Barefoot Running Not Recommended for Most Runners: NY Podiatrist
"I am not a barefoot running enthusiast. After the publishing of the book Born to Run, there were many injuries and many lawsuits from that book. It is certainly wonderful for runners who have no foot issues, but if you pronate, you want to be in a well cushioned supportive shoe," says Douglas Tumen, DPM, a podiatrist in Woodstock, NY and author of Ask the Foot Doctor.

"There was a big fad for a long time when people got minimalist shoes like Vibram 5 Fingers. All that is great if you have a soft surface to run on, but it does tend to lead to more injuries. I don't recommend it because we see way too many injuries," says Dr. Tumen.

Lab Abnormalities in Children Taking Terbinafine
At a single institution, 134 children (aged 3–18 years) were prescribed systemic terbinafine for either tinea corporis, tinea capitis, or onychomycosis. Most patients received baseline liver function tests (LFTs, 81.3%), baseline CBC (76.9%), monitoring LFTs (87.3%), and monitoring CBC (84.3%). No patients had baseline LFT abnormalities. Two of 117 (1.7%) had abnormalities on monitoring LFTs; in both cases, the patients were being treated for onychomycosis for 12 weeks, and abnormalities resolved within 4 weeks of treatment completion. Four of 113 (4.4%) had monitoring CBC abnormalities, which did not worsen with continued treatment and resolved within 4 weeks of treatment completion. All lab abnormalities were mild, and patients were asymptomatic.
Safety data on the use of systemic terbinafine in children are limited. This study suggests that lab abnormalities are rare in children and will likely resolve after completion of treatment.

2021 Remote Patient Monitoring Changes: Key Takeaways - Part 7
In December, the Centers for Medicare & Medicaid Services (CMS) released its 2021 Medicare physician fee schedule final rule. Within the final rule were some significant changes concerning remote patient/physiological monitoring (RPM) and the provision of other remote services. Here are the most significant takeaways from the final rule.
7. Expanding Coverage to Acute Conditions
CMS had initially described RPM as services rendered to patients with chronic conditions. With the final rule, it clarified that practitioners may furnish RPM services to patients with acute conditions as well.

My classmate brings-up an interesting subject. We were trained in Root biomechanics by Drs. Forman, Spencer, and Tax. I still follow those principles with non-weight-bearing neutral plaster impressions with basic measurements. The pearl that I learned many years ago was to indicate to the lab "no cast correction." I will also write, fabricate orthotics to full-casted arch height. This is especially important for orthotics fabricated for a cavus foot type. I often see "custom" orthotics that do not at all look like the patient's foot in neutral position. Is it because of blue foam boxes, poor technique, or a generation of podiatrists not trained to do the job properly?

When adding an accommodation to the forefoot of an orthotic for an IPK or to off-load a sesamoid or metatarsal head, I have found that even when you mark the lesion in the cast or the scan, it frequently does not line up correctly the first time. When you place the orthotic up against the patient’s foot it looks correct but when they stand on the orthotic in the shoe, the forefoot usually slides laterally.
I always use lipstick on the foot of the patient in the area you want to off-load when I dispense an orthotics. Then I have the patient walk barefoot on the orthotic in the shoes. I think you will find that the area you want to off-load is frequently slightly lateral to your accommodation. In addition, the accommodation often does not extend as far distally as the end of the symptomatic area.
I ask for all of my accommodations to end at the sulcus. I think you will be surprised how often the build-up meant to be around the painful area is actually pressing directly on it and either not helping or making it worse. Better to find out with lipstick right away then have to wait six months until the patient shows wear in the forefoot of the orthotic that shows it needs to be adjusted.

Kudos to Dr. Markinson for his astute observations. While the first 2 years in podiatric medical school are on par with our medical student counterparts, the 3rd and 4th year are not mirror images. We cannot simply say as a profession that every DPM should be granted another degree because "in our opinion" we go through the same number of years in training. This discussion has been ongoing for decades within our profession. The only way I see this degree change happening is through traditional pathways.
That means, as Dr. Roth points out, that podiatry schools will be absorbed by allopathic schools so that all students attending will attain an MD and decide on their specialty after graduation. Those who want to specialize in the foot/ankle/lower leg as podiatrists (it would be interesting to see how many) will do so after they obtain the MD. They will go through the exact same didactic and clinical externship education. The question of "parity" by our medical colleagues, hospitals, and the equally important insurance companies will never be an issue again.

I'm one of the silent majority content with my limited license but not content with the disparity in reimbursements, if they exist. My kid brother, a critical care surgeon, retired at 64 burnt out by the demands of his specialty and his low reimbursements. Years ago, when reimbursements were reasonable, whenever I would complain to him about the lack of acknowledgement and licensure limitations of my DPM degree, he would say just "laugh all the way to the bank."
However, I'm no longer laughing as reimbursements continue to decline. While legislation without proper training is not the answer for parity in scope for those who seek it, it might be more appropriate for approaching the insurance industry by our representatives. Who can we turn to for a realistic plan for the future? Can anyone reveal the actual disparity in reimbursements comparing DPM vs. MD?

Dr. Poggio is dead-on with his advice. Starting in 2018, our local BCBS was across the board denying any E/M code submitted with a -25 modifier. I appealed EVERY denial with treatment notes using the Availity portal for BCBS; and every denial was overturned, and payment made for the E/M code. Eventually, once 2020 rolled around, BCBS stopped denying our E/Ms with the -25 modifier, as they must have come to the realization that yes, we ARE billing properly. As Dr. Poggio indicates, make sure the E/M is truly significant and separately identifiable, and appeal, appeal, appeal!
