AT THE COLLEGES
WesternU Students Practice Critical Skills on Digital Cadavers

As a growing number of medical schools bring virtual reality into the classroom, students are finding it an effective way to learn complex subject matter, such as anatomy, that’s often easier to understand with hands-on practice. VR and similar technologies, including augmented reality, provide a “very effective way to teach anatomy in particular,” says Dr. Robert Hasel, associate dean of simulation, immersion, and digital learning at Western’s dental school.

Dr. Robert Hasel

Because most medical schools don’t have access to real cadavers, he notes, historically students have learned from photographs, drawings, and lengthy lectures. That’s not how things work at Western’s Virtual Reality Learning Center, where students use tools like the Anatomage Table. This virtual dissection table and its digital cadaver let students learn anatomy from every possible angle. “When they come in here, it’s entirely hands-on,” Hasel says. “They’re working in small groups, peer-to-peer, solving problems and moving from station to station. They’re so heavily engaged, it can be hard to get them to leave.”

PODIATRISTS AND SPORTS MEDICINE
Wearing a Sport-Specific Athletic Shoe Can Prevent Injuries: NY Podiatrist

You shouldn't be wearing running shoes to Zumba or basketball sneakers for hiking. "There is so much technology that goes into sneakers, especially for workouts," says New York podiatrist Dr. Jacqueline Sutera. "Wearing shoes that are inappropriate for your specific workout could cause injury."

Dr. Jacqueline Sutera

For example, running and walking sneakers are made for forward motion and have the maximum amount of cushioning and shock absorption, according to Dr. Sutera. Sports like basketball and tennis require shoes that provide lateral or side-to-side support. Cross-training shoes can be used for lower impact exercises like weight lifting, rowing, elliptical, and Zumba.

ACA UPDATE
Quiet IRS Change Could Affect Obamacare

Following an executive order from President Trump to “minimize the economic burden” of the Affordable Care Act, the Internal Revenue Service said it is backtracking on its plan to reject 2016 tax returns that do not indicate whether the taxpayer complied with the Act’s individual mandate. Supporters of the act, which is known as Obamacare, fear this behind-the-scenes change could undermine enrollment in health insurance.

The mandate requires Americans to have a minimum level of health insurance, qualify for one of myriad exemptions, or pay a penalty known as the “individual shared responsibility payment” when they file their return. Taxpayers must indicate on their tax returns whether each person included on the return had insurance for the full year, qualified for an exemption, or owed the penalty. Returns that don’t provide this information are known as silent.

PRACTICE MANAGEMENT TIP OF THE DAY
Healthcare Fraud 101: An Overview of the Key Statutes - Part 4

Criminal Statutes

The government may also bring a criminal case against you or your practice for violating certain statutes. Generally, these statutes require some level of intent (i.e., that you knew your conduct was wrongful), making it more difficult for the government to prevail. That said, the possible penalties are life-changing: conviction of a felony and prison time.

2. Criminal healthcare fraud statute. Under this statute (18 U.S.C. § 1347), a person can be held liable for a scheme to intentionally (1) defraud any healthcare benefit program or (2) use false statements to obtain funds held by a federal healthcare program. The penalties are steep: up to 10 years in prison and a fine up to $500,000 or twice the amount of the fraud.

RESPONSES/COMMENTS (CLINICAL)
From: Matthew Andrews DPM

Even with a negative MRI or bone scan, drilling into the calcaneus in a vasculopath with severe anemia and large tissue defect is not only likely to end in amputation, it is assuredly indefensible. It appears the wound would benefit from HBO therapy, a thorough debridement with Versajet or a sonic debrider, and application of a tissue substitute to encourage epithelialization. For larger defects like this, we have seen excellent results using Stravix with and without use of a wound VAC. This assumes, of course, that the patient's anemia is addressed and they are medically-optimized (nutrition, glycemic control, off-loading, etc.).

RESPONSES/COMMENTS (PRACTICE MANAGEMENT TIPS) - PART 1A
From: Jeffrey Kass, DPM

Dr. Udell - the problem is worse than you describe. When the patient goes to Hanger, unlike you, they will be reimbursed at a fair rate. They will profit.

RESPONSES/COMMENTS (PRACTICE MANAGEMENT TIPS) - PART 1B
From: Ira Baum, DPM

My policy was simple and very effective. All patients who were recommended orthotics were informed that we do not accept insurance as payment for orthotics. We had them sign a detailed waiver and leave a 50% non-refundable deposit.

I informed patients that if they want their insurance company to pay for their orthotics, I would reimburse them when the payment was received, but not more than they paid me. If that was unsatisfactory, I politely told them to find another podiatrist because insurance reimbursement did not cover my costs. It was also my policy never to write an Rx for orthotics. What I would provide is the "need" for orthotics, but not the Rx. Why would I take responsibility for the quality of a lab I didn't know and have patients come back and complain and waste my time?
