VA Podiatrist Assists Monks on Thousand Mile Walk
David Weiss, DPM was recently asked to provide podiatric medical support to the group of monks who undertook the extraordinary journey of walking from Texas to Washington, DC. Covering hundreds of miles over variable terrain with limited recovery time created a predictable cascade of lower extremity pathology: friction blisters with maceration risk, plantar fascial strain, Achilles tendinitis, forefoot capsulitis, and early metatarsal stress reactions. The cumulative load without structured rest cycles was the defining variable. Minor mechanical inefficiencies, when repeated daily, amplified quickly. "It was a humbling, amazing experience to witness both the physiologic limits of the lower extremity and the discipline required to push beyond them," said Dr. Weiss.

Management centered on prevention and maintaining forward progress — advanced blister care with selective drainage and hydrocolloid protection, strategic off-loading and taping, orthotic modifications rather than full device replacement, footwear counseling, and early identification of stress-related symptoms before progression. "The experience reinforced a simple truth we all appreciate: control shear and you prevent most complications. In high-mileage endeavors, success often hinges not on dramatic intervention, but on millimeters of padding, timely biomechanical support, and early clinical judgment," said Weiss.

Here are some valuable tips that have aided Savannah Santiago, DPM in navigating her experiences as a student and a resident:
3. Ask for Help
If there’s a procedure you haven’t done before—or one you haven’t performed a certain way— almost always ask an upper-level resident to walk you through it. In a recent cadaver lab, I had two third-year residents wearing their loupes alongside me as we practiced using micro instruments. I finally got to perform procedures I had only observed before.
Having two experienced sets of eyes on me was invaluable—they corrected my hand positioning, fine-tuned my loupe technique, and offered real-time feedback that immediately made me better. Another funny way I practice with my loupes... I paint my toenails wearing them and have found it has helped me practice with the magnification.

Query: Dispensing Shoes to Nursing Home Residents
As private practice podiatrists, are we allowed to dispense shoes to residents who live in nursing homes? We do have some that come to the office for treatment and would normally qualify. We no longer see patients in the nursing home, only office visits. Are there specific rules that differ from what we currently follow for office-based DME dispensing?

Response from Codinghelpline.com: If the patient is coming to your office from a SNF, you must be sure that they are not in the SNF under Part A Medicare (skilled nursing). If they are you will not be paid for a great deal of services, including DME, imaging studies (technical component, etc.) This is better known as Consolidated Billing.
Thus the first thing I would check is the patient's status and be sure they are in the SNF under Part B (non skilled) stay. The best way to check is not with your EHR as this information provided to all EHR often lags by 30 days. Check with the SNF practice administrator.
If the patient is under a Part B stay only, then, yes you can dispense therapeutic shoes to the patient in the SNF. The same rules would apply to them as applies to patients who reside in their own home.

Fran was one of my legendary attendings and I remember how the residents would contend with one another to cover his surgeries. It was never dull! The first case I scrubbed with him, it was just the two of us. It was a tibial nail, something not in the common cases we were used to back then. But for him it was just another day in the OR.
I remember that we had a great discussion about the literature that day. He taught me to have confidence if it was backed up by skill and knowledge. Wise words from a man who had all these qualities.

It was with sadness that I read of the passing of Dr. Zier. Whether he knew it or not, the man was my mentor. He was perhaps the perfect example of what the relationship between podiatry and the allopathic profession can and should look like.
He taught me what it means to be collegial, kind, and patient. His kindness is what I will always remember him for. His generous nature touched his students and his patients. I am forever grateful for his example of human compassion and dignity.
