AT THE COLLEGES
TUSPM Student Receives 1st Marc A. Karpo Endowed Memorial Scholarship

Fourth-year student-doctor John Martucci was the first recipient of Temple University School of Podiatric Medicine’s (TUSPM) Dr. Marc A. Karpo Endowed Memorial Scholarship. He was awarded a check for $2,000 from Mrs. Bonnie Karpo on February 13. Martucci received the scholarship because he is in the top percentile of his class, the Class of 2018 President, and had a close relationship with Dr. Karpo.

Mrs. Bonnie Karpo and John Martucci

The scholarship was set up in honor of Dr. Karpo, who passed away suddenly in 2016. Dr. Karpo’s pride and joy were his students at TUSPM, and this scholarship is a fitting memorial that will be awarded annually to deserving students in the years to come.

APMA STATE COMPONENT NEWS
BCPMA Forms Podiatric Residents Committee

The Broward County Podiatric Medical Association (BCPMA), in an effort to address the concerns and needs of our future podiatric practitioners, recently formed a new committee made up of podiatric residents from local hospitals. The committee was formed to engage residents early on and to offer them all the resources and advantages of membership in the Florida Podiatric Medical Association (FPMA) as well as the American Podiatric Medical Association.

Residents from Broward County division of FPMA with guest speakers Drs. Hal Ornstein, Nooshin Zolfaghari, Terence McDonald, and Karen Lee.

More importantly, this committee gives podiatric residents a voice both locally in Broward County and at the state and national level as well. Their first meeting was recently held and several topics were discussed that will help them transition from residency to private practition. The BCPMA, led by Drs. Terence McDonald, Karen Lee, Nooshin Zolfaghari, and Cary Zinkin is working to develop this initiative statewide with other FPMA state components and hopes to serve as a national model for other state societies to embrace.

PODIATRISTS AND FOOTWEAR
NY Podiatrist is Professional Shoemaker

Mark Nelson, DPM, owner of Custom Cobbler in Poughkeepsie, studied podiatry before becoming a professional shoemaker. “Very few people as 7-year-olds actually say I want to be a shoemaker when I grow up,”  Nelson said.

Dr. Mark Nelson (Photo: Patrick Oehler)

Dr. Nelson got his start by learning to make prosthetic footwear for people with diabetes and walking impediments. He spent a few years in the Bronx before relocating to the Hudson Valley with his wife after the birth of their first child. In 1994, he opened a small shop on Mill Street in Poughkeepsie, doing shoe repairs and custom shoe designs.

INTERNATIONAL PODIATRY IN THE NEWS
Podiatry Recognized By Israel Health Ministry

After standing in the line for years with other health professions waiting for the Health Ministry to prepare rules regarding the profession of foot and ankle treatment, the Knesset Labor, Social Welfare and Health Committee on Wednesday completed regulation of podiatrists .

The committee approved permitted activities of podiatrists and podiatric surgeons (qualified to perform minor operations for bone, joint, ligament, muscle and tendon pathology). As a result, the profession will be recognized and supervised by the ministry, and therapeutic standards will be set. Those who work in the profession will receive an Israeli license and new immigrants who have worked in their country of origin will also be able to work in Israel. In addition, hospitals will be able to employ podiatrists.

PRACTICE MANAGEMENT TIP OF THE DAY
Common Web Design Mistakes Most Medical Practices Make - Part 1

Here are medical website design mistakes that can hurt your practice:

1. Poor Loading Speed

Most visitors expect a webpage to load within five seconds or less. More than 79 percent of visitors may not return to your website after a poor experience. These statistics indicate that poor loading speed can impact your lead generation and conversion rates. All of the hard work that you have put into developing and designing your website will mean nothing if your webpages take forever to load. The longer a visitor waits for a webpage to load, the sooner she begins to look for other medical practitioners in your area.

Web hosting can play an important role in your website’s loading speed. Other factors that can reduce your website’s loading speed are the size of your graphics, video quality, and plugins and scripts that you use. Pro Tip: You must treat website performance as part of web design. Your website design is not limited to a visually appealing layout; it must also enhance the user experience. A number of online tools can make your website lightning-fast and easily accessible by search engines.

CODINGLINE CORNER
Query: ABN Requirements

I need clarification on ABN requirements. I understand that Medicare does not require an ABN for non-covered services BUT I need additional clarification about the frequency of ABNs for services that are potentially covered. For instance, with a diabetic, high-risk foot care patient, if they come in consistently every 5 weeks (maybe because their calluses require debridement more often than the 61 days), do we have to have an ABN signed every other time they come in for the service (obviously not the time Medicare does cover) OR can we just have them sign an ABN annually?

Daniel C Albertson, APRN, Bluegrass Regional Foot and Ankle Associates

Response: An ABN needs to be filled out every time you perform a service that you feel will not be covered by medicare. Keep in mind that the routine foot care 60-day rule is an edit, not a global issue, so technically an ABN would not be required but it is a good idea to fill one out anyway.

RE: The Passing of Sebastian "Seb" Bentivegna, DPM

RESPONSES/COMMENTS (CLINICAL) - PART 1A
From: Lloyd Nesbitt, DPM

Consider Aldara (iImiquimod 5%). This is a cream that is applied topically on a daily basis and has to be continued for about four months -- and sometimes longer. While it doesn't kill the wart, it does boost the immune system locally so that the verruca does not survive.

Over the years, I have used laser on verrucae and in those cases when the lesions recurred, I have found the Aldara worked nicely to clear them up. So much so, that in many cases, I'll prescribe it as a first line of treatment rather than laser surgery, especially with children. It is an expensive prescription and patients have to stick with it, which most don't seem to mind, since they usually have had the verruca for such a long period of time to begin with. There is also a slightly less potent version called Zyclara (imiquimod 3.75%) which the drug reps seem to feel works equally well and is a bit less expensive.

RESPONSES/COMMENTS (CLINICAL) - PART 1B
From: David Secord, DPM

I've used the Bleomycin treatment for verrucae for about 15 years now and think of it as my primary treatment course for adults with normal immune systems. With this treatment, I've seen an approximately 98% success rate with the added benefit that if the person has multiple verrucae, treating just the one lesion will allow resolution of all of them in the course of 5-6 weeks.

The procedure entails using the body’s own immune system to kill the wart and follows the path of driving some of the warty material into the dermis, where the body will identify it and raise killer T-cells to the HPV (I, II, IV). As these circulate throughout the body, any place with a wart will...

Editor's note: Dr. Secord's extended-length letter can be read here.

RESPONSES/COMMENTS (NON-CLINICAL)
From: John Parmelee. DPM

Here is what I used while in practice.

95% of patients had no problem with it. 5% did and they would probably be the ones that you would have had a hard time collecting a deductible balance from.
