NYCPM Students Participate in NYC's Salvation Army's Don't Walk By Event
 
NYCPM students led by Joanna LI, DPM and Dan Kline, Chair of Community Medicine, participated in the Salvation Army's annual Don't Walk By Event held in Manhattan this past weekend. Over 300 of those surviving on the streets of NYC in the neighborhood surrounding the Salvation Army's divisional headquarters were served a hot meal and provided access to an array of social services and medical screenings including foot screenings. Members of NYCPM's Public Health Club chaired by Rivka Kesler who organized the student group, staffed the foot screening portion of the event.

Clients also had their feet washed by the students and provided a pair of fresh new socks. The students displayed great empathy and understanding, listening to everyone's story and challenges they face living on the streets of Manhattan. MD Podiatrist Discusses School Ban on Wearing Crocs
 
Dozens of schools in at least 20 states across the U.S. have banned students from wearing Crocs to class. While many students don’t agree with the ban, experts shared that adults’ concerns aren’t irrational. “Crocs can easily get caught in equipment, causing the child to fall or injure their foot or rip a toenail off. Things I see in the office on a daily basis,” podiatrist Priya Parthasarathy, DPM explained.

A study published in Footwear Science found that the lack of heel support in Crocs makes wearers more likely to trip or fall. Dr. Parthasarathy said that the shoes can be worn in moderation without worry and are great for recovery after a workout — just don’t run in them.

There are different classes of medications used to treat peripheral neuropathy, including SSRIs, tri-cyclics, SSRNIs, anti-convulsants, and anti-depressants. My experience is with gabapentin, pregabalin, duloxetine, doxepin, and amitriptyline. The goal in dosing these (besides drug/drug interaction concerns) is a balance between effectiveness and anticholinergic side-effects while also looking at potential onset of extrapyramidal syndrome. The medication which has a flag for extrapyramidal symptoms is amitriptyline and I always avoid its usage in anyone with peripheral neuropathy who is over the age of 60 and may have a drug/drug interaction exacerbating this.
 
I’ve found that amitriptyline is very effective and inexpensive, but can have the most pronounced anticholinergic side-effects. My "go-to" med for peripheral neuropathy is duloxetine, as it appears to have the least anticholinergic side-effects and the least incidence of extrapyramidal...

Dr. Kesselman has asked PM News readers to comment regarding their experience with commonly utilized medications for the treatment of symptomatic diabetic sensory neuropathy and risk of falls. I would like to share my approach to this problem.
 
Frequently, the symptoms of symptomatic diabetic neuropathy are exacerbated or occur primarily during the evening hours. Therefore, in many patients, I will prescribe a shorter-acting adjunctive analgesic (e.g.-pregabalin, gabapentin, or a tricyclic antidepressant) at bedtime, reducing or eliminating the risk of falls. Oftentimes, a relatively low dosage, almost sub-therapeutic dosage, may be sufficient to provide the patient with a restful, restorative night's sleep and improve...

Dr. Jacobs brings up some great points as usual. A big problem with medicine and podiatry is medical school selection. If you aren't aware of this, you won't believe it, but it's true. Medical schools make so much money taking people on permanent SSI disability that some fill as many seats as they can with them, and then take a few of the best and brightest. Medical schools actually promise a seat to SSI students when they graduate from high school. 
 
College is just a formality. Since they are disabled, many don't have to study, show up for tests and class in college. When they get to medical school, they have little education and no study skills. Medical schools had to compensate for this by creating "alternative pathways of learning" that allow them to grade students subjectively. Some of these people end up being great doctors. Some have severe mental or personality disabilities that prevent them from doing so. Medical residency is a lottery and once they are in, they can't be removed, so they just treat them like a mushroom, graduate them, and turn them loose on the population.

Dr. Lullove implies that it is near malpractice to remove a suspected interdigital neuroma without confirmation of the diagnosis with pre-operative advanced imaging. Simply stated, his position is not only contrary to the standard of care (as reflected in PM News), it is contrary to published studies addressing this issue.
 
Numerous studies have demonstrated that the accuracy of clinical diagnosis of an interdigital neuroma is sufficient such that advanced imaging is seldom required (e.g.: Pastides, et al, Foot and Ankle Surgery 18 (1) 2012, Mahadevan, et al., JFAS 54 (4), 2015, Owens, et al., Foot and Ankle Surgery, 17 (3), 2011.) The clinical diagnosis has been shown to be safe and reliable. Advanced imaging is indicated when the diagnosis is unclear or...

However, over 50 years, I have removed hundreds of neuromas with no imaging. My schoolmates would say the same. In the first half of my career, imaging was not widely available.
