SVS, APMA and SVM Publish First-Ever Guidelines for Treating Diabetic Foot
The Society for Vascular Surgery, the American Podiatric Medical Association and the Society for Vascular Medicine collaboratively have published their first-ever set of clinical practice guidelines for treating the diabetic foot, based on a meta-analysis of the available literature. The guidelines, The Management of the Diabetic Foot, were developed after three years of study and were published online in late January and in print in the February 2016 edition of Journal for Vascular Surgery.

Podiatric contributors to the article were Lorraine Loretz, DPM, MSN, NP, Kathya M. Zinszer, DPM, MPH, Vickie R. Driver, DPM, MS, Robert Frykberg, DPM, MPH. The committee synthesized the points of view of the three different medical professions by researching one another's treatment protocols. Vascular surgeons and vascular medicine specialists read literature from podiatry and vice versa, for example, and all participants concentrated on infectious disease control and guidelines, a large component of the finished document.

IL Podiatrist Lectures in Costa Rica
Rockford native Dr. Philip W. Seeber recently lectured at Hospital CIMA San José in San José, Costa Rica. Seeber spoke about the treatment of torn Achilles tendons, correcting flat feet, and the use of local anesthetics.

Another Record-Breaking ACFAS Scientific Conference
The largest number ever of foot and ankle surgeon attendees from around the world took over Austin, Texas to discuss patient outcomes at the 74th Annual Scientific Conference of the American College of Foot and Ankle Surgeons. With over 1,750 foot and ankle surgeons and 140 exhibiting companies, the 74th gathering of the profession’s premier educational event made history as the largest-ever Annual Scientific Conference held by the College.

Neurogenx Opens Second NerveCenter in TN
The second Neurogenx NerveCenter in Tennessee opened Feb. 8 in Jackson, TN. One of only six NerveCenters across the country, the Jackson medical facility is a partnership between David Dowell, DPM, and Neurogenx, the creators of an innovative nerve treatment program that has helped physicians end neuropathy pain for hundreds of patients.

NerveCenters are staffed by specially trained clinicians, certified in the Neurogenx treatment protocol. The centers are independently owned and operated, but receive training, marketing, and business support from Neurogenx corporate headquarters. The treatment is non-surgical and non-narcotic. Query: Non-Responsive Plantar Fascial Pain
I have a patient that came into my clinic with classic plantar fascial pain symptoms. After injecting the area with steroid injection and providing pre-fab orthotics, he came back one week later with worse and more severe pain to the plantar and medial heel and foot. He has symptoms of allodynia and will not really allow me to touch his foot. I have treated him with immobilization and physical therapy with no improvement.
An MRI performed to rule out a suspected plantar fascial rupture shows thickening but no tears. He has a negative Tinel’s sign. There are no signs to suggest any sort of seronegative or seropositive arthropathy. I sent him to a pain management doctor who states that after a lumbar injection, there was no relief of the pain. He is not a diabetic. Radiculopathy has been ruled out. I do not suspect this person to be a drug seeker. Any suggestions for other conservative measures?

Query: Diabetic Therapeutic Shoe Bill
I was reading the the article regarding podiatric economics in the February issue of Podiatry Management, when I came across the section regarding diabetic shoes. When did the tide turn around that Paul Kesselman now states most claims "are processed without a hitch and are not subject to pre- or post-payment audits." I had stopped dispensing diabetic shoes due to oppressive audits and wondered what changed.

Query: Bulla - Open Wounds: ICD-10
My patient has complications from large bulla formation related to lymphedema producing open wounds on the anterior surface of each lower leg. This occurred within the last week. The patient is diabetic with acceptable glycemic levels. Would the appropriate code be found in the acute/traumatic S81- open wound lower leg codes or would this fall under the chronic ulcer category L97-? What is the ICD-10 definition of chronic? From my view, the wound is neither traumatic nor chronic.

Response: It sounds to me what you are describing are stasis ulcers. I believe your starting point for these ulcers is either ICD-10 I83.0- (varicose veins of lower extremities with ulcer) or I83.2- (varicose veins of lower extremities with both ulcer and inflammation). The hyphen (-) after both of those indicates more characters are necessary to complete the codes.
Both of those codes carry with them the instruction to use an additional code to identify severity of ulcer (L97-) so you would need the appropriate L97- code second. The word "chronic" is not always appropriate when we are directed to use it. Don't let it stop you from using the proper code even if you think your ulcer isn’t truly chronic. In ICD-10 language, a wound is something traumatic.

RE: Working Toward Parity though DPM Participation in MGMA Surveys

To assist our efforts in the ongoing quest for parity, our members need hard data in reliable industry sources reflecting the actual compensation of today’s podiatric physicians and surgeons relative to other medical professions. To this end, APMA has recently been encouraging its members to participate in the Medical Group Management Association (MGMA) Provider Compensation and Production Survey and Management and Staff Compensation Survey.
By participating in these surveys, participants achieve access to rich compensation data while also ensuring that the MGMA database contains the most up-to-date data reflecting the current education and training of today’s podiatrist and illuminating the full range of services we provide. Keep in mind that the medical industry is already accessing this data when making hiring and compensation decisions, and this profession has a clear interest in making sure they are getting it right.
We encourage all podiatrists to participate so that the data available to healthcare stakeholders and other decision-makers are reflective of the podiatric physicians and surgeons who contribute to the success of our profession. As to the various responses to this matter, it seems that our larger podiatric conferences (ACFAS/APMA/FPMA/NYSPMA/CPMA/Midwest, etc.) should have panels on this issue at their respective meetings for the practicing DPMs as well as our resident DPMs. There is a cadre of us who have been in orthopedic surgical practices for many years as well as those with few years of experience in these types of practices.
During my 23 years of active duty in the USAF, the DPMs were in the orthopedic clinics/operating rooms with MD/DO orthopedists every day. When I retired from the USAF in 2004, I joined an existing orthopedic practice with three orthopedic surgeons. To be honest, I only looked at this option because of...
