Wearing High Heels While Exercising Not a Good Idea: NY Podiatrist

Stiletto fitness classes seem to be this year's newest fad, but is it safe to wear high heels when exercising? "It depends on how high-impact the class is, but as a very general rule, it's not the best idea," says New York City based podiatrist Dr. Hillary Brenner.

Dr. Hillary Brenner

"If you're jumping up and down, you're forcing all your body weight to rest on the balls of your feet which can result in stress fractures and ligament damage. You can also hurt your knees and your lower back. In some cases, it can be a total body disaster."

PODIATRISTS IN THE COMMUNITY

VA Podiatrists Co-Sponsor Shoes for the Homeless Collection

On Saturday, if the shoe fits, people can wear it for free. After a month of collecting used but useable shoes, Shoes for the Homeless volunteers sorted through the donations for the needy on Friday afternoon. About thirty volunteers took about three hours to sort around 3,500 pairs of shoes.

Drs. Bryan and Heather Snyder

Shoes for the Homeless is an annual campaign, now in its third decade. It is a joint effort between the Salvation Army and local podiatrists. Organizers of the Shoe Drive say the recipients are typically very grateful. "They're all put to good use. We spend the whole night previously stacking piles of men's and women's and kid's shoes, they're all available, and we organize them and then open the doors the next morning and say 'come on in, help yourself' and we distribute bags and bags of shoes," said local podiatrist, Dr. Bryan Snyder.

MEDICAL MALPRACTICE

Full Disclosure Appears to Lower Malpractice Legal Costs

Preliminary data is starting to come in from an Agency for Healthcare Research and Quality-funded $3 million demonstration project in which the University of Illinois Medical Center at Chicago's “Seven Pillars” program has been expanded to 10 other hospitals. And it shows that the program's promotion of full disclosure of injuries and errors, apologizing and offering compensation appears to be having a positive effect on legal costs.

One of the foundations for disclosure-apology-offer programs is the research into why people sue their doctors. This was conducted by Dr. Gerald Hickson and James Pichert at Vanderbilt University Medical Centers' Center for Patient and Professional Advocacy. “Families want answers,” Hickson says. “If something bad happens or something unexpected happens to your loved ones or yourself, you want to know why.”

The traditional stonewalling that occurs with the deny-and-defend response to a medical injury often triggers malpractice suits, Hickson's and Pichert's research found, because suing was the only way to get answers to their questions. “You want an expression of human concern as an apology—not laying blame,” Hickson says. “You also want an explanation. How did this occur? What does it mean to me? How am I going to respond to this ongoing threat to my health?”

RESPONSES / COMMENTS (DME)

RE: DME and Diabetic Shoe Payment Problems
From: Michael M. Rosenblatt, DPM

A number of years ago, our Washington State Podiatric Medical Association had some payment issues concerning diabetics. At the time, I was Chair of the Insurance Committee (which has since been renamed). I contacted the president of the Washington State Diabetes Association, who was so alarmed by our predicament that she gave me her home phone number. These people are active politically to defend the rights of diabetic patients. I was astonished at how hard she worked with us to "correct" our problems.

If podiatrists are having a hard time getting paid for DME for diabetics, no matter what you prescribe, your local diabetes association is definitely an asset. I would go further and say they WANT to be informed of the problems you are having. Naturally, they are concerned about over-use or downright fraud, but they are more concerned that diabetic patients get the treatment they need. They already know that you are a very important part of that equation.

You will not have to convince them. All I said was, "I was chair of the Washington State Podiatric Medical Association Insurance Committee." That was all I needed to get them on our side. It should probably be done on the State Association Level. In our case, the president of their association volunteered to testify before a State Legislative Committee in our behalf.

RE: An App for Podiatrists
From: Steve Kominsky, DPM

Recently, I had the opportunity to use an App called QScripts. This app, which can be loaded on to most mobile devices, allows the user to connect with a compounding pharmacy (QMedRx) that specializes in servicing podiatrists. From the app, the user can prescribe and order home IV therapy (antibiotics and TPN), compounding medications, and custom wound care kits and supplies. The pharmacy works with the doctor in customizing the "menu" of compounding options so that you can have all of your favorites custom-made in the formulations that you want.

I appreciate the time-saving factor since I can place the order from my iPad instead of filling out the paperwork and faxing it in to the pharmacy. This app is suited for a busy office setting for sure, but also would come in handy for the in-hospital patient being prepared for discharge and needing an out-patient infusion. The Rx and order can be done on the mobile device, in the hospital when rounding on the patient.

My experience so far is that it is very user-friendly and incredibly time-efficient. I just wanted to share this with my fellow readers of PM News. The contact information is Bpowers@qmedrx.com

RE: An Inexpensive Chair/Table
From: Michael J. Schneider, DPM

I retired from practice in 2011, but was recently offered a space in Denver. I did not have a chair and everything I saw for sale was just too expensive. I then came across this chair/table sold by The Massage Table Store (themassagetablestore.com). This chair/table can lie flat, and rises and lowers with a hydraulic foot pedal. The chair/table costs $375 and the stool is $45, including shipping. It is perfect for a satellite office.

Disclosure: I have no involvement with this company.

RE: Kudos to Paul Kesselman, DPM (Eddie Davis, DPM)
From: Hal Ornstein, DPM

I would like to echo the comments of Eddie Davis, DPM about Paul Kesselman, DPM. Durable medical equipment and diabetic shoes have become an integral part of achieving best outcomes and quality for our patients as well as adding to the bottom line. I get to see much behind the scenes and the many hours of dedication and passion to help our profession that Paul has given. Paul, simply stated, thank you!

RE: CA Podiatrist Discusses Treatment of Plantar Tears
From: Jeff Mennuti, DPM

Remember, this is sports medicine. I don't think insurance and billing would be an issue. In this scenario, PRP injection, or injection with amniotic stem cells would count as a procedure.

I have seen excellent results with PRP. The problem, as usual, is insurance coverage. I am curious if anyone else has had success in getting coverage and clinical results with PRP.

RE: #1 Ranked Golfer Got His First Clubs From His Podiatrist Mother

British golf pro Lee Westwood is making his first start on the PGA Tour, and this week is a reminder to the 39-year-old Westwood where his career all started. It was his mother, a podiatrist, who bought a set of golf clubs from a patient in case her son wanted something to do while on holiday from school. It was his father, a math teacher, who had the idea of going to the golf course for the first time.
