PODIATRY IN THE NEWS
Podiatrist Among "12 Gross Jobs That Pay Pretty Well"

Feet. Some people love them. Most people try to avoid them. The first issue with feet is that they can smell. And smell pretty bad, too. Being trapped in socks and shoes all day doesn't help, but then you get things like corns, bunions, athlete's foot, ingrown toenails, and nail fungus. Not the kind of thing anyone wants to see first thing in the day. However, the financial rewards are great. The average podiatrist takes home $177,000 per year. Of course, it takes time and money to become qualified, but that soon pays off.

PODIATRISTS AND FOOTWEAR
Platforms Work Best for High Heel Wearers: NY Podiatrist

Many women are high on heels, but they neglect to realize that over-wearing them can be a step in the wrong direction. "Women do love their high heels, and rightfully so. They make a leg look fantastic. However, there are certain consequences with wearing high heels," says Dr. Thomas Goldman, a podiatrist from Mount Sinai Beth Israel.

Dr. Thomas Goldman

First, the incline is an anatomical disaster, so he says let your foot lay low. "Stick with a two-inch heel. When you start getting up above that, try to wear a platform type of heel that raises both the front of the foot and the back of the foot. This gives the image of a higher heel. However, it keeps the foot in a more optimal anatomical position," Goldman says.

CODINGLINE CORNER
Query: Tibialis Anterior Repair Coding

I performed a tibialis anterior repair with excision of damaged tendon, slide graft, and re-attachment to the cuneiform. What is the appropriate CPT coding for this procedure?

Carl Ganio, DPM, Vero Beach, FL

Response: A tendon repair is a tendon repair no matter what steps are required to repair it. I suggest CPT 27665 (repair of extensor tendon of the leg).

Tony Poggio, DPM, Alameda, CA

ACE USA and RPS Healthcare Expand Podiatrist Professional Liability Coverage

The Patient Protection and Affordable Care Act and Centers for Medicare & Medicaid guidelines and rules resulted in a record number of investigations and enforcement actions in 2011. RESPONSES/COMMENTS (CLINICAL)
From: Stephen Musser, DPM

With all due respect, if I were a betting man (I'm not), this looks/sounds like a case of granuloma annulare. A biopsy is the way to go to rule out this self-limiting condition. Reasons why ringworm would be lower on my list are, first: in a young girl, ringworm is not very common (yes, it can happen, but it is not common); second: the condition is on the dorsal aspect of the foot.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
From: Ken Hatch, DPM, Joseph Borreggine, DPM

I ALWAYS paid my staff if I shut down However, on MANY snow days my office manager came in anyway and WE worked on outstanding charts with insurance/billing questions.

Ken Hatch, DPM, Annapolis, MD, KLHDOC@aol.com

Our office written policy allows our employees three paid "emergency days" that are considered "acts of God", which will also cover any weather conditions preventing the employees from coming to work. All employees who are hired are given these days upon the start of their employment. They are renewed annually. Each employee signs a receipt of all office policies.

It is an executive decision on my part (as I am liable for all parties involved regarding employment and commerce). This decision is solely based on the safety and welfare of not only the employees, but for the patients' safety as well. When a general public warning is or has been given to the surrounding community regarding emergency driving only, or the weather conditions have resulted in school closures, then I usually close the office that day.

When all three of the "emergency days" are exhausted, then the employee may choose to use their allowed "Paid Time Off" (PTO) or vacation days to receive compensation for that day. If they choose to do neither, then then they will not be compensated for that day. If an employee has already arrived at work and the weather or "Act of God" will or has happened, then they will be told to go home and be compensated accordingly based on their available "emergency", PTO, and vacation days. If the employee opts not to leave the office and continues to stay and work, then they will be doing so at their own risk.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
From: Joel Lang, DPM

When I was in practice, we had 'defined policies' for handling snow days as well as most other office-related issues. I used paper appointment books at that time, but for those with electronic scheduling, the process can be modified accordingly.

When snow was in the forecast, my secretary would take home a photocopy of the appointment schedule for the day(s) in question, as well as the next 5 days of appointments. Next to each name for the next day or two, she would...

RESPONSES/COMMENTS (PM ARTICLES)
RE: Valleix’s Sign (Tarsal Tunnel Syndrome, Jan 2014 PM)
From: David Secord, DPM, Kyle Scholnick, DPM

On page 123 of the January 2014 Podiatry Management Magazine, Dr. Scholnick refers to “Apositive Tinel’s or Valleix’s sign is when paraesthesias course along the involved nerve distally or proximately, respectively.” This is my favorite 'medical urban legend' and has been my personal quest to expunge from our schools and usage.

As opposed to what many of us were taught in medical school, Tinel's sign is both proximal and
distal tingling upon percussion of a nerve. Valleix’s sign does not exist. There is no such thing. Valleix’s POINTS are as follows: 1. Where the nerve emerges from...

RESPONSES/COMMENTS (PM JURY VERDICT REPORTER)
From: Bryan C. Markinson, DPM

Once again another discourse about the righteousness of defense experts and their loathsome plaintiff expert counterparts. While the debate is perpetual, it simply hinges too much on our visceral reactions in this space. Of course, in this debate, it's easy to find support for the defense side, harder for the plaintiff side.

Making the statement, "I will never review for the plaintiff again (Jacobson)" and rationalizing it with, "They'll always find someone to testify against the truth," is simply out of touch and quite frankly immature. If Dr. Jacobson found himself on the bad side of a alleged deviation, you bet he would...
