PODIATRISTS IN THE NEWS
Pointy-Toed Shoes Cause "Crushed" Toes: PA Podiatrist

One of the most common foot problems Dr. Jane Pontious sees when she works with women on a day-to-day basis is “crushed” toes from shoes that are too narrow. When one’s toes are repeatedly shoved together in pointy-toed heels, the damage to their feet starts to surface in the form of bunions, hammertoes, muscle imbalances, and even nerve damage.

Dr. Jane Pontious

“I always tell patients to look at the front of your foot and at the shape of it, and at the shoe that you’re buying,” says Pontious. “If it’s coming to a point like a pump, remember that your toes are being pushed in from both sides, and eventually those toes are going to have problems because of it.”

PODIATRISTS IN THE COMMUNITY- PART 1
MT Podiatrists Open Sky Foot and Ankle Institute

Drs. Dallin Greene and Nathan Judd have lived all over the country in pursuit of their medical careers, but the duo says they’ve decided to locate a podiatry practice in Butte because they like Montana and because they like the people. In November, Greene and Judd opened Big Sky Foot and Ankle Institute to fulfill a need in the community but also to live the Montana lifestyle. But before moving to Butte, Greene already had experience with the Big Sky State. A native of Phoenix, Greene said his father often took him on hunting trips in the Gravelly Mountain Range, while his summers were filled with building log cabins and fishing near the Idaho-Montana border.

Judd, who’s married to Greene’s older sister, said he and the Phoenix native went to podiatry school together and also went to the same institution for their surgical residency. Community service is another important aspect of their philosophy. As medical missionaries, Greene and Judd have traveled throughout the world, including to Mexico and Guatemala, where they and other doctors set up pop-up clinics and sometimes treated over 100 people per day.

MT Podiatrist Joins St. Mary's Superior Clinic

Podiatrist and podiatric surgeon Dr. Paul Ouradnik joined St. Mary's Superior Clinic. Ouradnik will also serve patients at Essentia's clinics in Ashland and Hayward. He previously worked in private practice in Billings, MT for the past 12 years.

Dr. Paul Ouradnik

Ouradnik obtained his podiatric medical degree from Des Moines (Iowa) University College of Podiatric Medicine and completed his residency in podiatric surgery at Riverside Mercy Hospital in Toledo, OH. He is certified by the American Board of Foot and Ankle Surgery.

PRACTICE MANAGEMENT TIP OF THE DAY
12 Ways Physicians Can Maintain Sanity - Part 2

In pursuit of offering expert and effective patient care, physicians sometimes shortchange their own health and then discover, too late, that the path back to healthy living is quite challenging. Why not offer great care and maintain your sanity as well as health?

Here are 12 tiny changes you can put into practice now to maintain and/or improve your health:

3. Clean your desk. Too much paper and clutter can contribute to stress. If you keep your desk organized, it will be easier to focus on the task at hand and avoid feeling overwhelmed. With the hectic lives most of us lead today, even a little bit of de-stressing can make a big difference.

 4. Turn off the TV. Studies show that men who watch less than two hours of TV per day have, on average, lower blood pressure and a lower BMI than those who watch more than two hours daily. Sure, watch the Patriots play in the Super Bowl or enjoy your favorite weekly sitcom, but watching less TV leaves you time for other things — like working out, exploring new hobbies, or teaching your kids how to play football.

RESPONSES/COMMENTS (OBITUARIES)

RE: The Passing of Steven Richman, DPM

Dr. Steven Richman, Illinois College of Podiatric Medicine Class of 1981 has passed away. Gentleman, scholar, entrepreneur, innovator, teacher, counselor, colleague, and true friend. These are just a few words that characterized him.

Dr. Steven Richman

For all of us who knew him and were touched in so many ways by his kind heart and good will, Steve will be sorely missed. His spirit and memory will be everlasting. May G-d bless his soul.

RESPONSES/COMMENTS (ETHICS) - PART 1A
From: Thomas Graziano, DPM, MD

I encourage anyone doing surgery to opt out of these low paying private insurance plans. It's not worth the liability exposure. When you do opt out, your case volume will go down but your income will increase. Another interesting dynamic you'll find is that insurance companies will be contacting you to negotiate your fees. They'll ask you to accept a lower fee. My favorite and most gratifying thing is to tell them that when I was in-network they never negotiated my fees.

So now I won't be negotiating. Guess what? They have to pay you. I urge you to take more pride in the services you provide and stop being abused by the insurance industry.

RESPONSES/COMMENTS (ETHICS) - PART 1B
From: Eric Edelman, DPM

This is not an ethical dilemma. A discourse on euthanasia can present an ethical dilemma. In this case, you have assumed care for this patient, come to a diagnosis, and have a recommended/planned course of treatment. You have a fiduciary duty to place your patient's interest ahead of your own. It's part of the doctor - patient relationship.

If you are unhappy with the $237 fee for the surgery (and I would be too), consider dropping Amerigroup in the future. Or work a little more efficiently, devote 3 hours to the case and post-op care (17 year olds usually do pretty well with anything), and you're at $79 an hour. Almost as much as plumbers' wages!

RESPONSES/COMMENTS (PM NEWS QUICK POLLS )
From: Gary S Smith, DPM

Like many regulations, they tend to have just the opposite effect. I have actually increased my opioid prescriptions. I only prescribe them for post-op patients and I have always prescribed 20 pills at a time. Most patients don’t even use all of those. If patients needed more, they could call me at home or the office and we could call more in. The new regulations require these post-op patients to physically come to the office for an Rx. If it is on the weekend, it is even more of a problem. To try to avoid these issues, I have started prescribing 30 pills at a time, thus increasing my opioid prescribing by 50%.
