PODIATRISTS IN THE NEWS |
CT Podiatrist Helps Develop Geo Cart for OR Efficiency
Yale New Haven Hospital is the first in the country to have the Geo Cart. A CT podiatric surgeon assisted in developing it. Podiatrist Peter Blume, DPM is performing a foot and ankle procedure which normally takes four hours, but a hi-tech system in the OR aims at reducing that time by up to 30 percent. "To me, it's like an Uber and an Amazon, bringing technology into the operating room," says Dr. Blume.
|
Dr. Peter Blume |
The Geo Cart, which Dr. Blume helped design, serves up pre-sterilized instruments and implants - everything a surgeon would need. "It takes away the worries as you are operating - do you have the right size screws, do you have the right level screws, do you have all the equipment you need?"

MEDICARE UPDATE |
How Medicare Changes in 2020 Will Impact Beneficiaries - Part 1
Why First Dollar Coverage Plans are Being Discontinued
Starting in 2020, there will be no more first-dollar coverage plans available to those who are considered Medicare eligible after 2020. First-dollar coverage plans are Medicare Supplement Plans that leave you with zero out-of-pocket costs. The Medicare Access and CHIP Reauthorization Act of 2015 eliminates all Medigap Plans that cover the Medicare Part B deductible. The reason for the change is due to some members of Congress believing Medicare beneficiaries are over-using healthcare services since they don’t have to pay anything out of pocket at all.
By making everyone meet the Medicare Part B deductible, legislators hope to prevent beneficiaries from running to the doctor for every minor ailment. Critics argue that the deductible may keep people from getting care they need. When people don’t see a doctor, they can end up with more serious conditions down the road.

RESPONSES/COMMENTS (CLINICAL) |
From: Ira Baum, DPM
As podiatrists, we see many skin lesions on the foot. In a busy practice, it’s difficult to study a lesion when it’s not the primary complaint. I want to share with the comminity a personal experience that doesn’t exactly fit the above scenario, but one that profoundly affected my professional life. A respectable, intelligent, health conscious 40-something individual presented to my office with a wound on the plantar aspect of the toe. The lesion was diagnosed, by a dermatologist as a wart and treated multiple times. The lesion appeared like a typical non-healing wound that initially responded as expected with a granular base, although a bit exuberant. Lack of migration of the wound edges led to a biopsy with a wide excision of healthy tissue. Pathological diagnosis identified a Clark level IV.
Long story short, after multiple surgeries and experimental drug treatments, he died. What I am trying to teach here is the importance of having a clear plan of treatment that includes timelines with identifiable benchmarks that may help you quickly recognize when there is a problem that needs to be addressed. Treatment plan methodology is not only for potential melanoma, but for post-op course, diabetic foot ulcers, and every treatment we provide.
Ira Baum, DPM, Naples, FL

RESPONSES/COMMENTS (PM NEWS QUICK POLLS) |
From: Allen Jacobs, DPM
Dr. Krischer opines that podiatrists should not be allowed to prescribe marijuana as podiatrists do not receive psychology or psychiatry training sufficient to evaluate for side-effects. Really? What about cardiac, renal, GI side-effects of NSAIDs? Is cardiology, nephrology, GI training required?
What about opioid analgesics? Do they not potentially initiate the very side-effects with which he is concerned? How about corticosteroid injections? Or topical steroids? Is mandatory endocrinology education required? As an aside, at PCPM we did receive a full course and clinical rotation in psychiatry.
As for the “side-effects” of marijuana, perhaps the good doctor needs to educate himself regarding THC and cannabinoids and pain management. And while he’s at it, educate himself regarding the education of podiatry students and residents in 2019.
Allen Jacobs, DPM, St. Louis, MO

RESPONSES/COMMENTS (YOU CAN'T MAKE THESE THINGS UP) |
From: Todd Muhly, DPM
Ellen DeGeneres has a new stand-up comedy show on Netflix. In it, she mentions that she went to a podiatrist for a neuroma. She reports that she was told there was no treatment for neuromas and that she needed to wear cowboy boots. Maybe something got lost in translation, but it is concerning that this podiatrist seemed to offer no treatment options. Either way, our profession came out poorly there.
Todd Muhly, DPM, Eugene, OR
