Twin TX Podiatrists to Speak at Black History Event
Kayla Robinson, DPM and Shayla Robinson, DPM are two of a kind. No, really: they are identical twins, hold identical positions, go to the same church, and share living space in Nacogdoches. The women are both podiatrists with East Texas Foot Associates. Dr. Kayla Robinson practices in Jacksonville each Thursday, and travels to other cities on the other days. Her sister treats feet mostly in Lufkin, although she will also occasionally travel.

The doctors will speak Feb. 15 to students at Jacksonville High School for a Black History Program. They will be part of a Question-and-Answer Panel, coordinated by Disciplinary Secretary Elizabeth Reagan. Their discussion will focus on the topic, “How To Be Successful,” something they have obviously already learned.

"The data reveals more injuries are suffered in pickleball than walking or swimming. The mechanics of the sport lend themselves well to repetitive-use injuries, especially in the wrist and elbow, and injuries from sudden, sharp movements. Along with improving your level of fitness prior to undertaking the sport, prevention is an excellent practice, one recommended approach being to warm up before playing," says Dr. McLean.

Rosenbloom emphasized the importance of gait training, particularly focusing on the angle of the foot during walking and running. He noted that many people walk and run with their feet straight but recommended an angle more akin to a duck walk to reduce stress on the gait cycle. Rosenbloom noted different regions in the U.S. influence walking style. Pearl added to these observations, noting neural circuitry's role in cadence speech and movement, particularly under stressful conditions like athletic competitions.

Concierge Medicine Practice Considerations
A concierge practice can be a lucrative way for a provider to achieve work-life balance by reducing the number of patients, increasing time with patients, and making great profits. But before you transition, speak with an experienced attorney who can help the transition. Here are some possible pitfalls:

Review the way in which you are separating from patients who do not join your concierge services.
When providers switch over to a concierge model, some patients will not follow your practice.

Provide adequate notice to patients of your new business model, and provide patients enough time to find another healthcare provider if they choose not to follow your new business model.

Review state patient abandonment laws to ensure you remain in compliance with any state regulation.

Starting this year, we implemented charging 3% for credit cards. Now about 5% of my patients pay with a check, up from 1%. Other than that, there seems to be universal acceptance of this fee. I pondered and hesitated for quite a while trying to decide if I would charge a fee or not. Now that I am doing it I have basically given myself a nice raise of tens of thousands of dollars per year. Times have changed and so we must change with them to stay competitive. I recommend that my fellow practitioners consider charging this reasonable fee.

Due to the increase cost of care and no fee schedule increases, we have implemented a 3.5 % credit card convenience fee. The merchant process charges the patient and there is no rental cost for the equipment. Patients are aware at the time of scheduling and it is transparent on my website. Unfortunately, running a medical practice has higher fixed and fluctuating costs. Practices have to reduce expenses and offer more products/services to offset the stagnant reimbursement we are receiving.

RE: Laying On Of Hands and Technology

I eagerly followed the recent thread regarding plaster vs. scanning for negative custom foot orthotic impressions, wondering if anyone would address the topic from the perspective discussed in this piece I recently read this story published online in Jama Network. The perspective I am speaking of is the "laying on of hands" that does not go hand-in-hand with our advancing technology.
While we debate the value of these technologies from 3-D scans to nerve testing to advanced imaging, let's not forget there is a flesh and blood human being on the other end that most believe are primarily driven by the intangibles of emotion. As Lewis Thomas put it, "…[T]ouching was the real professional secret, never acknowledged as the central, essential skill, always obscured...but always busily there, the laying on of hands. There, I think, is the oldest and most effective act of doctors, the touching" ...and yes, of course this skill should be applied professionally, prudentially, and appropriately.

Public knowledge and educating the public is really part of the answer. The other part is the patient has to feel confident and comfortable with their choice when seeking medical attention.
Retailers are now selling custom-made orthotics utilizing shippable impression kits. These start at $200. Originally, they were only available through podiatrists who were selling to their patients custom orthotics for $200 back in the 1980s. Now it seems these products are widespread, not just in custom orthotics but for all kinds of podiatry products.
Podiatrists, on average, are seeing 10-20 patients per day. Overseeing their practice and growing it takes a lot of...
