DMU Clinic Expands Custom Arch Support Services
People with conditions like plantar fasciitis, flat feet, foot injuries, and other foot and ankle issues now have a new option for custom arch-support care. Starting March 4, Jarrod Smith, DPM will offer personalized arch support services at the Des Moines University (DMU) Clinic in the RecPlex.

"By offering arch-support services in the RecPlex, we're making expert care more convenient for area residents," says Kevin Smith, DPM, PhD, Dean of DMU's College of Podiatric Medicine and Surgery and a 1995 DMU alumnus. CO Podiatrist is the Only Podiatrist in a 200-Mile Radius
A foot injury she sustained while training for the Boston Marathon changed the trajectory of Jamie Mieras, DPM’s career. “When I found a podiatrist to help me, I was just in awe of what he could do,” Dr. Mieras remembered. “I was so impressed. I said, ‘Is this what you do all day?’ I decided right then and there that this was a good career path for me.”

Dr. Mieras attended Midwestern University in Arizona. She did a clinical internship in Phoenix and San Francisco before heading to Scripps San Diego for her residency. She worked in Sacramento where she treated a lot of foot and ankle trauma. Then she came back to Colorado and practiced for a year in Boulder before practicing rural health medicine in California. She is the only podiatrist in a 200-mile radius.

NJ Podiatrist Pleads Guilty to Healthcare Fraud Conspiracy
An Amtrak employee in New Jersey has become the latest to plead guilty to participation in a healthcare fraud scheme that led more than $11 million in fraudulent claims against the passenger carrier’s insurance. Podiatrist Michael DeNicola has also pleaded guilty to a conspiracy charge; his sentencing is pending.

There seem to be two recurring and interconnected questions on PM News lately. First, why is enrollment in colleges of podiatric medicine declining and secondly, why is there a push to increase the scope of practice to include the prescribing of systemic medications that do not directly affect the foot? Surprising as it may sound, the two may be closely associated.
When pre-med major students, not pre-podiatry majors, came to Des Moines to interview for the podiatry program, we stressed the fact that if they came to Des Moines, they would be completely integrated with DO students for their classes. Same lectures, same teachers, same time, same room, same exams. They could team up with DO students for study groups and had identical printed class notes approved by the lecturer and supplied to both DO and DPM students by the note service before...

Recommendation - please check with your state board of nursing to confirm whether or not this practice by an RN is within the scope of their license. Can they practice unsupervised? In their website under “what to expect”, they talk about 3-D imaging for orthoses. In Texas, there is a law specifying who may prescribe orthoses. Check your state “orthotic” law if you have one.
Are RNs allowed to prescribe orthoses in your state? Are nurses allowed to practice unsupervised to provide diabetic foot exams and diagnoses? Finally, are the American Foot Care Nurses billing insurance or is it strictly fee-for-service? Please do not just complain. Investigate what the state law is regarding nurse (RN) practice acts and scope of practice.

Dr. DiResta appears to be somewhat concerned that a podiatrist is serving as an instructor in foot care for nurses. I wonder if orthopedic surgeons express the same concern to their colleagues who participate in podiatric medical and surgical education. After all, by doing so, you are aiding and abetting the competition (enemy), are you not? At the NYSPMA meeting this year, there were interactive panels with orthopedic surgeons, and a program in which dermatologists were instructing in the evaluation and treatment of foot and ankle dermatologic disorders. Should they have refused to do so over concern for decreased patient revenue?
Some years ago, Dr. James Ganley asked me if I would be comfortable teaching family medicine residents how to manage ingrown toenails and common foot problems. He told me that he regularly instructed non-podiatrists in the evaluation and treatment of common foot disorders. His reasoning made sense to me. First of all, he told me that...

Dr. Jacobs alleges that what I wrote implies "that it is near malpractice" to not obtain advanced imaging or ultrasound before surgical excision. In no way did I imply that it was malpractice. My point was to relay that the discovery that advanced imaging was NOT performed in these cases may open up the treating DPM to litigation - regardless of the evidence-based literature.
I take offense to the fact that Dr. Jacobs references scientific studies with low power and low populations, and justifies his remarks as "established evidence-based" medicine, and implies that I am not adhering to the standard of care, and then alleges that I am giving plaintiff attorneys ammunition for ideas that blatantly were already there before my comments.
It is not Dr. Jacobs' right to assert how one provider sees a pathology and decides to treat one way versus another. To assert that my personal views of space-occupying mass lesions and the need for additional imaging is based on my diagnostic needs that is not the standard of care is not for him to decide my patient's comfort and need before electing to have a surgical or non-surgical decision. To assert that I am not adhering to the standard of care is arrogant, but also insulting. Like I said, an act of prudence and maybe less arrogance is required for our profession, considering the potential harm a missed diagnosis and subsequent litigation can hurt all of us.
