UT Health San Antonio Hosts Free Diabetic Foot Health Screenings
 
Podiatrists at UT Health San Antonio say San Antonio and South Texas have one of the highest rates of diabetes in the country, which is the leading cause of amputation. Now, they’re hosting free foot exams for diabetic patients to help them avoid amputation.

“It almost always starts with a wound, and then that wound gets infected, and then you might get gangrene, and then that leads to the amputation. But if we can get this thing really early, even before it gets to a wound, by changing your footwear or sometimes doing a preventative surgery, we can avoid all of that,” said Chief of Podiatry at UT Health San Antonio, Lee Rogers, DPM.

MEETING NEWS
U.S. Podiatrists Attend Northern Lights Wound Workshop
 
The Northern Lights Wound Workshop was held in Iceland and attended by nearly 2 dozen U.S podiatrists. Dermatologists, general surgeons, wound care nurses, and clinical researchers were also in attendance. The workshop featured keynote speakers and breakout sessions to review cases. The Health Prime Minister of Iceland gave a keynote address that emphasized healthcare solutions supported by robust research that can provide the most benefit for patients.

In the breakout sessions, clinical pearls were discussed such as recognizing calcified vessels (MAC) on foot x-rays as a sign of small vessel disease that could develop into cardiac risk. Different methods of rapid delivery of fish skin particulate and wraps were reviewed with adjunctive procedures to minimize trauma such as percutaneous Achilles tendon lengthening.

Dr. Roth laments his conversation with an individual in that allegedly, a graduate of a three-year podiatric residency had never performed a surgical procedure "skin to skin". He queries how this is possible.
 
Community podiatrists do not have an ethical or legal obligation to allow residents to participate in surgery on their patients. We must recall that patient sought the care of the attending, not a resident. Our first obligation when we participate in medical education is to always prioritize patient safety, not the education of the resident. A community podiatrist may determine that it is in the best interest of the patient to limit or not allow a particular resident's participation in a particular case. With that said, PM News readers know there is touch and sense to surgery that must be experienced and cannot...

My son will be graduating this spring from podiatry school. He recently called me and said he watched a 3 plus hour Lapidus performed by a resident. Yes, Dr. Smith is correct. Somehow, we turned a 3-year residency into a waste of time for some of our upcoming future docs. We need to be careful who we let in to train as podiatrists and not let the almighty dollar rule the schools in accepting students who cannot communicate well or have disabilities that would not allow them to function in society correctly.

Let's not forget the 30 second Mulder's sign test for neurofibroma. When you learn the two different compression techniques for providing it (shown on video), count on having near 100% accuracy.

This question really shook me with the answers posted as of Sunday. For over 50% of respondents to say that “Yes” to remove a neuroma without ANY imaging is amazing. In today’s world of advanced ultrasounds, MRI, and even 3D CT models — to not image a patient prior to surgery just to make sure you are doing the correct thing — but to make sure that the complaint is TRULY the pathology is disheartening. While I know the issues of “cost” and “defensive medicine” will come up — you would NEVER see a general surgeon or orthopedic surgeon EVER remove any part of human anatomy without imaging.
 
To those over 50% of DPMs who are answering, “Yes” — ask yourself what happens if you operate and it’s a Schwannoma? Nerve sheath tumor? Hemangioblastoma? It just could be a ganglion cyst. Either way, getting definitive advanced imaging would be a lifesaver. What if you don’t have privileges to remove a malignant tumor? What if you don’t have privileges to remove a neural tumor? How can you be 100% sure you’re acting according to scope and standard of practice by NOT ordering an MRI/ultrasound? 
 
Ask your malpractice carrier what happens next. Ask yourself would YOU rather your doctor order the MRI or US to MAKE sure it is what it is they are removing? Do the correct thing for your patients. One more step before jumping to the OR before a quick surgery usually demands prudence and patience.
