The Board of Directors at Morris Hospital & Healthcare Centers has approved the appointment of several new physicians to the associate category of the Morris Hospital medical staff. Among them is Dr. Kyle Pearson, podiatrist and foot surgeon with Rezin Orthopedics & Sports Medicine.

Dr. Pearson graduated from the Des Moines University College of Podiatric Medicine & Surgery and earned his Doctor of Podiatric Medicine and Masters of Health Care Administration degrees in 2011. He completed a residency in foot and ankle surgery at the University of Pittsburgh Medical Center in 2014.

VA Podiatrist, 82 Closes Historic Shoe Store 
 
Arlington will bid farewell to one of the region's last independent shoe stores this year as Public Shoe Store prepares to close its doors for good after 75 years on Wilson Boulevard. That's because owner S.H. "Doc" Friedman is ready to hang up his foot-measurer at the age of 82 and plans to lease the building — which his family owns — to a new tenant. A podiatrist by training, Friedman has the knack for "the perfect fit," and has been known to insert special padding into shoes to get it just right.

Friedman went to podiatry school at the behest of his father, Sam Friedman, who opened the original Public Shoe Store in 1938. It was originally located across Wilson Boulevard where the Clarendon Metro station now sits. But the skill of fitting shoes is one that's largely lost nowadays, Friedman said. "It's not as helpful for most fashion shoes because the manufacturers don't follow the measurements," he said. "How the shoe fits on the foot can now vary a full size or size and a half."

Pharmacies Seek to Withhold Records from Texas Investigators
 
A specialty pharmacy that drew the attention of state officials worried about possible kickbacks to doctors is now being investigated by the Texas attorney general’s office and is asking a judge to block demands for certain financial documents. State lawyers are examining HealthScripts Specialty Pharmacy for possible violations of the Texas Deceptive Trade Practices Act, according to a petition filed by HealthScripts and three affiliates: HealthScripts of America in North Austin, Houston, and Midland-Odessa. HealthScripts custom-makes, or compounds, prescription drugs for patients and seeks physician investors who refer patients to the pharmacy and share in the profits.
 
HealthScripts says in the petition it filed Jan. 30  that the attorney general’s vague, overly broad document demands were made to a foot doctor in Midland, Dr. Vincent Rascon, and to pharmacy company executive Bruce V. Gingrich, who lives in the Houston area. Rascon was disciplined 10 years ago by the Texas State Board of Podiatric Medical Examiners for incorrectly advertising himself as a fellow of the American College of Foot and Ankle Surgeons. He was required to correct the information and pay a $500 fine.

Query: Billing Program for Practice Fusion
 
I am involved in a small practice and wish to change our billing program. We do our own billing in-house. We are looking for a billing company to submit electronic claims and posting payments. We use Practice Fusion as our EHR. They recommended NeuMD, CollaborateMD, and AdvancedMD. Does anyone have any experience with these companies or suggestions on other podiatry-friendly billing services?

Query: Business Group Proposal to Purchase and Relocate Practice
 
I have been approached by a business group that wants to purchase my 30-year old solo practice and relocate it to a building they renovated where they have physicians and specialists of different specialities. They have 6 of these "medical communities" already established including pediatricians, dermatologists, family practitioners, chiropractors, mental health counselors, and labs just to name a few. There are currently no podiatrists. They want me to stay on as long as I would like (2-5 years) as they prefer this to starting with an unknown practitioner. I would be salaried and all expenses are paid. With ICD-10 on the horizon and the challenges of running a private practice, I am more than willing to take this leap; but I am anxious to know if any of my colleagues have dealt with this already.

Kindly refer to my 2005 JAPMA (Vol 95; No. 3) article "Cimetidine as a First-Line Therapy for Pedal Verrcua"
 
To summarize, the salient points of the manuscript, cimetidine's therapeutic dose is between 20-25 mg/kg/day in divided doses, not to exceed 1,600 mg/day. As such, your 15 year old is currently significantly under-dosed. It's likely he'll need a minimum of 1,200 mg/day. Secondly, the article's graphs depict median response times of six weeks with a bell curve ranging from 3-12 weeks. With proper dosing, one can expect success rates around 85%.

The dosage is 30-40mg/kg in divided doses. It can be anywhere from 3-12 weeks for eradication, but I also recommend some topical as well.

The dosage for cimetidine is 1200 mg daily in a single or divided dose for a 15 year old male. I also use topical application of 10% formalin solution once daily. I see the patient in 3 weeks to debride the lesion and every 6 weeks after that until the lesion has resolved. There is no fast response. It could take weeks, but 2-3 months is typical.

Cimetidine should be dosed at 1200-1600 mg daily in divided doses, depending upon the weight of the 15 year old. I rarely see a response in less than two months, and it may take 3-4 months, if ever. For that reason, I only use cimetidine for unusual cases, such as children who are impossible to otherwise treat, and cases involving so many verrucae that it makes you (the podiatrist) ill.

From just reading the PM News article, I thought Dr. Innocenzi treated patients in a nursing home, but billed as if he saw them in his own office, a separate location, which allowed him to bill at a higher rate, and collect way more money, to cover his "rent, utilities, and insurance premiums" (Does Medicare reimbursement really cover those things?). In the original quoted article, it states "According to the bill of information, the Grand Junction nursing home where Innocenzi claimed to have an office was really only a storage room/beauty salon where he was allowed to perform podiatry services rent free for three months. Under the original bill of information, Innocenzi could face ten years in prison, a $250,000 fine, or both"
 
I don't really think his intent was true fraud. I feel sorry for him. How many other podiatrists have done this also? What codes should he have billed with? What was the place of service he should have used? The nursing home probably gave him a location, a spot to see patients that could be seen in that setting. It is sure easier than treating patients in their beds. Does anyone else think this is a little overboard to be considered fraud? He did this for three months and can face ten years in prison? A $250,000 fine? Or both?

Wise move. Making a deal when the prosecutors have "information" typically is translated to mean that this podiatrist will receive relatively less harsh penalties than facing a deal after a federal grand jury indictment. We have all seen the results of podiatrists trying to fight these types of charges.
