Obesity is a Growing Problem Affecting the Feet: IN Podiatrist

"A growing health issue we are seeing is carrying too much weight, whether this is 10 pounds or 100, says podiatrist Dr. Michael Nirenberg. "It does not take much extra weight to cause a foot problem, such as heel pain. If you walk 6,000 steps a day and you are 20 pounds overweight, that ends up being thousands of pounds of extra pressure crashing down on the heel by the end of the day."

"Worse, if your heel hurts, it is harder to walk or do other exercise, which creates a vicious cycle where you just gain more weight and your heel hurts more. Some overweight people believe if they lose the weight, their heel, foot, or ankle pain will go away. This is often not the case. Once a tendon or ligament is injured, it needs to be treated," says Nirenberg.

No Surprises in Final Essential-Benefits Rule

HHS on Wednesday released a final regulation on the health reform law's essential health benefits, actuarial value, and accreditation provisions that closely follow the department's proposed rule from last November. The final rule maps out standards for the core set of benefits that health insurance issuers must cover in the individual and small group markets—both inside and outside of the health insurance exchanges—for 2014 and 2015.

Those essential health benefits must include items and services within at least 10 categories, including emergency services, hospitalization, mental health and substance abuse services, prescription drugs, maternity care, and pediatric services. HHS will revisit the provisions for the 2016 benefit year.

Web Ratings Aren't Key in Choosing Doc: Study

Geography and physician referral still top the Internet when it comes to patients' healthcare choices, but insurance coverage is by far the biggest driver when it comes to parents choosing a doctor for their children, according to a new national survey.

“Accepts my health insurance” was rated as “very important” by 92% of the respondents in a survey commissioned by the University of Michigan C.S. Mott Children's Hospital in Ann Arbor. Coming in a distant second was a convenient office location, which was rated as a very important decision-making factor by 65% of the respondents; followed by doctor's years of experience, 52%; recommendations from family and friends, 50%; referral by another doctor, 40%; and doctor's online ratings, 25%.

Does anyone know if Dan Bluth, DPM still puts out the Podiatry Manual. I have unsuccessfully been attempting to obtain the 2013 edition, if there is one.

RE: Fixation Vs. Non-Fixation of Osteotomies

Metatarsal and phalangeal osteotomies have been useful procedures for correcting a lot of forefoot pathology. The subject of whether it is better to fixate or not to fixate these osteotomies is an important topic. Dale Austin, who was a podiatrist and orthopedist, performed 1,200 non-fixated Austin bunionectomies without one non-union or aseptic necrosis. Dr. Stephen Barrett writes in Podiatry Today that a non-fixated metatarsal osteotomy has much less of a chance of becoming a non-union than a fixated osteotomy. The orthopedic community outside the United States has enthusiastically embraced MIS procedures with non-fixation or percutaneous fixation.

Another interesting discussion is whether you get more transfer lesions with non-fixated metatarsal osteotomies. It appears to me that, of the podiatrists who have had extensive experience with both techniques, most of them were happier and had better results with non-fixated osteotomies. Mariano de Prado, an orthopedist and a co-author of Minimally Invasive Foot Surgery prefers non-fixated osteotomies. When he was challenged by an American orthopedist, he said, "I used to do one or two foot surgeries a month, but with MIS surgery and non-fixated osteotomies (some MIS surgeons use percutaneous fixation), I now do twenty foot surgeries a week and my patients get better results and are much happier."

RE: The Biomechanics of The First Ray in Dance (Michael Forman, DPM)

We believe Dr. Forman’s following comment is dated and slightly un-informed: “Even if we were able to make a device for a ballet shoe, it is not going to have any effect when the dancer is completely on the distal aspect of the toes.”

We do have orthotic technology that can enhance the stability of the toes when the foot is “en pointe”. It is called the Spring Orthotic Device, which was patented last month by Kingetics LLC.

A recent article in Podiatry Management magazine has caused a bit of a controversy. One of our American Academy of Podiatric Sports Medicine members wrote an article on running form and shoe types. The Academy's logo was inserted at the beginning of the article leading many to believe that we were endorsing this individual's thoughts. This article led to many phone conversations and Internet correspondence.

The reality is that the author was just expressing his ideas. As an Academy, we applaud those members who are willing to share their knowledge through writing. This is where it gets interesting. The AAPSM also strongly urges...

RE: Life-Saving Role of Podiatric Physicians

It is time for podiatric medicine to focus seriously, including acquiring evidence-based data, on the critical importance of the profession in reducing morbidity due to cardiovascular and other life-threatening disorders.  One scenario makes the point, even if simplistically. A patient visiting a primary care provider is advised to engage in an exercise regimen. The patient leaves, goes home, and fails to do so. 

The reason: a painful pedal problem discourages or even makes it impossible for the patient to engage in common exercise regimens that they might follow. Such scenarios could result in increased cardiovascular morbidity, some with tragic but completely preventable outcomes. It is long overdue for podiatric physician researchers to measure the frequency of this and similar scenarios and the impact of podiatric medical care. Much more focus is needed on major contributions we can make in saving lives or bettering one's quality of life.

RE: Proper Shoes Can Prevent Many Common Foot Problems: AL Podiatrist

With my understanding that there are some days that have greater amounts of newsworthy stories than others, I would appreciate PM News not printing those articles in “Podiatrists in the News”, where there is discussion of overly simplistic foot treatments. I believe it is a disservice to the quality of PM News, where there are articles relating quotes such as recommending carefully washing and drying between the toes, wearing properly fitting shoes, and cutting one’s toenails straight across.  Given the evolution of podiatry and the many other articles related to a higher caliber of education and thinking, I would personally feel embarrassed for other medical colleagues to read those parts of PM News.

Editor's Comment: It is surprising that Dr. Kaye would complain about this type of promotional story when he has been the subject of recent similar stories in PM News. "CA Podiatrist Discusses Breaking in Shoes" and "High Heels Create Potential Health Issues for Young Girls: CA Podiatrist."

We think that Dr. Kaye is missing the point. The motivation for publishing these stories is NOT to educate podiatrists about feet, but to inspire podiatrists to appear in local newspapers or on TV, and thus create interest for the profession within those podiatrists' communities. Podiatrists featured in media stories will gain many new patients as a result of this exposure, and other podiatrists will also benefit from this publicity. If Dr. Kaye wishes podiatric education, he can find many clinical articles in Podiatry Management magazine.
