More than one in five adult Americans currently suffer from bunions, and upwards of two-thirds will develop a bunion in their lifetime. For those considering surgery to relieve bunion pain, there is good news from the American College of Foot and Ankle Surgeons (ACFAS) – bunion surgery may not be as bad as you think.

 Childhood Foot Care Requires Specialized Approach: SC Podiatrist
 
“Pediatric foot problems are a distinct class of care and diagnosis, but parents often think that these conditions are a matter of growth that will eventually resolve,” says Michelle Butterworth, DPM, FACFAS, a South Carolina foot and ankle surgeon and Fellow Member and past president of ACFAS. “Every bone in the foot has a growth plate. Many of them remain open, allowing growth, through the teenage years. Before the growth plate is closed and fully developed, foot problems in youth can be difficult to treat, requiring a complex, specialized approach. Foot and ankle surgeons are specially trained in this area of caring for bones that are still developing in children.”

According to Dr. Butterworth, consultation with a foot and ankle expert is important for children to ensure that foot issues developed in childhood don’t become more serious as they get older. “A common misconception I see in patients who previously visited a physician or pediatrician is that children will ‘grow out of’ foot conditions like pediatric flatfoot, which is simply not true,” Dr. Butterworth adds.

The more coders dual-code, the more they will feel like it is their regular work pattern and become very proficient, says Mary Beth Haugen, CEO of Haugen Consulting Group. Coders in the ICD-10 era will never get back to their productivity levels under ICD-9, she cautions. But the more proficient coders become, the lower the productivity hit will be--maybe 20 percent instead of 60-70 percent. Consequently, dual-coding throughout 2015 will give practices a good idea of the productivity levels they can expect and aid in setting appropriate coder staff levels.

Are there any circumstances/medications etc. when a podiatrist can bill for CPT 96372 (therapeutic, prophylactic, or diagnostic injection [specify substance or drug]; subcutaneous or intramuscular). If so, what would be the circumstances (i.e., when would it be appropriate, what medications, etc.)? Is it allowable by Medicare and/or commercial payers to bill the code in addition to other CPT injection codes or, conversely, when the code not be billed with/bundled to other injection codes or medications?

Response: I don't know of any podiatrists who administer IM injections of antibiotics within their offices. However, I could easily foresee injection of tetanus toxoid or antitoxin as long as it is within the state scope of practice. You can bill any payer for the injection administration, as well as the therapeutic supply

There is a problem with a doctor injecting sugar water into a patient and charging 300 dollars for it. Insurance companies will not pay because the evidence is not clear that it does anything. On the other hand, I had a patient who paid 600 dollars to a local doctor for prolotherapy injections and said he got good results. Was it a placebo or was the therapy working? I don't know, but I did buy a bottle of injectable glucose and repeated it for the patient at a fraction of what he was being charged, and the patient continued to say it was helping him. Others with whom I tried the same injection, got no results. I am not a fan of the insurance industry as it relates to healthcare, however, if no insurances will cover a therapy because it is weak on clinical evidence, a red flag should go up on whether to utilize it in an honest clinical practice.

Prolotherapy is being employed here and with some interesting agents (although not unknown agents). A number of agents used in the past for prolotherapy include extracts from several plants, the least unfamiliar being psyllium seed extract or Sylnasol, a product no longer available, and an extract from fish oil, still available in the pharmacopeia - sodium morrhuate. 
 
The chief proliferant agents as judged by the frequency of usage are, however: 1. Glucose, 2. Glycerin and 3. Phenol. They are usually used in the following combination:

I feel compelled to comment on the inquiry regarding prolotherapy. This treatment has been helpful for me in treating many muskuloskeletal problems that we see as podiatrists. Simply put, prolotherapy counts on the body’s own ability to heal itself. I spent a lot of time with a physiatrist (MD) who helped me learn these techniques. My father was a chiropractor, and I have always kept an open mind to “alternative health techniques.”
