NY Podiatrist Publishes Article in Forensic Journal

Bryan B. Kagan, DPM has published an article titled "Hand-Foot Dominance and Foot Morphology: A Comparison of the Dominant Foot with Foot Morphology and Relationship to Handedness" in the peer-reviewed Journal of Forensic Identification. The Journal is the official publication of the International Association for Identification and is devoted to the publication of research and information about Forensic Identification in its various disciplines.

Dr. Kagan is a board certified podiatrist on staff at White Plains Hospital. He is a member of the American Podiatric Medical Association, the International Association for Identification and is on the executive board of the American Society of Forensic Podiatry. He currently resides in New Rochelle, New York.

APMWA Announces 27th Annual Student Writing Competition

The American Podiatric Medical Writers Association has announced its 27th Annual Student Writing Competition.
 

  • All papers MUST be non-technical in nature. Appropriate subjects include practice management, ethics, law, education, or any topic that would be suitable for a lay publication.
     
  • There is no word limitation. Papers will be graded for content, style, grammar, neatness, and overall impact.
     
  • First prize will be one thousand dollars ($1,000), sponsored by an APMA Educational Foundation endowment from Dr. and Mrs. Steven Berlin, and recognition in the APMA NEWS and the APMWA Newsletter. Honorable Mention Certificates may also be awarded.
     
  • This competition is open to ANY enrolled podiatric student.

Query: EMR and Shredding Paper Charts

If a practice is using an EMR, after scanning all previous notes and contents of the paper chart on an active patient, is it reasonable to shred the paper chart?

Editor's comment: PM News does not provide legal advice. Once a medical record is scanned to EHR, it is no longer necessary to maintain a paper record. This, however, assumes that you maintain at least one back-up copy of the record, either web-based or at a safe and secure off-site location. One of the lessons of Katrina and Sandy is that your office is not necessarily safe. In addition to floods, fires can also destroy office records. Additionally, digital data can become corrupted.

In a recent AMA survey, 58% of EMR users said that they plan to shred paper records once scanning is complete. 38% said that they plan to store paper files in on-site records rooms or off-site storage facilities.

I agree with Dr. Shavelson's "pointe" 100%. The FHL weakness is an EFFECT, not the CAUSE of poor ballet form. The lone exception is the "knuckling" that occurs in a first time en pointe dancer who begins training too soon (generally before age 12). This is BEFORE the FHL has had adequate strengthening time to allow for proper "en pointe" form.

RE: Etiology of Hallux Varus (Dennis Shavelson, DPM)

While I agree with post-op orthotic concepts with respect to hallux valgus correction maintenance long-term, and appreciate Dr. Shavelson's point of view, one is hard-pressed to agree that an orthotic provided within the initial 2-week post-op period has ANY positive impact on osteotomy alignment, as clearly implied in Dr. Shavelson's post.

On the contrary, I'd opine, the biomechanic reality is that NO orthotic is capable of stabilizing an unstable osteotomy site! So, if osteotomy stability was never achieved on the OR table, then...

We have been using customized educational videos playing on TVs with built-in DVD players for many years. We have one placed in each exam room. These videos were made with the combined efforts of Tony Tyler from Visual Media Concepts (800-815-3444) and Gary Ignotofsky from webpowervideo.com.

We have two videos and each are shown as a 45-minute continuous loop.  On each video, we have sections with all of the doctors in the practice speaking. Professional voice-overs are also used to explain and illustrate various foot conditions. We also have static pages with the products we have available in our office. We are currently in the process of making a third DVD. The DVDs are also integrated into our website. Our patients have found them to be very informative, and we have found them to be an excellent marketing tool.

RE: DOJ and HHS Announce Record-Breaking Healthcare Fraud Recoveries (Alan Meyerberg, DPM)

Unfortunately, there is some truth to this story. Get a subscription to the OIG's daily news bulletin and you can quickly add up the amount of real crime there is in healthcare. This is not being committed by the vast majority of hard-working physicians, home health agencies, DME suppliers, or other licensed practitioners. Rather, it is being conducted by those who have seized on a target which is far too easy to steal from. Until recently, OIG did not have the tools to recognize the ongoing crime until it had been completed.

It's a crime that Medicare, Medicaid, and private insurance companies have not seen the light of day in creating software which can track these individuals faster before they've paid out billions of dollars in fraudulent claims. Perhaps Federal and State OIG ought to prosecute insurance companies (Medicare, Medicaid and private) who don't prevent these hucksters from being paid in the first place.

RE: Lisfranc Injury: Easy to Miss, Hard to Get Over- OH Podiatrist

I suspect the 30% missed diagnosis quote Dr. Hardy referred to pertains to instances when Lisfranc injuries are initially evaluated in emergency room or urgent care settings by non-podiatrists. Pending the area one practices in, these are often where the initial evaluation of such injuries takes place.

I too, would be perplexed if members of our profession, surgically trained or not, missed them at such a high rate, though, because of a ligament's poor blood supply, one must appreciate that occasional grade 1 injuries can be missed due to the relatively low level inflammatory response associated with them. The history of the chief complaint (with respect to the mechanism of injury) and the physical exam (pertaining to where the patient's symptoms lie) should facilitate diagnosis, even with the lowest grade injury levels.
