IN Podiatrist Writes Memoir of Loss of Daughter to Drugs

Wanting to prevent others from enduring the excruciating agony of watching their son or daughter succumb to drug or alcohol abuse, Mann Spitler, III, DPM wrote Manda’s Beast: A True Life Addiction Story to Help Parents Protect Their Sons and Daughters from Self-Abuse with Drugs
Dr. Mann Spitler, III

In a heart-wrenching memoir, he shares Manda’s haunting journey into the “beast” of heroin addiction, culminating in her tragic death at age 20 from a self-administered lethal dose. This guide was lovingly crafted with kids and teens in mind, with the preventive measures that he and his wife Phyllis wish they had taken.

PODIATRISTS IN THE COMMUNITY
WY Podiatrist Finds His True and Rewarding Calling

There is one particular medical practice that seems to go relatively overlooked, despite the fact that we as human beings use this particular appendage constantly every day. Torrington resident and local physician, Austin Matthews, DPM just so happens to practice that one particular thing.

Dr. Austin Matthews

Dr. Matthews has been practicing podiatry for years now, and by his own account has come to view his practice as not just a job but a true and rewarding calling in life. “I deal with pretty much any problem that involves the foot or the ankle,” Dr. Matthews said with a grin. “Having pain in your feet or ankles is not normal, and oftentimes there are plenty of options to get you feeling better.”

FDA NEWS
FDA Approves First Medication to Treat Severe Frostbite

The U.S. Food and Drug Administration has approved Aurlumyn (iloprost) injection to treat severe frostbite in adults to reduce the risk of finger or toe amputation. Iloprost’s efficacy in treating severe frostbite was primarily established in an open-label, controlled trial. The most common side-effects of Aurlumyn include headache, flushing, heart palpitations, fast heart rate, nausea, vomiting, dizziness, and hypotension (blood pressure that is too low). Aurlumyn also has a warning and precaution noting that it may cause symptomatic hypotension.

Aurlumyn received Priority Review and Orphan Drug designations for this indication. Iloprost was originally approved in 2004 for the treatment of pulmonary arterial hypertension. The FDA granted the approval of Aurlumyn to Actelion Pharmaceuticals US, Inc.

MEDICAL ECONOMCS
Orthopedic Surgeons on Track to Earn Less Than Taco Bell Workers

Declining reimbursements to surgeons under Medicare and private insurance for total joint replacement procedures are creating an untenable situation, researchers warned. Already, according to a study presented here at the American Academy of Orthopaedic Surgeons' (AAOS) annual meeting, hourly rates for early-career orthopedic surgeons performing hip replacements are about the same as for dentists ($89) and travel nurses ($88).

If those numbers aren't disappointing enough for young surgeons who've already spent a dozen years in training, the same group fit a third-order polynomial model to historical data for Medicare reimbursements for arthroplasties, yielding a projection out to 2030. Actual figures from 2007 to 2023 showed mean reimbursements falling over time, and falling faster with each passing year from 2019 onward. And the same group projected that Medicare payments to physicians for hip replacement surgeries would drop below $11 per hour if the current downward-accelerating trend continues.

PRACTICE MANAGEMENT TIP OF THE DAY
7 Ways Telehealth Can Help Your Practice

The healthcare industry is witnessing a remarkable transformation in the way medical care is delivered. Telehealth, a concept once considered futuristic, has now become an integral part of the healthcare landscape, in large part due to its wide use and adoption during the pandemic. Despite its revolutionizing impact on patient access and streamlining practice operations, telehealth remains under-used.

2. Streamlined Workflow

By incorporating telehealth into their practice, doctors can optimize their schedule, reducing wait times for patients during in-person visits. This efficiency benefits both patients and staff, enhancing overall practice productivity and patient satisfaction.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
From: Richard Stess, DPM

Dr. Robert Creighton's email regarding "Laying on of Hands..." is particularly appropriate in these days of advancing technologies. 20 years ago, when my associate and I developed the STS casting sock, it was designed to improve office efficiency while maintaining accuracy without compromising the techniques designed by Merton Root, DPM.  3D scanning of the foot can capture the anatomy of the foot but often positioning of the foot appears to be neglected.  Ads regularly appearing in podiatric publications often depict 3D scanners while clearly sacrificing any consideration for accurate positioning.  A retail employee theoretically can scan a foot with no knowledge of foot function or foot biomechanics.

Podiatric physicians possess the knowledge to provide accurate functional orthotic devices and thereby distinguishing themselves from non-professionals. The principles of casting that were taught to us 50 years ago continue to produce successful clinical outcomes. Patients react positively to a podiatrist or his well-trained assistants when they apply casting material with care being taken to accurately position the STJ, mid tarsal, and first ray. The added therapeutic benefit of "laying on of hands" contributes to both patient satisfaction and successful outcomes.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
From: Ivar E. Roth DPM, MPH

I think you could sue for failure to refer to an appropriate medical doctor. If a generalist cannot figure out the problem, there is a duty to refer to a specialist.

RESPONSES/COMMENTS (PODIATRISTS AND SPORTS MEDICINE)
From: Doug Richie, DPM

Regarding the advice about prevention of pickleball injuries given by Dr. Udell, I have two questions for him:
1. How do you warm up after playing?
2. What is a "twisted muscle"?

As a Fellow of the American Academy of Podiatric Sports Medicine and active pickleball player myself, I would love to know the answers to these questions.
