Feet Need the Same TLC as Other Body Parts: NJ Podiatrist
“Most people spend a lot of time each day taking care of their face, skin, hair, and teeth, but feet often end up being overlooked when it comes to daily routines,” says Velimir Petkov, DPM, a board-certified podiatrist and resident doctor at Premier Podiatry. Though you might think that your feet can handle anything—the skin on your soles is quite literally the thickest skin you have, after all—your tootsies require as much TLC as any other body part, at least if you want to keep them healthy.

To keep your feet happy and healthy, Dr. Petkov recommends never wearing old footwear and never wearing high heels for an extended period of time. Additionally, the doctor suggests seeking the help of a professional as soon as you notice any pain or discoloration in your lower extremities. “Checking with a podiatrist in a timely manner allows for early diagnosis and easier treatment,” he explains.

How Medicare Changes in 2020 Will Impact Beneficiaries
These changes will only impact beneficiaries who are not considered Medicare “eligible” until after 2020. As long as you turned 65 prior to January 1st, 2020, you can continue to enroll in these first-dollar coverage plans after they have been discontinued. They’re only being discontinued to those who are not considered Medicare eligible until AFTER 2020.
For those currently enrolled in a first-dollar coverage plan, you’ll be grandfathered in. You don’t need to make any changes to your coverage, unless you’re simply comparing benefits and/or rates to see if there’s a better plan for you.

Query: I & D with Digital Amputation
I recently took a patient to surgery and performed an extensive incision and drainage of a subfascial abscess, as well as amputation of the right second toe at the metatarsal phalangeal joint. Can I bill for both? The second toe had to be removed secondary to wet gangrene, presumably from the abscess. I just wanted the opinions of others on this.

Response: I would not bill for both; the I&D portion would be included in the amputation. By removing the toe, you removed the site of infection.

Query: Pain 2nd MTPJ S/P Weil Osteotomy
A 61 year old female presented with pain at the dorsal lateral aspect of the left 2nd MTPJ, which has been increasingly symptomatic for the past 6 months. Direct palpation and attempts to dorsiflex the MTPJ exacerbate the condition. The patient is S/P a Weil osteotomy with arthrodesis of the left 2nd PIPJ 1-1/2 years ago.

X-rays reveal arthritic changes with lateral subluxation of the left 2nd toe. I am considering a left 2nd proximal phalangeal base resection. I welcome any recommendations for treatment that may ensure the best possible outcome for the patient.

Dr. Levy posted an alert regarding the utility of the ABCDE rule for ensuring a high index of suspicion for lesions that may be melanoma. The ABCDE mnemonic was developed in the early '80s as an aid for PATIENTS to self-evaluate lesions and bring them to the attention of the clinician. It proved to be so reliable that it was then taught at the medical school level and residency level for use by clinicians. The validity of the ABCDE rule is still strong, but it is not the whole story. There is one well referenced and important study published in December 2006 out of Australia by Wendy Liu, et al., Rate of Growth in Melanomas, Arch Dermatol/Vol 142. In this study, there were some very important findings regarding rapidly growing invasive melanoma, highlighted with the fact that in these cases, ABCDE may not apply. Specifically, rapidly growing invasive melanomas often demonstrate symmetry, regular borders, and amelanosis.
Additionally, they tended to be in patients with less nevi and freckles. These are all contrary to the accepted ABCDE features and may deflect the clinicians away from suspicion and cause them to inappropriately re-assure the patient. The authors point out that this in no way invalidates the ABCDE rule, but indeed these findings reinforce the skin biopsy as the gold standard in diagnosis, and should remind all clinicians, especially non-dermatologists that any initiation of therapy that does not provide rapid resolution should prompt a biopsy or a consultation.

I predicted the separation in podiatry a decade or more ago. At my most recent inspection, the 3-year surgical resident logs many hundreds of surgical cases and only 50 biomechanical evaluations.
When I was an active ABPS foot and ankle surgeon, a bio evaluation and foot typing was the core of every initial examination, surgery or not. My orthotics, then and now, are custom cast, dispensed and monitored along with foot centering training with an end goal of reducing their need (weaning them away), before, during, or after a surgery. As there are more and more podiatrists better skilled than I am, I now dedicate myself to performance issues, prevention, and quality of life upgrading successfully interjecting procedures, prn with consultants.
My dilemma question is, does the surgically specialized DPM dispense custom orthotics and perform rehab or skilled maintenance of their patients? If they do, then they are not surgically specialized and if they don't, why aren't we cross-consulting within our profession for the benefit of our mutual patients and podiatry’s survival?

Checking the online curriculum of Temple (formerly PCPM), they still do not list a course in psychiatry. Secondly, we did receive training in endocrinology, and the side-effects of steroidal and NSAID medications. Third your last statement... I agree with. I would need to have education and training regarding THC and cannaboids and pain management.
Fourth, I was merely responding to a posted question: “I am curious as to why almost 30% of respondents either said no or are unsure regarding being able to prescribe medical cannabis, when there is a plethora of data that supports its benefit for a myriad of conditions.”
Perhaps I was being naïve in assuming that someone actually wanted an answer as to why 30 percent of respondents said "no". And I have absolutely no problem with the 70 percent of the respondents who disagree. I respect the passion you have regarding the topic, but do not appreciate the snarkiness in your correspondence.
