WI Podiatrist Discusses Causes or Foot Cramps
There are a few reasons your feet may be cramping up. When it comes to your feet specifically, the most likely culprit is ill-fitting shoes, says Alex Kor, DPM, a podiatrist at Froedtert & the Medical College of Wisconsin, and immediate past president of the American Academy of Podiatric Sports Medicine. As you exercise and your feet swell with the heat, ultra-tight shoes can compress the nerves on top of your foot. You can mitigate it temporarily by tying your shoes differently to relieve pressure on the top, says Kor.

In fact, tendinitis—either caused by poor footwear or other overuse events—is a prime culprit of foot cramping, Kor says. This inflammation in your tendons can cause not just pain but also potential swelling. But you won’t know if it’s a soft tissue condition, nerve issue, or bone problem unless you see a doc, Kor adds. Metabolic disorder is another potential cause of foot cramping—albeit a much less likely one—because low potassium and low sodium levels can cause cramps. But if this were the case, you'd suffer from cramps outside the gym as well, Kor adds.

SC Podiatrist Tells Students "The Sky's the Limit"
South Carolina State graduate Dr. Kevin Ray, an Orangeburg podiatrist, told students that the sky will be their limit when they earn degrees from the university. Ray is the owner of The Foot Institute.

"We don't produce haphazard students," Ray said. He told them, “If you are going to excel in entrepreneurship and business … it starts right here at South Carolina State University. I have done it." Ray is the founder of the R.A.Y. Foundation which co-sponsored the event.

Podiatrist Dr. J. Cliff Gilley joined Mango Medical this month in Waimea. He has visited Hawaii more than a dozen times, and the first time he and his wife stepped on Hawaii Island they fell in love with it and its people.

Dr. Gilley earned a doctorate of podiatric medicine in 2001 from the Barry University School of Podiatric Medicine in Miami. He completed his residency in foot and ankle surgery at Grant Medical Center in Columbus, Ohio in 2004, and most recently practiced in Knoxville. He is a member of the American Podiatry Medical Association, American College of Foot and Ankle Surgery, and Hawaii Podiatric Medical Association.

Query: Obligation to Treat Undocumented Immigrants
As healthcare providers, what is our fiduciary responsibility in the case that we become aware that a patient whom we care for is illegally in our country? Can we look the other way? Are we at any legal risk for their actions if we are aware that our patient is illegally in this country, choose to look the other way, then he/she commits some act of terrorism? Is this part of the doctor/patient protected information (HIPAA)?

Editor's Comment: PM News does not provide legal advice. There is no specific requirement to treat or not treat someone based on immigration status. One thing to note is that a podiatrist cannot, in most circumstances, report someone to the federal government or any other authority without violating patient confidentiality. One exception would be if the patient committed a crime in your office or you had reason to believe that any patient was planning a terrorist attack.
While there is no legal responsibilty to treat an undocumented immigrant, there is likely an ethical responsibility under the APMA Code of Ethics ME4.21 "The podiatrist shall not discriminate against any patient because of race, religion, ethnicity, gender, sexual orientation, disability, socioeconomic status, or health status."

Query: Ethical Dilemma
I was recently referred a 17 year old male who had been in a 4 wheeler accident 2 years prior, sustaining a compound fracture of his left lower leg. Over the last year, he has developed a symptomatic hallux hammertoe of the left foot. It appears that he needs a Jones teno-suspension with an arthrodesis of the left HIPJ. The patient has Amerigroup, and my reimbursement for this will be a grand total of $237. Considering my peri-operative time, along with the 90-day global, this will hardly be the $15 minimum wage. Here's the quandary: Perform the surgery? Decline to perform the procedure? How do my colleagues handle this "predicament", and what is your justification for your decision?

Query: Nitroglycerin Patches for Overuse injuries
Recently, a patient of mine went to his orthopedist for an Achilles tear and plantar fasciitis. His orthopedist gave him nitroglycerin patches to induce neovascularization. He is improving. Is anyone utilizing Nitro patches for tendon overuse injuries, and if so, what has your experience been?

I question the patient’s ambulatory status. If he is any sort of ambulatory state, the best option is a sural flap. It's based on the perforating peroneal artery which is the last of the trifurcation to be affected with DM and is often open even in people with PVD. This needs aggressive management and closure, not a trial of something without proven results.
