Ballet Shoe Fad is Causing Rise in Ingrown Toenails: UK Podiatrist
 
They're seen as a sensible yet stylish alternative to killer heels. But experts have warned that ballet pumps can lead to a host of health problems, from calluses and corns to collapsed arches. Now, a leading podiatric surgeon says the trend for ballet pumps is causing a rise in ingrown toenails.

Mike O’Neill, a consultant podiatric surgeon and spokesman for the Society of Chiropodists and Podiatrists, told MailOnline: "Ballet pumps put a lot of pressure on a toenail, and the pressure means the nail bends and becomes ingrown. There’s been a big rise in that in the last five years." Left untreated, an ingrown toenail can lead to infection of the bone and, in a worst case scenario, a person might have to have their toe amputated, he said.

OH Podiatrist to Enter Plea in Healthcare Fraud Case
 
A Canfield podiatrist who is accused of healthcare fraud is expected to plead guilty to a federal charge this week. In December, the Assistant U.S. Attorney in Denver issued a bill of information against 45-year-old Dr. Anthony Innocenzi, who is licensed to practice medicine in both Ohio and Colorado. Federal prosecutors allege that Dr. Innocenzi billed Medicare for treating patients at two offices in Colorado. However, the Assistant U.S. Attorney says that the treatment was not administered in offices, but at nursing homes in Grand Junction and Trinidad, Colorado. Prosecutors say Dr. Innocenzi was able to collect larger reimbursements from the government by claiming that the treatment took place in an office because Medicare compensates providers who administer treatment in offices at higher rates to cover costs such as rent, utilities, and insurance premiums.

Michael Brody: It is the risk that exists to the data in your office, regardless of whether it is in your computer or on a paper chart. Some of the risks are that data: 1) could be stolen, 2) seen by people who shouldn't, 3) damaged, 4) destroyed 5) present risks to the confidentiality of the data, 6) present risks to the integrity of the data, or 7) present risks to the availability of the data. Each doctor needs a plan on how to deal with these risks.

Query: Cimetidine for Plantar Warts
 
I am currently treating a 15 year old male for plantar warts using cimetidine 200 mg BID. It has been a week and a half now since he has been on the medication, and I don't see much improvement. I know that he has not reached the maximum daily dose. Can someone share their experiences treating warts with higher dosages, i.e. more than 400 mg daily. How long does it take before you see any results?

Query: Digital Block for Corn Debridement
 
A patient presented to the office with a painful dorsolateral corn on her right 5th digit. I attempted to debride it, however, it was so tender, she required a digital block prior to debridement. I was considering using CPT 64450 (injection anesthetic agent, other peripheral nerve or branch), but when reviewing the Medicare LCD, I didn't find an appropriate ICD-9 code to justify the injection.

Response: Performance of a procedure (e.g., CPT 11055) that usually does not require local anesthesia in rare cases, like you stated, may require administration of local anesthesia. In those relatively rare cases, the anesthesia would be included in the procedure. I do not recommend billing the injection as CPT 64450.

I recently came across an interesting Op-Ed piece for the Seeking Alpha Investment website. Here is an excerpt:
 
MiMedx management has repeatedly made reference to the "published studies" which tout its product. Yet, it should be clear from the demographics of the 13 patients who were treated in a single site that these studies possess predictable marketing value to a much greater extent than they do scientific rigor.
 
One might wonder what sort of doctor/scientist would put his name on such a tiny and skewed study. The name of the lead author was Charles Zelen, and it turns out that he was the lead author on three of the six "clinical publications" that are listed on MiMedx's website. The other three were co-authored by Donald E. Fetterolf, who happens to be chief medical officer at MiMedx. These six studies are the only "clinical publications" listed on the MiMedx website.
 
The point is that it is very easy to write a "study" and submit it to a specialized journal such as the International Wound Journal or the Journal of Wound Care. The study will, of course, be published. And then companies such as MiMedx get to make simple statements touting their publication in these journals. It is easy and guaranteed marketing material, even when it lacks the scientific rigor of legitimate FDA trials.
 
After reading the entire article, I think this represents a cautionary tale for physician investigators and their industry relationships.

What are people thinking? What they've always thought - that all doctors are rich and can either afford to wait to get paid, or that they should forgive those pesky deductibles, co-insurances, and co-pays. What's wrong with those money-grubbing doctors anyway? Don't they know we have real bills to pay? 
 
Doctors don't have bills. I've actually had patients tell me that they think that doctors don't pay health insurance premiums because "you guys take care of each other." The only people who pay their bills promptly are the ones who run their own businesses, as they know what it costs to keep one's doors open. The average working Joe or Joan has no clue how much we pay for rent, utilities, insurance (liability and malpractice), supplies, staff, etc., and most care even less. After all, we're all rich - in my counting house, counting all my (deductible) money.

Any patient who protests about paying their deductible and co-pays is not worth having as a patient. You may want to check your contract with the third-party payer as there may be nothing wrong with collecting something from the patient as a "down payment" on their deductible. 
 
Anyone who has such a high deductible ($3K), and has not met any of it, certainly is going to have to pay something. If you have the fee schedule, it is certainly much easier to...
