PRACTICE MANAGEMENT TIP OF THE DAY
6 Tips for Starting an RPM Program

Here are six steps to get you started with a remote patient monitoring program:

5. You may end up working with a partner.

Physicians must consider “build or buy,” creating an RM system with technology they are familiar with or joining with a vendor to offer a program, said Emily K. Hurst, DO, national medical director of intensive care remote monitoring for Avel eCare.

Don’t just shop around to see what vendors offer. “Figure out what you need and what you want first and then see if you can match it to (the vendor),” she said. “Because you’re going to get something that’s a lot more customized to what your needs are and you’re not going to be paying for things you don’t need.”

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: $250,000 Challenge to Podiatric Medical Schools
From: Sev Hrywnak, DPM, MD

I am writing to express my strong belief that for podiatry to thrive in the future, it is essential to establish a full license that reflects the comprehensive training in this field. To achieve this, we must invest in podiatric medical schools that offer the necessary courses and clinical rotations at teaching hospitals.

I have had the pleasure in teaching the past 27 years, 3,611 podiatric medical students, 1,490 allopathic medical students, and 477 osteopathic medical students. The data shows that in order to keep a high applicant pool to podiatric medical schools and to keep pace with healthcare providers in mainstream medicine,  i.e., physician associates and nurse practitioners who have a full license, podiatric physicians should have a full license.

I am committed to supporting this vision and will personally donate $250,000 to a podiatric medical school that prioritizes a curriculum and practical experiences in teaching hospitals leading to a full license (plenary license). This investment aims to foster a new generation of highly skilled podiatric physicians who can elevate the profession and ensure its relevance in the evolving healthcare landscape. The goal is to have this in place by 2026. Let us work together to create a future where podiatry is not only respected but also essential in patient care.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
RE: American Foot Care Nurses Association
From: Tyler Manson, DPM, Joseph Borreggine, DPM

I recently noticed a new business in town called Portland Foot Care Clinic run by an RN. https://www.portlandfootcareclinic.com/Services. Advertised services include custom orthotics, ingrown toenails, and diabetic foot exams.

Tyler Manson, DPM, Lake Oswego, OR

The APMA, I would opine, is totally fine with this? And, I assume so is the profession? We are the first line of defense when it comes to routine foot care. That’s nice. I would consider this to be an insult to the legacy of our profession. We are the primary foot care provider, period end of story! So, if we cannot get paid for “routine foot care”, then we just do not see the patient? Really?

So, just send them to a registered nurse? Really? That makes no sense. I guarantee that if a podiatrist were to truly do a foot examination on a patient who qualifies for routine care, other pathology would be identified with respect to any/all...

Editor's note: Dr. Borreggine's extended-length letter can  be read here.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 3
From: Rod Tomczak, DPM, MD, EdD

Filling med schools with SSI students, mentally challenged? Please Dr. Smith, one reference for your comments and Jerry Springer does not count. I have never heard such hog wash in my 40 years in medical education.

Rod Tomczak, DPM, MD, EdD, Columbus, OH

RESPONSES/COMMENTS (PM NEWS QUICK POLLS)
From : Elliot Udell, DPM

Dr. Harvey writes that during most of his career imaging for neuromas was not readily available, and he removed most of them based on clinical evaluation. In 2025, where we do have imaging available, could a podiatrist get into trouble if he or she does not order these tests and post-op findings show that the patient did not have a neuroma?

Could the patient say in court, "hey doc, you operated on me and it turned out to not be a neuroma and you could have known that by sending me for appropriate tests?"  Perhaps some of the legal experts could weigh in on this.

Elliot Udell, DPM, Hicksville, NY

Editor's comment: PM News does not provide legal advice. According to the FDA, the definition of stand of care is "Treatment for a disease that is accepted and widely used by doctors." Although the PM News Quick Poll is not scientific, it is likely that not imaging prior to neuroma surgery fits that definition.
