IN Podiatrist Discusses Kansas City Quarter Back's Turf Toe
 
Patrick Mahomes' injury report for the remainder of the post-season included his well-documented concussion but also a left turf toe injury he had suffered earlier in the same playoff victory over Cleveland. Mahomes recovered from the concussion. No such luck with the turf toe. On Wednesday, he had the injury surgically repaired, which is rarely a necessity, perhaps because the diagnosis is so generic, synonymous with the mechanism of the injury rather than severity of the tissue actually damaged.

Usually, the damage is not severe enough to require victims to go under the knife, according to Rob Clemency, DPM, a podiatrist with Community Care Network in Munster and Schererville. “They typically do well with conservative treatment,” he said. Prior to arriving in Northwest Indiana, Clemency was the podiatrist for the Notre Dame athletic department. “People fail to appreciate, when you are talking about professional athletes, you are talking about a difference in milliseconds,” he said in reference to success or failure. “Little things matter a great deal, not only physically but mentally.” Upon recovery from the surgery, which usually requires 12-16 weeks, Dr. Clemency expects Mahomes to have recovered those milliseconds and his confidence.

Prince Harry's Bunions May be Result of High Impact Training: UK Podiatrist
 
Prince Harry may be suffering from bunions as a result of his high impact training and action in the Army, a podiatrist has claimed. Dina Gohil, who runs the practice DG Podiatrist and is an ambassador for CCS Footcare, told FEMAIL that while he boasts a "good arch profile," there are signs he could have a mild bunion deformity.

His feet seem to appear healthy and there is a good arch profile visible. However potentially he may have mild bunion deformity, whereby there is a medial deviation of the joint attached to the big toe, making the bony appearance prominent. "In his case, a likely cause of this condition is the high impact training and action he underwent while in the Armed Forces."

2021 Remote Patient Monitoring Changes:  Key Takeaways - Part 10
 
In December, the Centers for Medicare & Medicaid Services (CMS) released its 2021 Medicare physician fee schedule final rule. Within the final rule were some significant changes concerning remote patient/physiological monitoring (RPM) and the provision of other remote services. Here are the most significant takeaways from the final rule.
 
10. Approved Billing CPT 99091 Concurrently with Newer RPM Codes
 
Up until the 2021 final rule was released, it was widely believed that it was not permissible to bill the old (i.e., original) provider-specific CPT 99091 used for RPM at the same time as 99457, which went into effect in 2019. But the final rule seems to essentially reverse this interpretation, with CMS now permitting providers to bill for "complex" RPM management when the provider must spend significant time managing the patient and their RPM care plan.
 
Further, CMS noted that 99091 can be billed each 30 days whenever such complex provider management occurs without affecting the practice's ability to bill clinical staff time via CPT 99457 each calendar month.

Query: Problems With L Codes
 
I practice in Virginia and we are experiencing denials for L codes. We are being told that patients are only allowed one L code per every 5 years! It does not have to be from our practice. Has anyone had any issues with this and is it true?

Response: We have been up against this 'Same and Similar" issue now for 2 years and it is not going away. The RACs are denying L1900-L4631 DME as are both Noridan and MyCGS along with SMRCs too! Even if we provide what we feel is a medically necessary AFO, it is possible they have received any device L1900-L4631 within the past 5 years known as “RUL” (Reasonable Useful Lifetime). For Medicare, payment can be made for replacement of DME that is lost, stolen, irreparably damaged, or has been in continuous use for the equipment’s RUL. It is important that your staff does check with all 4 DME MAC’s portals, so both JA/JD at Noridian and MyCGS JB/JC for Same and Similar.
 
When looking at the reasons on the surface, they seem you can get at least one of these “lost, stolen, irreparably damaged” but the DME MACs make it very hard to get to one of these three. What we have found works best is a new condition that makes the new device medically necessary and explains why the previous device is no longer appropriate. Can you document that the old device is no longer medically necessary, or harming the patient? You will have to appeal these denials so be prepared to do so. Make sure your patient has signed their proof of delivery and have them sign the ABN stating a reason why the same and similar issue may preclude Medicare from paying for the AFO you are dispensing.

In response to the question about treatment for Raynaud’s, if my patient exhibited fixed coloration changes in the digits, I routinely prescribed Nitropaste in the ointment form and had it applied proximal to the affected area. The application was done using an applicator and was done up to three times a day. I instructed the patient to surround the area, meaning if the coloration change was in a distal digit, the circumference of the base of the digit was subjected to the application.

podiatrists are trained in residency training programs in the basic concepts of wound healing.

For High arch feet: Use standard arch fill, deep heel cup, medial very skive, 4 degrees of varus heel post.
 
For Low arch feet: use low arch fill, deep heel cup, medial very skive, vertical heel post, overlap like a medial arch area extension of only the top cover over the device giving extra padding to the medial arch in the shoe. 
 
The patient should be plaster-casted NWB with locking in the mid-tarsal joint position.
