NJ Podiatrist Donates His Work to the Smithsonian
Dr. Jonathan Singer, a retired podiatrist, is a full-time artist, painting everything from graffiti walls to guitars. But it's his exquisite photographs of flowers, collected on handmade paper in Botanica Magnifica, which now reside in the National Museum of Natural History's rare book room, that cemented his reputation in the worlds of art and botany. Last Summer, Singer made another donation, Photo Nuage, 56 photgraphic prints, double-elephant size, in a custom-made clamshell box.

Singer, who is 67, didn't pursue photography in earnest until around 13 years ago when he received a diagnosis of Parkinson's disease, which forced him to give up surgery but not photography. To offset the tremors, he puts the cable release in his mouth and manually focuses with his good hand.

| RESPONSES/COMMENTS (CLINICAL) - PART 1A | |
From: Evan Meltzer, DPM
I have used micronized EpiFix by MiMedx suspended in 0.5% Marcaine for injection. The product comes in 40, 100, and 160 mg sizes. I suggest using the 100 mg size for plantar fasciitis suspended in 3 ml of Marcaine. Inject the same way you do with cortisone. This product is anti-inflammatory, a stem cell magnet, and contains a variety of growth factors. A repeat injection may be needed after 6 months. Note: the patient will have 2 days of post-injection pain and needs an Rx for pain control for those days. Tramadol works well (I am having my second knee injection with EpiFix today after clinic for a partial medial meniscus tear. The first one gave me 90% improvement).
Evan Meltzer, DPM, San Antonio, TX

| RESPONSES/COMMENTS (CLINICAL) - PART 1B | |
From: Richard Adams, DPM
Try evaluation with diagnostic ultrasound. Steve Barrett has performed exhaustive studies regarding recalcitrant heel pain evaluation utilizing MSK ultrasonography. If MSK ultrasound is negative, the patient is likely suffering from medial calcaneal neuritis. Try a diagnostic block of the medial calcaneal nerve with simple Xylocaine plain. The patient will think you are a genius when, five minutes later, he is able to stand without pain. The experience will be rewarding and positive for the patient and physician alike.
Richard Adams, DPM, Granbury, TX

| RESPONSES/COMMENTS (CLINICAL) - PART 1C | |
From: Elliot Udell, DPM
We have all seen cases of recalcitrant heel pain in our practices, and we inevitably think: "what the heck am I missing?" It appears from the clinical description that the patient was thoroughly worked up and evaluated from radiological, rheumatological and neurological perspectives, and no clinical diagnosis was obtained. What you might attempt is to treat this patient symptomatically with a combination of intensive physical medicine, such as interferential therapy, injection therapy which would include posterior tibial injections, strappings to off-load the plantar fascia, and oral non-steroidals.
The patient must be told that these conditions often take many weeks to totally heal and he or she should not expect "overnight" relief.
Elliot Udell, DPM, Hicksville, NY

| RESPONSES/COMMENTS ( DME) | |
From: Philbert Kuo, DPM
In my practice, I do not think the number of Medicare shoe audits has increased or decreased in the last few years. I am still waiting to be paid for shoes dispensed in November, 2015. Medicare has apparently contracted with a company to scrutinize documentation. The paperwork remains as onerous as it ever was.
Philbert Kuo, DPM, Chesapeake, VA

|
| RESPONSES/COMMENTS (NON-CLINICAL) | |
From: Ira Baum, DPM
Dr. Agostinelli and others, who are involved with orthopedic groups, have amplified my concerns regarding "Does podiatry have a place in the future of healthcare?" It was not my intent to have podiatrists express their satisfaction as part of an orthopedic group, but to illuminate the possibility, that if podiatry is or becomes solely a surgical specialty, then podiatry may be on the path to extinction.
As I have stated previously, the residency requirements have moved the profession in a distinct surgical direction, and may be the spark that ignited the focus of my concern: Is there a future for podiatry in its current...
