Hyperhidrosis Can Cause Peeling of the Feet: IL Podiatrist
Hyperhidrosis, which causes excessive sweating, is a genetic condition brought on by overactivity in the sympathetic nerves responsible for your fight-or-flight response, explains Sidney Weiser, DPM, founder/clinical director of Quality Podiatry Group. This also controls the sweat glands in your hands, feet, and other areas. “Moisture and darkness cause fungus to grow on your feet, and fungus causes itching and peeling of your skin.”
|
|
Dr. Sidney Weiser |
To battle peeling skin caused by hyperhidrosis, Dr. Weiser suggests using anti-fungal prescription medications such as Lamasil cream or Loprox Gel 77%. “There are also systematic fungal medications such as Sporanox and OTC creams such as Lotrimin and Lotrisone.” If your peeling feet are caused by this condition, it’s important to thoroughly wash and dry them daily. “If you are wearing socks, you may want to change them multiple times a day to prevent moisture build-up.”

| PRACTICE MANAGEMENT TIP OF THE DAY | |
Coding Mistakes That Cause Practices to Lose Revenue
These are only five areas of improper coding that lead to lost revenue; however, focusing on this list should make a significant, positive difference for the financial and operational well-being of your medical practice.
2. Missing E/M Codes
Oftentimes, this is the result of incomplete charting, typically due to provider distraction. Charts without follow-up typically result in the claim being sent late or unbilled.

| CODINGLINE CORNER |
Query: New at the Wound Care
Our group has recently started working at a wound care clinic in town and this has brought us some coding scenario with skin substitutes that we have not had experience with before. When applying skin substitutes for chronic wounds, we generally use CPT 15004 and CPT 15275. However, it does not appear that billing these together is appropriate. Any guidance would be appreciated.
Codingline Member
Response: With the advent of these products, I provided evidence of their efficacy in front of many MACs with the question of the viability of the wound always on the minds of those representing the MAC. Others in wound care involved in similar discussions, also came to the same conclusion; that is, the body part had to be ready for the acceptance of the graft, otherwise the graft would be rejected. Can you imagine a third-party payer covering the heavy reimbursement for these products for patients with poor vascularity or high HgbA1C? A total waste of taxpayers' money. Most, if not all, agreed the preparation of the wound was included in the application. And so the current principle of the LCD and/or CPT and that any “tidying” up one has to do on the wound to have it ready for the CTP is included in the payment for the application of the CTP.
Certainly one should be doing everything possible to assure that the recipient site for the CTP is ready for its receipt. In my opinion, that starts prior to the date of application. This includes all the standard elements of good wound care. And on the DOS of CTP application, once again, one should be checking the wound to be sure it is in optimal condition for receipt of the CTP. Any tidying up of the wound on the date of application should be relatively minor and thus included in the application of the CTP. As far as which CPT code you select for the preparation of the wound site, that is going to be based on the procedure you actually performed.

RE: Diabetes and Eye Health: A Guide for Primary Care Clinicians
From: Leonard A. Levy, DPM
Why not a guide on diabetes and foot health? The American Diabetes Association (ADA) just published a guide, Focus on Diabetes, a three-part series on diabetes-related eye health education for primary care providers. Topics include Overview of Diabetes and Eye Health, Ocular Manifestations of Diabetes, and Supporting Patients and Fostering Collaborative Care.
Why is there not an effort by the podiatric medical profession to encourage the ADA to produce such a guide focusing on foot health in people with diabetes? As in the guide on eye health where each part is written by optometrists and an ophthalmologist, the guide on foot health should be written by podiatric physicians.
With a rapidly rising population of over 65-year-olds in the U.S. population and the corresponding rapid increase of those with type 2 diabetes, such a guide is needed. We desperately need to concentrate more on preventive care, especially but not limited to foot problems and their sequelae among people with diabetes.
Leonard A. Levy, DPM, MPH, Ft. Lauderdale, FL

From: Paul Kesselman, DPM
RTM is not remote temperature monitoring. I applaud the recent article in Podiatry Management about utilizing a mat to remotely measure a patient's plantar temperature and the study performed at the VA. However, the use of the acronym RTM mentioned for remote temperature monitoring is certainly bound to cause confusion. RTM more accurately should be reserved for remote therapeutic monitoring, which will give the provider the ability to monitor their patient's response to therapeutic modalities. RPM (remote physiologic monitoring) actually measures physiologic parameters such as temperature, which is exactly what the authors were aptly identifying.
It is thus important for readers to understand that the future use of RTM should be reserved for therapeutic monitoring.
Disclosure: I am a consultant for Siren Care.
