OH Podiatrist Discusses Treatments for Plantar Fasciitis 
 
"Plantar fasciitis is a common foot ailment that involves microtrauma to the plantar fascia structure on the bottom of the foot," says Mark J. Mendeszoon, DPM, podiatrist at Precision Orthopaedic Specialties in Ohio. The most common treatment options are to avoid going barefoot and to wear appropriate and supportive shoes or sneakers. This includes stretching the Achilles tendon, as the majority of people with plantar fasciitis have a tight Achilles, notes Dr. Mendeszoon.

A podiatrist or specialist may also do special taping on the foot, or suggest over-the-counter insoles or custom orthotic devices to provide support to the arch. And treatments using physical therapy modalities including ultrasound, electric stimulation, Graston techniques (a manual scrapping of the tissue with a metal tool), massage therapy, dry needling, cupping, and even laser treatment are also options, according to Dr. Mendeszoon.

OhioHealth has announced plans to expand its Pickerington Medical Campus, which was opened in 2015. “From the moment we broke ground, we knew the Pickerington Medical Campus was only the beginning of our commitment to Pickerington and nearby communities,” said Kevin Lutz, DPM, president, OhioHealth Grove City Methodist Hospital and accountable executive for the Westerville and Pickerington Medical Campuses and Freestanding Emergency Department Network.

COVID-19 Kept FL Podiatrist Out of Prison Until Now
 
In the 11 months since Palm Springs North podiatrist Richard Mallia pleaded guilty to his role in a $3.4 million Medicare fraud, the COVID-19 pandemic has put millions of Americans out of work. Meanwhile, Mallia has kept his license and COVID has kept him out of prison. Now, U.S. District Court Judge Cecilia Altonaga has ruled Mallia had to report to Federal Prison Camp in Pensacola by Feb. 26 to serve five years and three months.

2021 Remote Patient Monitoring Changes:  Key Takeaways - Part 9
 
In December, the Centers for Medicare & Medicaid Services (CMS) released its 2021 Medicare physician fee schedule final rule. Within the final rule were some significant changes concerning remote patient/physiological monitoring (RPM) and the provision of other remote services. Here are the most significant takeaways from the final rule.
 
9.  Clarification on the two measurement-day waiver for suspected/diagnosed COVID-19 patients.
 
One major point of confusion was the current waiver on the 16 measurement-day rule for CPT 99454 that allows billing with only two measurement-days when the patient is suspected to have or has been diagnosed with COVID-19. A fact sheet issued by CMS concerning the final rule reiterated the current existence of this waiver and confirmed that it will expire once the public health emergency ends. The two measurement-day waiver only applies—as the waiver itself states—to patients who have a suspected or confirmed cases of COVID-19.

An important reminder to anyone who received an EIDL loan from the SBA: You cannot take distributions from your business or pay bonuses to yourself or your staff for the entire time you have this loan money. It is in the promissory note in very clear terminology. If you need this money, it is a wonderful lifeline to have. If you find you don't really need it, it is better to pay all of it back so you don't get penalized. This does not apply to the HSA grant or the PPP (if you qualify for forgiveness.)

Here are my pearls for this discussion.
 
A good deal of pronation is retrograde, i.e., forefoot to rearfoot. Using various sized 1st ray cut-outs works wonders and permits a reduction in amount of rearfoot posting required.
 
The length of the orthotic shell is of paramount importance. Making sure that it ends PROXIMAL to the metatarsal heads improves the comfort of the shell.
 
The addition of fascia grooves makes the device far more comfortable, particularly in cavus feet.
 
Don't cast patients the day they present. Their feet are invariably swollen so the accuracy of the fit will be compromised if casting is performed too soon. Consider some type of temporary orthotic or several taping visits to reduce swelling prior to impression casting.

The biggest thing that has helped me is to know that there are both intrinsic and extrinsic pronatory factors. Intrinsic factors are varus/valgus influences in the foot and functional hallux limitus, while extrinsic factors are all the factors outside of the foot. This includes varus/valgus influences above the foot; functional and structural leg lengths; and equinus.
 
Learn how to evaluate these factors and apply these to your orthoses.

Here are some of my ideas. I believe this is a great subject, so others should feel welcome to respond.
 
1) Do not order one set of "compromised" custom orthotics that will work in all varieties of shoe styles for your patients. You will end up with an orthotic that still doesn't fit in some shoes and also doesn't function well enough in the shoes they do fit into. On those patients who will require different types of orthotics because they wear different shoe styles, make one set for the most important shoes and wait a bit and then make other sets at a different date when you see how your patient responds to the first ones.
 
2) Never, never, never make multiple sets of orthotics at the...

I read about the parity (“equality, equivalence" according to Webster) issue before I began my podiatry career as a student at NYCPM in 1975, practiced from 1979 through October 2015, and I am STILL reading, 46 years later, about whether podiatry should remain a separate profession or be absorbed into allopathic medicine. Dr. Kornfeld said it very plainly today. What prospective medical student, given the option of attending a school that will give him/her limited licensure and reduced reimbursement in all states, versus attending a school that has none of those limitations, would intentionally choose the former? The profession will indeed die if it does not change with the times.  
 
For the most part, I enjoyed my time in practice, but the increased costs and frustrations of administration and government oversight were simply too much for me, so I walked away. I am hopeful that those who have remained are finding their practices fulfilling and profitable. But it is the ultimate in head-in-the-sand thinking that future students will find it so without significant change, and that means an MD/DO designation.
