OHI & ACFAOM Establish Educational Partnership

OHI, a leading manufacturer in North America of custom, lower extremity orthotic devices and related products, and the American College of Foot & Ankle Orthopedics & Medicine (ACFAOM) today announced a joint educational partnership whose mission is to support the podiatric profession at large and to advise on the impact of the rapidly shifting healthcare industry. The partnership is built around a $120,000 education and advocacy grant from OHI for programs that will support biomechanics and the podiatric profession over a four year period.

ACFAOM is a national organization of highly educated and trained podiatric physicians dedicated to assuring that primary podiatric medicine, broadly defined as orthopedics and medicine of the lower extremities, is the cornerstone of contemporary podiatric practice. It believes that collaborating with OHI is a powerful stride forward and an important evolution for the 70-year-old organization.

PODIATRISTS IN THE COMMUNITY - PART 1
For 8th Year, NY Podiatrist Supports Local Shoe Drive

For the eighth year in a row, Dr. Bruce Pinker of Progressive Foot Care in White Plains, invited his patients to donate their lightly used shoes and boots to support a shoe drive for Westchester’s men, women, and children in need. They responded by donating almost 400 pairs of shoes and boots in all shapes and sizes, many of them new.

The shoes were donated to Lifting Up Westchester, a Westchester County non-profit social services agency formerly called Grace Church Community Center, for distribution to individuals from their Open Arms Men’s Shelter, Samaritan House Women’s Shelter and Soup Kitchen. Pictured are students from The Pleasantville Laboratory School at Pace University with Dr. Pinker, who volunteered to sort the shoes.

MEDICARE NEWS
Medicare Providers Mull Pay-for-Performance Options

A number of healthcare providers are deciding whether to join an accountable care organization (ACO) or to be part of Medicare’s Merit-Based Incentive Payment System (MPIS), Healthcare IT News recently reported. Included in the 2015 Medicare Access and CHIP Reauthorization Act, MPIS is set to begin in 2017. In the meantime, providers are anticipating rules, which should be finalized by November 2016 and could come as early as spring 2017, the article reported. "People are waiting with bated breath for MIPS rules, just like they were for meaningful use," said Tom Lee, CEO and founder, SA Ignite, which develops analytics tools to help providers manage value-based programs. "The biggest thing on the horizon is there are a lot of details on this that will come to light when the rules come out."

“When choosing a program, providers must assess the risk and whether they want the guaranteed 5 percent from ACOs or choose MPIS, where the value-based swing between the lowest and highest reimbursement could be up to 36% when accounting for bonuses. MIPS has higher bonuses, but higher risks," Lee told the publication. "Providers don't need to choose a physician group to be a part of the program, but those providers who don't remain on top of the heap lose money,” Healthcare IT News reported.

HIPAA UPDATE
Unencrypted Emails of PHI OK if Patient Assumes the Risk: OCR

Because “emails are generally considered readily producible by all covered entities,” The Office of Civil Rights (OCR) says in its newest guidance, the agency expects that “all covered entities have the capability to transmit PHI by mail or email (except in the limited case where email cannot accommodate the file size of requested images), and transmitting PHI in such a manner does not present unacceptable security risks to the systems of covered entities, even though there may be security risks to the PHI while in transit (such as where an individual has requested to receive her PHI by, and accepted the risks associated with, unencrypted email).”

Deven McGraw, OCR deputy director for health information privacy says the use of unencrypted email, if requested by the patient, isn’t a “slippery slope” toward a free-for-all of abandoning safeguards or extended communications between patients and providers. The use embodies “exercising the HIPAA right of access.”

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Bryan C. Markinson, DPM, Roodabeh Samimi, DPM

Consider cutaneous larva migrans.

I recommend a biopsy. It looks fungal or tapeworm-like (parasitic).

RESPONSES/COMMENTS (CLINICAL) - PART 1B
From: Jeffrey Kass, DPM

I am sure many of my astute colleagues will be able to identify this condition based on the picture and information provided. I am just curious why an unknown dermatological lesion wouldn't get a skin biopsy as part of the work-up? The skin biopsy would lend more information than the blood work-up.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: House Passes H.R. 3016, VA Provider Equity Act
From: Evan Meltzer, DPM

I am pleased to announce that H.R. 3016, also known as the VA Provider Equity Act was passed today by the House. Thanks to all who wrote to their Congressmen in support of this important bill. The next hurdle is for the Senate to pass S.2175, which is essentially the same Bill. As before, I urge all of you to write your senators in support of this Bill. If passed, it goes to the President for his signature, and the VA then has 30 days to implement the significant pay raise and physician equivalency for all VA podiatrists. If passed, this Bill will no doubt have a positive effect on all federally-employed podiatrists.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
RE: Podiatrists Joining Orthopedic Groups
From: Ira M. Baum, DPM

Podiatrists joining orthopedic surgical groups confuses me. I can't understand why an individual trains to become a podiatrist, then joins an orthopedic group. What motivates these individuals? Is it the money? Orthopedists receive higher reimbursement for equal services, so maybe that's the draw? Is it prestige? You know, running with the "big boys."

It's almost insulting to podiatrists. Were some of these students unable to get into medical school to become orthopedists? Or is it because...

Editor's note: Dr. Baum's extended-length letter can be read here.

RE: Outrageous Shoe of the Day

Here's what happens when an architect designs a shoe.

Originally being an architect, Jayda Hany seeks to enhance the structural strength of catwalk shoes to make them more wearable. Her method was deconstructing the internal components of the shoe by exteriorizing impact on walking gait through exploring structural architecture such as truss systems and load distribution.
