Proper Shoes Can Prevent Many Common Foot Problems: AL Podiatrist

Too tight shoes are often the cause of a trip to the podiatrist. Dr. Joseph Dupper said he sees the consequences all the time. "Obviously, you're going to run into issues with pain. One of the things we see in people with tight shoes are ingrown toenails from the nails getting squeezed by the shoe," he said. Dupper said other problems include bunions, hammertoes, crossed toes, and corns. All are problems that could lead to foot surgery.

Many problems can be prevented by picking the right shoe. For women, that means wearing a lower heel and toes that are not pointed. "Women tend to push the envelope as far as shoes are concerned. That's always an issue," added Dupper. He said sensible is the key.

Dr. Jess T. Drezner of Podiatry Associates Group was recently elected to the board of the South Mountain YMCA. Drezner is a graduate of the Pennsylvania State University and the Illinois College of Podiatric Medicine. Dr. Drezner also holds an MBA, with a concentration in Health Administration. Dr. Drezner is affiliated with the Reading Hospital and Medical Center's Department of Surgery (1996-present).

New Deficit-Reduction Plan Seeks $600B in Healthcare Cuts

The former co-chairmen of the president's bipartisan fiscal commission have introduced a deficit-reduction proposal that calls for an additional $2.4 trillion in savings over 10 years, with roughly a quarter of those savings—or about $600 billion—coming from Medicare and Medicaid. Erskine Bowles, former White House chief of staff in the Clinton administration, and former Sen. Alan Simpson (R-WY) unveiled the framework as a way to show lawmakers that a so-called grand bargain to lower the nation's ballooning deficit is still within reach.

To reduce spending in the nation's healthcare entitlement programs, the plan calls for reducing provider payments, changing cost-sharing rules, increasing premiums for higher earners, and “savings from lower drug costs and adjustments to account for an aging population,” according to additional information provided by the Moment of Truth project, for which the former leaders of the president's National Commission on Fiscal Responsibility and Reform serve as co-chairmen.

Query: Hyper Dosiflexion

A 57 year old retired fireman presented with heel pain. I noticed almost 30 degrees straight-leg dorsiflexion bilateral at the ankles. I didn't note extreme ROM at any other lower extremity joint. I know excess lengthening during a TAL causes problems. Any hints about what I should be watching for in this patient with natural hyper-dorsiflexion?

If we accept Dr. Shavelson's theories, then nearly all podiatrists and podiatry school biomechanics are wrong, and we have been treating patients unsuccessfully for a generation. Where are the biomechanic experts from our hallowed institutions to tell us who's right?  Or, do they also believe orthotic biomechanics is so much alchemy?

Initially, this discussion started with what appeared to be a case of idiopathic hallux varus. Now, it has progressed or digressed into a discussion on the basics of what I like to call BIOMAGIC. Root vs. Shavelson (FLEB). I do not claim to be a biomechanics expert or an innovative thinker. I do claim to have years of experience in surgery which requires a sound understanding of the way the foot works. What is successful in some does not work in others.

RE: Complaints Continue to Pile Up Against WI Podiatrist

I am as disgusted as anyone with cheaters and crooks. I may be under the wrong impression, but I learned in elementary school that an individual was innocent until proven guilty. The recent post in PM News (reprinted from Fox News) describes Dr. Lanham as having billed Medicare $10,000 for a first visit. It's hard to believe that anyone could justify a fee such as that unless they performed bilateral triple arthrodesis and an excision of a pheocromocytoma (one of my favorite diseases to pronounce). 

I have always said, somewhat in jest, that we should only have four fees, $10, $100, $1000, and $10,000. If we belong to an insurance panel or accept Medicare allowance, what we charge is moot. We are forced to accept their "usual and customary" pre-determined fees. Did this doctor charge his patient $10,000? Unlike Fox News, I don't know.

RE: TX Podiatrists Prevail Against Hospital Which Tried to Restrict Ankle Privileges

As you may have noticed, the February 5, 2013 publication of PM News included an article about the Hendrick Medical Center litigation. The PM News article correctly reported that the Texas Eleventh Court of Appeals upheld the trial court’s denial of Hendrick’s motion to dismiss because claims brought by plaintiffs TPMA, Dr. Cory Brown, and Dr. Martin V. Sloan are not healthcare liability claims under Chapter 74, Texas Civil Practices & Remedies Code. However, the PM News article also implied that the litigation was now over and that the Texas podiatrists had finally prevailed over the hospital which tried to restrict ankle privileges...which is not correct.

Instead, the Court of Appeals decision will now send the case back to the trial court for...
