Singapore Podiatrist Discusses Buying Running Shoes
Running shoes must be carefully chosen to suit your running style in order to prevent injury while running. Consumers should be discerning of technological and marketing fads when choosing shoes, said Tay Kai Ming, a podiatrist at the Changi General Hospital.

The key considerations include stability, cushioning, durability, and especially comfort, he added. Runners should take into account mileage, pace and performance requirements when choosing their footwear. For example, a marathon runner will need shoes with increased durability, whereas a non-competitive runner covering far shorter distances may require shoes with only general cushioning.

Scholl Holds Runners University
Rosalind Franklin University of Medicine and Science (RFUMS), in conjunction with Advocate Lutheran General Hospital and Illinois Bone and Joint Institute, LLC collaborated to provide a Runners University program entitled “New Year, New Goals: How to Get Ready for Your First 5K” on Saturday, February 2, 2013, at the RFUMS campus in North Chicago, IL. The moderator of the program was Robert Joseph, DPM, PhD, Chair of the Department of Podiatric Medicine and Radiology at the Dr. William M. Scholl College of Podiatric Medicine, one of five colleges or schools at RFUMS.

Dr. Beth Jarrett (kneeling) works with Runners University participants during the shoe evaluation and biomechanics workshop. |
This free half-day interprofessional seminar attracted more than 80 participants and offered a panel of experts in podiatry, physical therapy, family practice, and nutrition. These experts advised attendees on developing a training plan, analyzed their walking and running mechanics, and discussed ways to avoid injury. This event was for clinicians and non-clinicians alike and included a combination of short lectures and three hands-on workshops. Runners University is a collaborative, interprofessional initiative that provides an on-going series of community education and wellness events.

INTERNATIONAL PODIATRISTS IN THE NEWS |
UK Podiatrist is Witness to Student Protest Against Royals
Student protesters vented their anger on the most elite, and supposedly well-protected, of targets - a car containing the heir to the British throne and his consort, the Duchess of Cornwall. Adnan Nazir, a 25-year-old podiatrist who was following the protesters was among the first to recognize the royal couple in their customized Rolls Royce with its extra-large windows.
|
Adnan Nazir |
"I said, 'It's Camilla!' I wasn't trying to alert them. I was just surprised to see her," he said. "A few people turned around and started hitting the windows. People started kicking the car." In the confusion, Nazir said, a window opened. Charles kept his calm, gently pushing his wife toward the floor to get her out of the line of fire."Charles got her on the floor and put his hands on her," Nazir said.

HEALTHCARE NEWS |
Physicians Face Limits on Implant Choices
Gagged by their supply contracts, some hospitals have devised a simple way to educate physicians about the cost of pricey implants: using color-coded stickers to indicate the level of a device's price. Many of these hospitals are barred by confidentiality clauses with device manufacturers that limit, in some instances, whether hospitals in the same health system can share pricing data about the devices they purchase. Instead, they mark the devices with colored tags specifying high-, medium-, or low-cost options.
The widespread use of confidentiality clauses—which limit price transparency and hospitals' ability to shop for devices based on price—and longstanding relationships between physicians and device companies are the two major factors driving costs higher on implantable devices. Only over the past five years or so, have some hospital administrators started to implement strategies to reduce the costs of these items.

QUERIES (MEDICAL-LEGAL) |
Query: Patient Non-Responsive to Insurance Company Request
I treated a patient in November 2012 who has BC/BS of IL insurance. We checked her benefits prior to seeing her in the office. However, we have yet to be paid for those services. We contacted the insurance company, and they stated that they have received the claim, but cannot process the claim until they receive a filled-out survey from the patient regarding coordination of benefits. We have tried to contact the patient numerous times, but we receive no response. What is my recourse?
Joseph Borreggine, DPM, Charleston, IL
Editor's comment: PM News does not supply legal advice. Hopefully, part of your patient intake form states, "I am responsible for any fees not paid by my insurance company." The next step is to send a letter (via certified mail, return receipt requested) informing the patient that if she does not reply to the insurance company's request, she will be responsible for the balance due. If she still does not respond, you have the option of small claims court or a collection agency.

Query: Meaningful Use
Do we have to sign up every year for meaningful use if we sign up for the previous year (e.g., 2012)?
Amanda Bartell, DPM, Jacksonville, FL
Response: Once you have registered your certified electronic health record, it is not necessary to register again. Each time you attest, you simply go back to the same page that you registered on and choose the option to attest for that particular year.

RE: Etiology of Hallux Varus (Dennis Shavelson, DPM)
From: Stephen M. Pribut, DPM
When I think of the muscle engine that drives a ballerina from plié to en point, I think of the flexor hallucis longus. This is the muscle that is driving the ballerina onto point. The peroneus longus is functioning as an important stabilizer of the ankle. Yes, it has a significant impact on the first ray in gait, let alone ballet, but let us not confuse the peroneus longus with the FHL which is the primary en point enabling muscle.

RE: Etiology of Hallux Varus (Dennis Shavelson, DPM)
From: Eric Fuller, DPM
Dennis Shavelson, DPM wrote: "Hallux valgus occurs in the flexible forefoot functional foot types, with few exceptions. As Dr. Fuller mentions, this foot type characteristically is attached to an inhibited (exhausted), poorly leveraged, poorly trained peroneus longus muscle engine that cannot oppose the increased dorsiflectory stiffness of the first ray that exists in this foot type."
Dr. Shavelson, I never said anything like that. I've examined your foot typing system and would not use it to characterize this situation. Also, the rest of your statement doesn't make any sense. Please quote me correctly.

RE: Etiology of Hallux Varus (Ed Cohen, DPM)
From: Dennis Shavelson, DPM
All my hallux valgus/varus corrections have included a post-op orthotic. This ensures a better positional healing than a flat post-op shoe or CAM Walker. My lab fabricates PostopThotics™ cast 2-3 weeks pre-op and dispensed into the post-op dressing or footgear. These single (or paired if preferred) reasonably-priced devices are not covered by insurance. As the patient gets back to regular shoes, these are replaced with more permanent devices.
This is especially great when performing MIS, where osteotomies are often purposefully left unstable, as in Dr. Cohen’s case. I don’t know of any orthopedic foot surgeons who have gotten their heads around this concept, once again proving my point that DPMs are the leaders in biomechanics when it comes to foot surgery.

RE: Repair Info on Ritter Chairs
From: Carl Solomon, DPM
The old Ritter chairs were built like battleships, but parts and service information are getting scarce, as indicated by questions that pop up here from time to time. I found some information that's too good not to pass along.
ritterdental.com/ModelsChair.htm is a website that specifically addresses dental chairs, although electrically and hydraulically, they're almost identical to the models that most of us have: There are downloadable schematic diagrams, hydraulic diagrams, parts lists, etc. The name of the business is Schrader Electronics in Ashville, NC. Ph 828-665-0015. They are very knowledgeable.

RE: Web-Based Billing Systems (Mark K. Johnson, DPM)
From: John H. Susz, DPM
I have been using CollaborateMD for over 3 years. Set-up was very straightforward. Support has been excellent and very helpful with setting you up with the clearinghouse. Once the program is downloaded to your computer, you can access the system anywhere via the Internet. I have been told that the scheduler is easier to navigate and more user-friendly than MediSoft. You can also limit which employees have access to certain aspects of the program.
I recently integrated CollaborateMD billing software with MitochonEMR, a free, web-based EMR system that meets meaningful use criteria. All patient demographic information is pulled into the EMR from CollaborateMD. The only aspect of the EMR system that is not integrated is the scheduler. Other options available include a claim scrubbing tool, unlimited eligibility checking, and an electronic appointment reminder. Overall, I have been very happy with the program, support, and low cost.
