MD Podiatrist Discusses Distraction Osteogenesis
 
Sierra, 22, was born without bones in the majority of her toes on her left foot, as well as severely malformed toes on her right foot. She says that her condition, caused by an in-utero occurrence called amniotic band syndrome, has affected her physically and emotionally throughout her life. Podiatric surgeon Dr. Bradley Lamm explains that a minimally invasive procedure called distraction osteogenesis can be used to help naturally lengthen the bones in the feet. The surgery would involve breaking the bone and inserting a device to create space in which the bones can grow.

Plastic surgeon Dr. Andrew Ordon says that if done slowly enough, the skin should stretch naturally as well, though skin grafts may become necessary. Dr. Lamm adds that if Sierra proves to be a good candidate for the surgery, he will happily perform it for her free of charge.

MI Podiatrist Discusses Frostbite 
 
Frostbite is a serious tissue destroying disorder. It is also something that can be avoided. The Michigan Podiatric Medical Association (MPMA) has some advice to share during these winter months.

“When you’re out in the cold, your body works hard to stay warm by altering blood flow toward your heart and lungs,” said Dr. Jodie Sengstock, MPMA Director of Professional Relations. “This leaves your extremities — arms, legs, and feet — vulnerable to cold injury, especially toes and fingers.” Depending on the severity of the exposure, frostbite can affect the skin or underlying tissue. In most cases, the area becomes numb and feels frozen. Skin will appear waxy, white, or grayish. Any exposure should be evaluated and treated by a physician.

Concierge Medicine Firm Found Liable for Doctor's Negligence
 
MDVIP, the nation's largest concierge medicine practice, has seen meteoric growth since it was founded 15 years ago promising "exceptional care" and quick access to doctors in exchange for a $1,500 annual membership fee. But it took a big hit Tuesday when a Palm Beach County, FL jury returned an $8.5 million malpractice verdict against the company, which has nearly 800 affiliated physicians in 41 states. It was the first malpractice verdict against MDVIP and is believed to be the first against any concierge management firm. The companies offer such perks as same-day appointments and more personalized care with contracted doctors in return for a retainer.
 
The jury found that MDVIP was liable for the negligence of one of its physicians, who was sued for misdiagnosing the cause of a patient's leg pain, leading to its amputation. The jury also found the firm had falsely advertised its exceptional doctors and patient care. Industry experts say the ruling is significant because it shows concierge companies can be held liable for the care provided by their contracted doctors.

Federal Health IT Payments to Top $28 billion After December Surge
 
Federal dollars flowing to hospitals, physicians, and other eligible healthcare professionals under the federal electronic health-record incentive payment program have topped $28.1 billion, the latest CMS data show. Through December, 4,740 hospitals, or 95% of the 4,993 hospitals deemed eligible for participation in the program, have been paid, sharing $17.4 billion.
 
426,597 physicians and other “EPs”, or more than 79% of the 537,600 entities who are eligible, have received $10.7 billion. Total payments surged by about $2 billion over November numbers, in part because physicians and other professionals needed to meet their meaningful-use targets by the end of the calendar year.

RESPONSES/COMMENTS (ICD-10)- PART 1A

What Dr. Lukoff and most providers do not realize is that the sites that are free generally code to unspecified ICD-10 coding when you crosswalk the coding. This also happens in the majority of EHR software programs. So, the false sense of security is that you can stop with the coding that the website offers. Take, for example, a very simple diagnosis of hammertoe, ICD-9 735.4 - what the ICD10date.com site provides, like the majority of other mapping tools, is an unspecified code. Try it for yourself - you will get "2015 ICD-10-CM M20.40 Other hammertoe(s) (acquired), unspecified foot".  So, if you drill down into the tabular version they provide, you will get the final coding (for this example) after 3 clicks. Do the same for more difficult codes in injuries like fracture of toe - 826.0. It takes several steps to get there.
 
ICDtenhelp.com, is a site I created; you pay an annual subscription. For people who are unsure, I offer a 7-day free trial to prove my point.  It was designed to convert the ICD-9 to all the ICD-10 code possibilities, not just an unspecified code. You will need to know this when laterality exists and when the 7th character coding is required. Try the example I gave above (735.4 hammertoe in ICD-9). My site provides you all the options. It gives laterality, which will be needed. It does not require the doctor or biller to spend more time drilling down into the coding. The most important thing for me was to develop a site that is user-friendly and is quick. I don't know about you, but I value my time. Each click takes time, and time is money.

Any ICD code number description is only useful when it corresponds to a particular CPT code number description (and vice-versa).  Any CPT code number description is only useful when it corresponds to a particular ICD code number description. A claim can be wrong, incorrect, and even falsely submitted if the ICD and CPT codes are not matched properly  - the wrong ICD for the CPT, or the right CPT but the wrong ICD. 
 
The way the two pair up is important. A CPT which was once used for billing using a given ICD-9 code may not still be used with that same ICD, because some of the ICD code number descriptions are changing. It's important to also know if the CPT still can be used with that ICD-10 code number description as it was used for a corresponding ICD-9 code number description.
