Parents Should Buy Shoes When Child Starts Walking: CT Podiatrist

For parents of young children, purchasing a fresh pair before the old ones even seem broken in can be a burden. But a child's foot can grow up to a half size every three months, which means staying on top of proper fit.

AT THE COLLEGES

Achilles Master's Course by Western University a Success

This course began with a pre-event fireside chat on February 3 with a case work-up at PAMF Palo Alto Sports Medical Clinic. The next day consisted of lectures and a cadaver workshop covering percutaneous lengthening, open and percutaneous acute rupture repair, peritenolysis, endo and open gastroc recession, FHL and PB transfer for chronic Achilles rupture, and insertion repair  with anchors for Haglund's deformity .

NY Podiatrist Speaks To Rotary About "Diabesity"

Dr. Anthony T. Tuccio recently spoke to members of the Rotary Club of Jamestown. "Diabesity, the combination of obesity and diabetes, has become a major medical problem in the United States as well as around the world", said Tuccio.

Tuccio graduacted from the Ohio College of Podiatric Medicine and completed his residency at Harrison Community Hospital, now St. Anne's Hospital, in Mt. Clemens, MI. He is on the staff of WCA Hospital and Warren General Hospital, where he serves in the hospitals' wound care clinics. .

Canadian Podiatrist Co-Founder of Exterior Skate Protection

Pretty much every kid who has laced ‘em up to play hockey has experienced that agonizing sensation of taking a shot off the ankle or foot. Now, working under the company name Exterior Skate Protection, Vernon’s Dr. Bruce Booth and Larry Jensen are making Shotblockers, using a polypropylene graphite material to ensure they are light (roughly 80 grams per skate) and have a low profile which doesn’t obstruct a player’s stride.

Shotblockers earned some notoriety a few seasons ago when former Vancouver Canucks’ forward Ryan Johnson, then one of the team’s best shot blockers, was recovering from a foot injury. Hockey Night in Canada’s Scott Oakes did a small on-air segment during a CBC broadcast, detailing how Johnson was using the product to protect his feet. “Johnson wore them for six or seven games, decided to take them off right before the playoffs and broke his foot again,” said Booth, a podiatrist who operates the Vernon Foot Clinic.

Query: Hyperhidrosis

A 14 y/o young woman with no other problems presented with an ongoing case of sweaty hands and feet. Her mother stated that she has suffered from this her whole life. The patient has tried many OTC products with little effect. The problem has brought her much anxiety, which further exacerbates the issue. She is currently changing her socks 3-4 times per day. She is also currently looking into surgical procedures to correct the issue. What options are available to me?

Given the severity, I would seriously lean towards a hemiphalanx removal at the PIP joint. This should allow for quick recovery and keep the remainder toes in good alignment.

I would look into Krystexxa. This is a new medication put out by Savient Pharmaceuticals. It is a unique anti-gout medication as it is the only one that will actually dissolve tophi. This medication is given by infusion therapy. Contact the company directly and ask them for the name of a doc (usually a rheumatologist) who is currently administering such therapy. While an amputation is certainly a plausible option - trying to save the toe is another option. If you should try the Krstexxa route, let us know how the patient fared.

Generally, green color in the nails say Pseudomonas. If symptomatic, do a nail avulsion, and as the nail is growing back, have the patient apply Lotrimin Ultra bid. Odds are this will take care of the problem.

Given your patient's active lifestyle, hallux length, and symmetrical nail plate appearance, this seems like a classic case of "black toenail syndrome" from repetitive micro-trauma from ill-fitting aerobic shoes; in spite of your patient's assertion to the contrary.

I'd recommend re-checking your patient's shoe length and toe box taper, as one finger's breadth from the longest toe is generally accepted as proper fit. Egyptian foot types where the hallux is considerably longer than the lesser toes (your patient), active lifestyles, rapid stop and start, decelerating movements, and heel raises are typical mechanisms. Second toe involvement is common in "Morton's" foot types. Short shoes abrade the digit against the toe box; long shoes allow a piston effect. Providing a large foam toe sleeve after nail debridement helps avoid additional trauma to the nail plate.

RE: The Use of Uloric vs. Allopurinol in the Management of Gout (Elliot Udell, DPM)

I respectfully disagree with part of Dr. Udell's second paragraph comments. The problem with re-examining Uloric or allopurinol in the treamtent options is that they are not supposed to be used in cases of acute gout attacks and can actually spur the attack. Colchicine was used as a preventitive and could also be used as a treatment. I do agree with Dr. Udell's first observation: shame on the FDA.
 
The public should be outraged. The problem is that patients don't actually understand what happened. I have many gout patients who complain of the expense and some who can't afford it. If the public actually knew what had occurred - collectively, possibly something could have been done. I wonder if arthritis associations and health plans like AARP ever voiced opposition to what had occurred.

I was raised to believe that you don't request or demand a reputation - you earn it and are granted it by others. Yes, don't be shy, show them what you do, show them what you got and the results you get...but you don't give yourself creds. You can apply for them, but they are given, not taken.

Leonard Levy has been an inspiration to me since I was a student at CCPM in 1977, and has been among our greatest ambassadors in the world of medicine. He has upped our reputation, not just by what he says, but by what he has done. I agree with him that we are physicians, we are surgeons, just as we are...

RE: CA Podiatrist Discusses Plantar Fasciitis

If quoted accurately in the Ladies Home Journal, then it's quite disappointing to read a colleague in a professional leadership position comment, "high heels are not the evil doer" when discussing plantar fasciitis. That might induce women to maintain high heel wear on a consistent, long-term basis. Horrible for feet...great for business.

Hopefully, that comment was taken completely out of context and will eventually be retracted, or modified. While plantar fasciitis' biomechanical etiology is multi-factorial, how does anyone justify their denial/omission of gastrocnemius equinus as one of, if not the most notorious, devastating etiologies? In our professional capacity as foot clinicians, don't we...
