing recommended is to take with B12 to avoid anemia. It often comes mixed with the B12 to simplify this process. It may be contraindicated with cancer or a history of seizures. If this is the case, have them clear with their PCP prior to starting. In less severe cases, I have them use it seasonally. It can be used in the pediatric through geriatric patient. 
 
There is no problem having the patient start or stop on a dime. Sometimes there can be some temporary insomnia for the first two weeks when first starting the medication. So have them take qd in the AM for a couple of weeks. The patient usually needs to use at least 3 months to decide if it will work for them. It is easier to use without side- effects than Minipress or Nitro paste, and possibly even cheaper. As Allen Jacobs, DPM has mentioned before, a good source of this product is through EBM medical.

It is perhaps the best foot prevention and intervention article ever written for the medical profession. It educates the practitioner when to refer patients to the appropriate specialty (frequently podiatry) for routine and urgent care.

About 10 years ago, my wife was having some upper GI symptoms. Upon a recommendation, we made an appointment with a local GI group. We were told during that call that she would have to prep before the appointment for full colonoscopy and endoscopy. Thinking that it would save time once the doctor interviewed and examined her, and if it was indicated, we followed the instructions.
 
Upon prepping, and going to the appointment, filling in the paperwork, they attempted to bring us directly to the procedure room. Never seeing a doctor yet! We protested and refused. We were told this was standard procedure, and everyone coming in goes through this. We demanded the doctor consultation first. We found out, after the fact, that they were known as a scoping factory. We never went back there again. Do what is appropriate, nothing more, nothing less.
