APMWA Announces 39th Annual Student Writing Competition

The American Podiatric Medical Writers Association (APMWA) has announced its 39th Annual Student Writing Competition.

  • All papers MUST be non-technical in nature and original. (NOT AI generated) Appropriate subjects include practice management, ethics, law, education, healthcare, or any topic that would be suitable for a lay publication. Entries shou;d be submitted as a Word (.docx) file.
  • There is no word limitation. Papers will be graded for content, style, grammar, neatness, and overall impact.
  • First prize one thousand dollars ($1,000), Second prize seven hundred and fifty dollars ($750) Third prize five hundred dollars ($500) sponsored by an APMA Educational Foundation endowment from Dr. and Mrs. Steven Berlin, and recognition in the APMA NEWS and the APMWA Newsletter. MI Podiatrist Discusses Tarsal Coalitions
 
According to Conway McLean, DPM, "A tarsal coalition is a condition resulting in a problematic stiffness, is due to a failure of differentiation during development. Two bones that should ultimately be separate and distinct don’t become so, retaining an inappropriate connection. This is the essence of a coalition: an abnormal union. Without a high index of suspicion, it is frequently missed on regular x-rays, and the individual’s pain goes undiagnosed and untreated. Visualization of this bony bridge can be difficult, requiring a particular x-ray. And since the connection is often not bony but cartilaginous, x-rays may show nothing."

"Symptoms do vary person to person, as can the time of onset (i.e. when the pain becomes noticeable). Naturally, the treatment also differs and depends on multiple factors. Many live with this deformity and have no treatment whatsoever, but chances are good at some point they suffered from some musculoskeletal condition, which was the result of altered gait biomechanics. A prescription device can provide tremendous symptomatic relief while also serving to reduce stress to neighboring body parts. Surgery is an option and can be the best option for the right person at the right time, typically a younger person who hasn’t experienced the joint degradation of one who has lived with this situation for years. By removing the bony bridge, normal motion is frequently obtained," says Dr. McLean.

High Prevalence of Diabetic Retinopathy in an Outpatient Podiatry Clinic 
 
Diabetic retinopathy (DR) is a leading cause of vision loss among working-age adults. However, the prevalence of DR among patients with diabetic foot disease—a signal of advanced systemic diabetes complications—is underexplored. Additionally, the substantial co-morbidity burden associated with diabetic foot disease may result in a higher incidence of DR, or distinct barriers to ophthalmic care, including structural (access to healthcare), behavioral (prioritization of care), and economic (cost of care) factors, compounding risk of vision loss. This study assesses the prevalence of DR in a podiatric clinic while also investigating participant-reported barriers to routine ophthalmic follow-up.
 
The high prevalence of DR, especially undiagnosed DR, in conjunction with significant barriers to ophthalmic care highlights a critical need for improved screening in outpatient podiatric settings. Integrating digital fundus cameras into outpatient podiatric clinic workflow may enhance DR detection and prevent vision loss in this high-risk population. Addressing identified barriers to routine ophthalmic care may further improve the rate of follow-up care and reduce the burden of DR-related vision loss among patients with diabetic foot disease.

3 Ways Scheduling Automation is Improving Patient Access and the Bottom Line
 
2. Patient self-scheduling. Research suggests that patients overwhelmingly want self-scheduling options. Busy practices facing high staff turnover rates and high-volume calls find that some patients struggle to schedule an appointment. In these instances, patients are likely to seek out another provider.
 
To be well-positioned for the future, providers should offer multiple, integrated access points, including mobile options for patient scheduling, check-in, communication, and payment. Personalized, automated communication – such as online chat and text reminders tailored to patient preferences – keep patients engaged and simplifies the interactions with the practice.

I agree with Dr. Feinman. It does appear that today’s graduates look to lifestyle and convenience as their priorities. I remember when I was so thirsty for knowledge. The competition to get a residency, which only 50% of the class got was what caused us all to be very competitive. Now everyone is guaranteed a residency and so complacency seems to have taken hold with a good number of the graduates.
 
I have recently interviewed candidates for an associate position, and I can tell you most of them are just looking to work the bare minimum 40 hours a week with no intention or drive to work more hours or harder than the minimum required. Needless to say, I have been disappointed.

Point of Clarification: I’ve received comments about “what a podiatrist SHOULD be”. The intent of my post was not that I agree with the current podiatry residency training model, but rather how we should deal with the existing gap of traditional podiatric care IF the current residency model remains as is. These are two intertwined, but different topics.
 
Bottom line:  I would love to have podiatry satisfy all the general foot care needs (nail, callus, diabetic, etc.) of the population. I feel we have lost our identity as to what podiatry “SHOULD” be and residency program revision needs to be a topic of discussion at the table. However, if that’s not an option and it is truly a “bridge too far”, then we need to deal with the reality that IS, not the “SHOULD” be.

While Dr. Seuferling views a foot nurse issue as an opportunity not a threat, in Canada they will have a conference that will include these topics: MedFlex nail restoration, Onyfix nail correction, onychomycosis diagnostics and photodisinfection, SWIFT wart removal, proper footwear, padding and off-loading, pedorthic management, Vandenbos procedures, heel pain that could be something else, not plantar fasciitis, and "Rash: a review of dermatitis, infections, and suspicious skin lesions." 
 
Another nurse’s foot care association in Canada states, “our members have obtained the knowledge, skill, and judgement to competently provide medical foot and lower limb care… to determine which orthotic devices will improve one’s overall health and wellness. Our members have the option to consult with laboratory technicians who are qualified HCPs who specialize in the design and build of orthotic devices.”
 
It appears that they want to do more than trim toenails, reduce plantar calluses, and enucleate IPKs. If they’re doing this in Canada, my guess is they’re doing the same or soon will be performing SWIFT wart treatments, performing OnyFix nail correction, performing the Vandenbois procedures, and casting (or scanning) and dispensing orthotics.

Looking at this week’s survey, less than 13 percent of respondents so far would pick the current system of 4 years of podiatry school with a three-year residency. Nearly half would pick 4 years of DO training with a 4-year residency. Why are we contemplating why the application pool is so low for an option the majority of respondents (87%) would not take.  
 
If  a student came to me and asked if they should apply to podiatry school, even though I love my career, I would caution them to explore other options as well. If you graduate as a DPM and complete your residency, you are pigeonholed into podiatry forever. With an unlimited degree you have so many more options to pursue. And there is no longer a significant time differential with podiatry being a 7-year commitment.
