PODIATRISTS IN THE NEWS
For Bunions, Size Doesn't Matter: NJ Podiatrist

According to Marco Ucciferri, DPM, a podiatrist at Summit Medical Group in New Jersey, bunions are either arthritic or genetic, and both kinds can cause mild, moderate, or severe pain.

Dr. Marco Ucciferri

Size does not matter when it comes to pain. “Your pain relates to the amount of pressure you put on the joint and the amount of walking you do regularly,” says Dr. Ucciferri. “You could have a huge bunion that doesn’t bother you, but if you wear dress shoes and walk to work every day, your small bunion could be very painful.”

PODIATRISTS AND DIABETES
Diabetics at Risk in Cold Weather: IL Podiatrist

For people living with diabetes, foot care is paramount. Minor issues can turn into serious infections which can lead to amputations. Diligent foot care is more important during winter when cold temperatures and slushy sidewalks make it more difficult to keep feet warm and dry. Frostbite and blistering of the toes may result, warned Dr. Marc Leonard, OSF HealthCare podiatrist.

Dr. Marc Leonard

Dr. Leonard recommends wearing shoes or boots that provide protection from the cold, snow, and ice. Daily foot inspections are vital to check for breaks in the skin, changes in color, or other abnormalities. Early detection makes treatment easier before issues become complicated. In addition, keep blood sugar levels in check and follow your healthcare provider's advice regarding nutrition, exercise, and medication, he said.

Dr. Barry Rosenblum

He is just the person behind the scenes that this Distinguished Service Award is designed to recognize, and I am proud to present it to him.”

CODINGLINE CORNER
Query: Debridement with Application of Graft

We performed a debridement of an ulcer and application of a graft. We billed CPT 15275 -RT and CPT 11043 -58, -RT, -59 for application of graft and debridement of ulcer, respectively. CPT 16275 was reimbursed at $96.07 and the EOB stated that CPT 11043 is bundled with the CPT 15275 code. Any advice on how to best bill for both codes?

Alex Gorenshtein, DPM, Winchester, VA

Response: When applying a skin substitute, the wound needs to be ready to accept the product. So if extensive debridement is required (CPT 1104X), then the wound bed may not be ready. Application of a skin substitute includes some wound prep. So billing CPT 15275 and CPT 11043 (further debridement) is in certain respects redundant. Select the code that most closely reflects the extent of the debridement performed.

In addition, you should document the condition of the wound bed, HgbA1C, presence of infection, vascular supply, etc. to validate that the use of the skin substitute is appropriate. The amount of money spent on these products is increasing so insurance carriers, especially Medicare, are scrutinizing these billings more closely.

RESPONSES/COMMENTS (OBITUARIES)
RE: The Passing of Robert Jacobs, DPM

Robert Jacobs, Age 88, passed away peacefully surrounded by his loving family on Feb. 15, 2019. He was born in Milwaukee and lost his father at an early age. He worked very hard to support himself through college and became an accomplished podiatrist, holding many important executive positions in various professional organizations. He used his professional skills over the years to help others through his volunteer work in Guatemala, with Repairers of the Breach, and many others.

RESPONSES/COMMENTS (CLINICAL)
From: Harold Koehler, DPM

If you remove the base, you may have some instability but the pain should resolve. It is equivalent to doing a Keller which generally yields a good result also. One thing you may consider is to add GraftJacket as a spacer in the joint and/or wrapped around the met head.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS)
From: Joan Schiller, DPM

Podiatrists already have privileges to prescribe opioids. Why is marijuana any different? It seems to me that this is an exclusionary decision.

YOU CAN'T MAKE THESE THINGS UP
CO Woman Pens Break-up Note to Amputated Foot

A Colorado woman who had part of her leg amputated in the fall after spending years in agonizing pain said she penned a “breakup” note to her limb before undergoing the 2.5-hour surgery. “It’s not me it’s you,” read one message Jordan Beckwith scrawled across her right foot. “I’m sure we’ve had some good times together, but it’s time we parted ways. I wish you all the best.

CO Woman Pens Break-up Note to Amputated Foot

The 27-year-old, who signed her note, “Your blood supplier,” reached the agonizing decision to amputate her right leg below the knee after years of pain stemming from a horse-riding accident at age 13, according to SWNS. Beckwith, of Colorado Springs, said she flipped over the horse’s shoulder and broke her tibia, with the shattered fragment ending up lodged in her leg.
