NY Podiatrist Discusses Neuromas
 
According to podiatrist Bruce Pinker, DPM, "Neuromas are inflammatory conditions of the nerves in between the toes. Commonly brought on by tight or narrow footwear that is worn repetitively, neuromas can also be addressed conservatively and surgically. Most patients elect for cortisone injections to start, usually performed in a series. If unsuccessful in providing relief, custom orthotics can also be utilized."

"Surgery is reserved for the more challenging cases, and usually results are long-lasting. Recovery from neuroma surgery is usually two to three weeks, performed as an outpatient procedure, and often covered by health insurance plans," says Dr. Pinker.

NY Podiatrist Appointed to Congressman's Seniors Advisory Group
 
Jean Chen-Vitulli, DPM, MS has been appointed to Congressman Sean Maloney’s Seniors Advisory Group. This group will meet on a regular basis to address the problems facing the senior population. They will discuss the issues, network, and find solutions regardless of political party affiliations. Seniors are often targeted for scams and face predicaments regarding quality of nursing homes, healthcare, and health issues.

Working with home bound patients, Dr. Chen-Vitulli reached out to her Congressman for issues she has observed in the field. The Congressional office offered assistance and support to her patients, as well as to veterans she distinguished as in need or facing homelessness. The advisory group's goals include finding ways to educate seniors to empower them, including finding funding and creating legislation to help with problems with an  emphasis on keeping people at home for as long as possible.

Village Podiatry Centers has announced that they have acquired Turner Podiatry and Wound Care in Royston, GA. Turner Podiatry and Wound Care was previously owned by William Barry Turner, DPM who passed away earlier this year. Dr. Turner`s practice focused on all podiatric conditions of the foot and ankle but specialized in wound care and limb salvage.

ACFAS Annual Conference Sets Attendance Record

 

History repeated itself once again! The largest number of foot and ankle surgeons from around the world took over New Orleans to help progress patient care at the 77th Anniversary Annual Scientific Conference of the American College of Foot and Ankle Surgeons.

With nearly 1,900 foot and ankle surgeons and a sold-out Exhibit Hall, the 77th educational meeting gave the opportunity for the profession of foot and ankle surgery to shine in New Orleans! ACFAS’ 78th Annual Scientific Conference will be held on February 19-22, 2020 in San Antonio, TX.

RELEVANT RESEARCH
Triamcinolone Compares Favorably to Verapamil for Treatment of Keloids: Study 
 
A randomized, single-blind, single-group comparison with 15 patients (30 scars) was carried out to compare the effects of intralesional triamcinolone with verapamil injections. Injections were scheduled every 3 weeks accompanied by cryotherapy until complete flattening of the scar or a maximum of 8 sessions, whichever came earlier. Scar evaluation at each stage was done by serial photographic records as well as by the Vancouver scar scale. 
 
Conclusion: Given the anti-inflammatory and antimitotic effects of triamcinolone plus its vasoconstrictor properties, triamcinolone would be an effective treatment for keloids in comparison to verapamil. However, further studies involving a higher number of participants with a longer period of observation are encouraged to shed more light on this subject.

If you look at the original x-rays, it appears that metatarsals two and three are adducted. Not surprisingly, the second and third toes are abducted since the toes tend to move in the opposite direction of the metatarsals. I think the post-operative problem is due to the fact that the surgeon decided to fixate the second metatarsal osteotomy. Had he or she left it unfixated, chances are that the metatarsal head would have shifted slightly laterally. This would have caused the second toe proximal phalanx to shift slightly medially and give a more congruent joint.
 
Personally, I would now simply perform an unfixated transverse...

All podiatric surgeons are podiatrists, not visa-versa. I did a podiatric surgical residency and certified by a board recognized by APMA and most hospitals (ABFAS). I also was a residency director of a PSR program for 5 years. I taught my residents that if they perform surgery as part of their practice, to refer to themselves as podiatric surgeons. Yes, you have to pay dues and recertify every 10 years...I do feel you should refer to yourself as podiatric surgeons. 
 
Most DPMs have hospital privileges under a department of surgery, not medicine. There should be no discredited feelings or embarrassment if you do not do surgery every day or just once or twice or month. It doesn’t matter. I know orthopedists who limit themselves primarily to sports medicine and do not operate most of the time. There are ENTs who do a lot clinic work and operate once a week. Be proud to call yourself a podiatric surgeon if you are certified and trained in it. My grandfather was a podiatrist and not a surgeon. That was fine with him and with me.

The American College of Surgeons recognizes 14 surgical specialties. Podiatry is not recognized as one such surgical specialty. The American Board of Medical Specialties (ABMS) does not recognize podiatry as a surgical specialty. The performance of foot and ankle surgery is variably a part of the practice of podiatry. For some, the performance of foot and ankle surgery is the sine qua non of practice. For others, it is of minor importance to daily practice.
 
Is podiatry a surgical specialty? No. Is surgery a specialty within podiatry? For some. For those who do not agree, I suggest that you attend the ACFAS annual scientific conference. Or read JFAS. Should you choose to do so, you will quickly learn that there are some whose practices are dedicated to surgical care of the foot and ankle.
