UK Podiatrist Provides Running Shoe Buying Advice

"The vast majority of running injuries are due to poor foot function and poor muscle balance, says podiatrist Wayne Edwards, director of operations at HFS Clinics. "When choosing a pair of running shoes, ensure that they fit properly and feel comfortable. It is a myth that you need to go up a shoe size to ensure this comfort; half a size is adequate," says Edwards.

"People have a wide variety of foot shapes. Low-arched mobile feet need more support from the shoe - those available for this are often grouped as stability or motion control shoes. Average-arched feet can be accommodated in most neutral shoe designs. High-arched feet benefit from cushioning. We recommend that people go to a specialist running shop and have video gait analysis to work out the right shoe for them," says Edwards .

AT THE COLLEGES

OCPM Joining With Kent State University on Track for This Summer

Kent State University and the Ohio College of Podiatric Medicine (OCPM) plan to come together into one single, academic entity as early as this summer. Both institutions are collaborating to allow for a smooth transition. The move to join Kent State is part of the strategic plan of the podiatric school to take the college to the next level in teaching and research. Faculty and students of the podiatric college will have the opportunity to engage in research programs with public health, biomedical sciences, and sports medicine. Additionally, access to all men’s and women’s sports teams that exist at Kent State and the ability to work with Kent State faculty, trainers, and team physicians, and be exposed to a variety of varsity and intramural sports activities, could probably lead to a new area of specialization in the Doctor of Podiatric Medicine program.

“This is a great opportunity for the OCPM community to be partnered with a high-caliber institution, such as Kent State,” said Ohio College of Podiatric Medicine President Dr. Thomas V. Podiatrist & Wife Named Duke and Duchess of Mardi Gras Court

Dr. Jayson and Lisa Cortez have been selected as Duke and Duchess of the 2012 King Christopher XXXVII Mardi Gras Court. Dr. Jayson G. Cortez was born in the Philippines and moved to the United States at age 15. He resided in Lawton, OK. Dr. Cortez received his bachelor's degree in zoology from the University of Oklahoma in Norman, OK. He then attended the Dr. Scholl College of Podiatric Medicine in Chicago where he received his Doctor of Podiatric Medicine degree.

Dr. Cortez completed his surgical residency at Wyckoff Heights Medical Center, an affiliate of Cornell University College of Medicine, in New York City. He moved to Leesville in 2001 to begin his medical practice with Dr. Jim Harvey at The Foot Clinic of West Louisiana. He is board certified by the American Board of Podiatric Surgery and The American Board of Lower Extremity Surgery. He is a fellow of the American College of Foot and Ankle Surgery (ACFAS) and The American Society of Podiatric Surgery (ASPS). Dr. Cortez is an active member of Doctors Hospital at Deer Creek in Leesville.

Query: G8553 & eRxing

Since January 1st, I have submitted claims with encounter codes, but did not add the G8553, even though I eRx'ed during those visits and documented it in the encounter note.  What is my recourse? Can I add the G8553 to the specific claims with the encounter codes, and resubmit the claims even though some of the claims have already have been processed and will come back CO18 (duplicate billing)?

Response: You cannot add G8553 after the fact. You have no recourse. Just get 10 in before June 30. There is plenty of time.

RE: Achilles Tendon Pathologies (Norm Wortzman, DPM)

Three classes of medication affect tendon pathology: cortiscosteroids, quinalone antibiotics, and statin cholesterol-reducing agents.
 
The mechanism of action that statins and quinalones have on tendon pathology remains unclear. Statins and quinalones are specific for the Achilles tendon. On rare occasions, rhabdomyolysis has been reported. If a patient on one of these medications suddenly complains of muscle fatigue, myalgia, or Achilles tendon pain, a CPK is indicated; if elevated, the medication should be stopped ASAP, and the patient referred to an internist for additional evaluation.

Myotendinopathy is documented. I suggest running a blood creatine kinase to evaluate subclinical rhabdomyolysis. With the approval of the patient's MD, discontinuance of the Crestor for several weeks may cause an amelioration of the symptomology, adding credence to your diagnosis of iatrogenic disease.

The lesions could be an atypical pyoderma gangrenosum; biopsies are usually negative or reveal neutrophilic infiltrates. The diagnosis therefore is one of exclusion and there is no underlying disease in 50% of cases. Also consider pemphigoid and perform immunoflorescence the next time the lesions erupt.

Try a rocker bottom shoe with a heel pad made of foam with a cut-out in the center.  Make sure the shoe rocks at the MPJs and has minimal heel contact, if any. The surgical alternative would be possible transfer of the anterior tibial tendon to the calcaneous - if the Achilles is not functioning due to the polio.

Also check her limb length, and if the affected foot is the same length or longer, you can do a dorsal V calcaneal osteotomy to decrease the calcaneal inclination angle. Try to keep the surgically-shortened leg within half an inch of the unaffected limb. Try to maintain dorsiflexion to 15 degrees following your procedures.

We use Teregen Labs in Willoughby, OH at 800-848-0055 for all of our Marcaine, lidocaine, Dex acetate and Dex phosphate (they compound them for individual patients with prescriptions from us). I fax the patient's name, address, and phone on the prescriptions, and these products arrive at our office a few days later.

We've been doing business with them for almost 30 years and have never had a problem. We also get our PumiBars from them.

Many years ago, our state association made an effort to STOP using the term podiatrist in our communications. I personally made an effort at a national level to stop using the term podiatrist in favor of the term podiatric physician and pushed others to do so. I feel the other terms "foot and ankle physician" or "surgeons" is just grandiosing ourselves rather than defining ourselves. We are podiatric physicians and surgeons! Podiatrist is just a short version of the same. What we do and how we present ourselves defines us, but we should ALL use the term podiatric physician in communicating to the public and outside agencies.
 
We are podiatric physicians and surgeons just like allopathic physicians are medical physicians and surgeons and osteopaths are osteopathic physicians and surgeons. They all have short terms for who they are too - ENTS, pathologists, orthopedists, dermatologists, etc. Let's not get hung up on beating each other up on terms. Our presentation to the outside world should always be podiatric physicians or, if you prefer, podiatric physicians and surgeons (just a bigger mouthful to get out in conversation).

RE: TX Podiatrist Awarded Patent For Flat Foot Implant (Steven J. Kaniadakis, DPM)
 
I agree with Dr. Kaniadakis that we should hear more about the achievements of our colleagues who have obtained a patent for a new invention. It is surprising that we do not get such information included in some of our seminars as there are a lot of us who have thought of great ideas but failed to move forward due to lack of funds or lack of information of where to go and whom to talk to.

As I understand it, the process of obtaining a patent is very costly, time-consuming, and riddled with legal paperwork. Such apparent deterrence may be in place to avoid the filing of numerous "useless" applications for patents, but I do not know for sure. Again, we need more information or a step-by-step guide on how to apply for a patent, whom to consult, and how to keep secure our newly discovered intellectual property.
