Ketogenic Diet as a Treatment For Neuropathy: MI Podiatrist

According to Conway McLean, DPM, "An estimated 30 million Americans suffer from some form of neuropathy. Unfortunately, a clear majority are never informed of the presence of this condition, unaware they cannot trust how their feet feel. Because they are not experiencing pain does not mean there isn’t a problem. Diabetic neuropathy is one of the most prevalent complications of diabetes."

Dr. Conway McLean

"The B vitamins are well known to be important to nerve function, and indeed, many people have obtained relief with precision nutrition. This is the practice of treating some malady by ingesting foods and supplements known to improve the functioning of that system. Recent research efforts have been directed at how a ketogenic diet might aid those suffering from neuropathy. Considering how few options for relief of neuropathic pain are available, the ketogenic diet would seem to be an exciting means of intervening in this debilitating condition," says Dr. McLean.

PODIATRISTS IN THE COMMUNITY
IL Podiatry Practice Adds 2 New Locations

Knowing when to expand your medical practice, whether it be investing in new locations or acquiring existing ones, is always a careful and measured move for any practice owner. For Patrick McEneaney, DPM, CEO of Northern Illinois Foot & Ankle Specialists based in Crystal Lake, Illinois, that business sense has only gotten more refined during the pandemic. As he has seen his practice quickly grow, he has had to be more agile and comfortable in regard to risk with where he decides to expand new locations.

Dr. Patrick McEneaney

On December 31, 2021, Dr. McEneaney bought two new podiatry locations, one in Chicago and the other in both Chicago and Arlington Heights, a fast-growing suburb of Chicago. Today, he has 14 total practice locations across the North, Northwest, and West suburbs of Chicago as well as two locations in the City of Chicago.

INTERNATIONAL PODIATRISTS IN THE NEWS
Your Guiding Eye Affects Posture: French Podiatrist

Just like the choice of a hand to catch an object or to write, the choice of the eye which directs the vision and the axis of the glance is determined in childhood, generally before 5 years, without one knowing why. One also speaks of preferential eye or aiming eye. “It also impacts the position of the gaze, the head, the shoulders,” notes Virginie Ruet, a podiatrist trained in posturology. We can have a “homogeneous” eye/hand laterality: right-handed with a right dominant eye, left-handed with a left dominant eye, or “crossed” (right-handed with a left dominant eye or vice versa).

Virginie Ruet

About 90% of people are right-handed but only 67% of people have the right eye as the dominant eye. “When you have to look at a specific point, the dominant eye provides more central vision and guides the second eye,” explains Ruet. The dominant eye contributes to manual skill. How to determine the dominant eye? Stretch out your arms, aim at an object with your hands forming a heart shape with both eyes open, close one eye: if the targeted object is still there, you have found your master eye. Otherwise… it’s the other one!

PRACTICE MANAGEMENT TIP OF THE DAY
Tips to Survive February - Part 3

With only 28 days, February is a short month but it can still add to physician burnout. Here are some ways that physicians can treat February the same as any other month in terms of maintaining an effective work/life balance:

3. Involve others in your quest to maintain work/life balance. "The more the merrier" is a relevant axiom when it comes to leisure activities, exercise, and getting out and about. When you have a partner - a family member or a friend to accompany you - you tend to do more, stay out longer, and derive more from your day.

CODINGLINE CORNER
Query: Removal of Subtalar Arthroereisis Implant

I have an adult patient who suffered from posterior tibial tendon dysfunction. A local surgeon addressed this by placing a subtalar joint arthroereisis implant. This had a very positive effect on the structure of the foot and reducing the symptoms at the posterior tibial tendon. Unfortunately, 5 months after the procedure, she is now having symptoms at the subtalar joint and would like the implant removed. There has been no movement of the implant and no breakage of the implant. With the 2022 CPT changes, does the removal of this implant count as an “implant” or “foreign body?”

Codingline Member

Response: With the 2022 CPT changes, this scenario was addressed. In this case, as you stated, since the implant has not moved and it has not broken, this would be coded as the removal of an implant. IF the implant had moved or had broken and is not serving its intended purpose, then this would be the removal of a foreign body.

RESPONSES/COMMENTS (PODIATRISTS AND DIABETES)
From: Steven Kravitz, DPM

It's not just DFUs; it is all non-healing wounds. I applaud any effort to attract more attention to the importance of nutrition for the medical community. This is especially true for my specialty of dealing with non-healing wounds. The report endorsed by the American Limb Preservation Society and cited by Dr. Armstrong makes important points but is focused on the diabetic foot ulcer.

Podiatry has paid much attention to the diabetic foot, and for good reason. But the question I bring to the floor is: has this been at the expense of being more articulate and informed as we should be on other chronic wounds? An example is the venous leg ulcer (VLE). These ulcers have a published recurrence rate of 75-80% within six months. A better understanding, and coordinated care, has been shown to bring this down to 25%. Nutrition is one aspect out of several to be addressed.

To the point regarding this article cited by Dr. Armstrong - All non-healing wounds require a nutritional assessment. It's not just diabetes.
