AT THE COLLEGES
Kent State’s College of Podiatric Medicine Names New Dean

Allan M. Boike, DPM, FACFAS, has been named dean of Kent State University’s College of Podiatric Medicine. Boike currently serves as the director of the Podiatric Medical and Surgical Residency Training Programs and section head of Podiatric Medicine and Surgery in the Foot and Ankle Center of the Orthopaedic and Rheumatology Institute at the Cleveland Clinic. His primary practice locations are at the Cleveland Clinic’s main campus and the Strongsville Family Health Center.

Dr. Allan M. Boike

“Dr. Boike is a nationally known podiatrist and educator who understands the future of podiatric medicine, and who is committed to steering the Kent State University College of Podiatric Medicine to the top of all podiatric schools in the country,” said Todd Diacon, Kent State’s senior vice president for academic affairs and provost. “We are fortunate to have someone with Dr. Boike’s skill, experience, and reputation for excellence joining Kent State University."

INTERNATIONAL PODIATRISTS IN THE NEWS
Cost of Diabetes Care Expensive - Barbados Podiatrist

Diabetes is a costly disease. “When we talk about the cost of diabetes, we never hear the figures that are attached to diabetes care,” says Dr. Simone Mc Connie, Chief Operating Officer at the Barbados Diabetes Foundation. “As a podiatrist, I see a lot of diabetics with foot problems, and if that individual needs to have a dressing done three times a week at $60-$70 each time, and that person is earning $500 per week – you realize how expensive it is."

Dr. Simone Mc Connie

"Fifteen per cent of persons who are diabetic will have a foot problem, and 15 per cent of those people will be at risk of an amputation. The other thing we don’t recognize is that there is a lot that can be done to prevent that amputation, and the challenge we have in our society is that not enough of that is done,” said Dr. McConnie.

PODIATRISTS IN THE COMMUNITY
VA Podiatrists Help Salvation Army Collect over 7,000 Shoes

The Salvation Army collects shoes all year long and local businesses host shoe drives. “Dr. Samuel Schustek and I have been doing this together since 1999,” says Dr. Bryan Snyder, Charlottesville podiatrist. “Every year we put a lot of time and preparation into it and we're really thankful that the area is so generous.”

Dr. Bryan Snyder

This year, over 7,000 shoes were donated for the event, half of what was donated last year, but any amount is better than none.

QUERIES (MEDICAL-LEGAL)
Query: Stark Laws and Diabetic Shoe Certification

Our office dispenses quite a few diabetic shoes, and does the Medicare billing for them. One of the referring doctors (PCP) says he is not comfortable filling out the certification form. He says he fears violating Stark Laws, especially since he does not want to have to sign off on any type of podiatric diagnosis. He is also concerned that there will be a Stark Law violation because a podiatrist is prescribing and dispensing the same product.

Can anyone give me some documentation to present to this doctor that is concise and from a reputable source that will convince this doctor that he is not at risk by signing the certifying letters?

QUERIES (NON-CLINICAL)
RE: Billing Equality
From: Alan Mauser, DPM

Recently, I performed a bunionectomy with a double osteotomy, and an osteotomy of the fifth metatarsal on a 16-year-old female. The patient's mother asked me if I knew how much the hospital billed and then again how much the hospital collected for the surgery.

The hospital billed $37,332 for which they were approved $30,559. The patient was there a total of four hours. I, on the other hand, received from the insurance company based on my $1,858 fee a whopping total of $588. Needless to say, I was astounded.

The following  questions are posed:
1. Is this normal practice and reimbursement for hospitals?
2. Are all physicians (orthopedists vs. podiatrists) paid the same?
3. What can be done to ensure a reasonable fee-for-service?

Alan Mauser, DPM, Louisville, KY

CODINGLINE CORNER
Query: Laser Wound Debridement

Does anyone know if there is a specific separate and payable CPT code for the use of a laser post-debridement of a wound? If not, what is the CPT code that one would bill for when a laser is used in conjunction with debridement?

David Zuckerman, DPM, Cherry Hill, NJ

Response: The use of the laser after or during wound debridement would be bundled into the code for the debridement itself. Just use the wound debridement code, presuming you are performing a wound debridement, CPT 97597 or CPT 1104x.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: Recertify the American Board of Podiatric Public Health
From: Leonard A. Levy, DPM, MPH

The time is long overdue for podiatric medicine to reassess some of its longstanding rhetoric which contributes to the isolation of the profession. Two examples include:

Preventive Medicine and podiatric public health– public health in podiatric medicine includes not only foot health but also the total health of the public (e.g., serving major leadership roles in public health institutions and agencies).

Routine foot care is a major part of practice - there is (or should be) no such thing as “routine foot care” any more than there is routine medical care. Providing less complex podiatric medical services should also include examinations that may identify people at risk for podiatric as well as general medical problems (e.g., elevated blood pressure), resulting in the integration of podiatric physicians into the interprofessional health team.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Robert Wunderlich, DPM

I remember the first time I picked up a copy of Podiatry Management as a 1st year student at CCPM in 1992. One of things I remember reading in that issue of PM was the idea of a curriculum and degree change from DPM to MD. Over 20 years later, we're still just talking about it.  It's time to stop talking and start walking.

RESPONSES/COMMENTS (PM NEWS QUICK POLL)
From: Keith L. Gurnick, DPM

As a 4th year podiatry student in 1979 at CCPM, I did an externship in the office of Dr. John Weed, who practiced in San Jose, CA. He was one of my favorite biomechanics teachers and my expectation was that during the two weeks in his office, I would improve only my biomechanics skills by observing him performing range of motion exams, muscle testing, gait analysis and casting, Rx writing, and dispensing orthotics on his patients. Because this was his main teaching subject, I naively figured that's what he probably did exclusively in his private office practice.

He had many satisfied and happy patients, and he was a well-rounded podiatrist of his time. Sure he did biomechanics, but he also clipped toenails, he trimmed calluses,...

RESPONSES/COMMENTS (PM NEWS QUICK POLL) - PART 1B
From: Steven J. Kaniadakis, DPM

What do the topics of a three-year surgical residency, the results of the recent survey about the percentages of podiatrists doing RFC, and the past survey about percentages of DPMs performing bone versus soft tissue surgeries indicate about today's podiatrist? Are we training our residents for the vision we wish to achieve or for the realities of today's podiatrist?
