CA Podiatrist Publishes First Successful “Smart Boot” Diabetic Wound Study
As part of its partnership with Center to Stream Healthcare in Place (C2SHIP), Keck Medicine of USC has developed a new, interactive smart boot to help diabetic patients recover from dangerous foot wounds. “The first phase of the boot project was just to protect the wound,” explained David Armstrong, PhD, DPM, director of C2SHIP and professor of surgery at the Keck School of Medicine of USC. “If we can take off the pressure, we can heal the wound.”

The boots worked, but the team had a problem: Patient compliance with wearing the boot was hovering at around 28%. That led to phase two: a boot that patients couldn’t remove. Patients healed more quickly, but it wasn’t ideal. In time, phase three emerged: a more sophisticated wearable. Driven by cloud computing, the new model allows patients to put on or remove the boot whenever they wish. To further increase compliance, the boot gives feedback to the patient about their activity level and the amount of time they spend with the boot on. This “smart boot” also sends data directly to the care team.

Coding Mistakes That Cause Practices to Lose Revenue
These are only five areas of improper coding that lead to lost revenue; however, focusing on this list should make a significant, positive difference for the financial and operational well-being of your medical practice.
1. Inappropriate E/M Coding - Coding for evaluation/management services is often either too aggressive or too passive, and these coding errors are largely attributed to misinterpretation of E/M coding guidelines and the frantic pace of the clinical environment. Aggressive coding occurs when there isn’t proper documentation to prove what was done. On the other hand, passive coding doesn’t take the entirety of the work performed into account.

Query: Replacement Device Challenges
We had a patient who was dispensed an AFO about 4 1/2 years ago; this device broke and he was having difficulty walking without it. We dispersed a new AFO and it was denied as the original wasn’t 5 years old. We had a very long phone conversation with Medicare along with the patient and his attorney. Unfortunately, Medicare wouldn’t budge as they said he should have gone back to the provider of the original brace, even though it was in another state.
We did have an ABN but the patient said that he’s not going to pay for the replacement. We are working with him now to at least get our lab costs. Unfortunately, appeals don’t always work. We requested a peer-to-peer, but after several months we have not heard anything as they are too backlogged! I suspect that there are other stories like this out there. I wish we could just tell patients what it costs and they hand over their credit cards.

Response: You answered your own question: “I wish we could just tell patients what it costs and they hand over their credit cards.” The ABN indicates that you told them the brace would not be covered and the cost. They can then choose to use the old brace and use duct tape or they buy a new one. With the ABN, you can collect your fee upfront. You can bill Medicare as a courtesy to the patient. If Medicare were to pay, you could refund the money.
Remember, “the same and similar” is a CMS rule, not some arbitrary office policy you created. Your hands are tied. You have nothing to feel guilty about. It always amazes me that a person will shell out big time, cash money up front for veterinarian costs yet balk at paying co-pays and deductibles in our offices. Inform the patient that they can take this issue up with their congressperson as that person could influence CMS policies.
