Amol Saxena, DPM of Palo Alto, CA was recently a guest instructor for Stanford University Medical School's course "Anatomy in Society." He was asked to critique undergrad students' projects on foot and ankle anatomy, gait, and footgear.

Saxena recently published a Level II study on comparing ESWT, Placebo ESWT, and EPF surgery for plantar fasciitis on athletes in the Muscles Ligaments and Tendons Journal. The ESWT patients were part of a nationwide Level I study sponsored by Storz Medical AG previously presented at the AAOS Annual meeting, which will be published in another journal.

Court Inundated With Letters of Support for IN Podiatrist Convicted of Embezzlement

Fifty family members, longtime friends, former podiatry patients, neighbors, business associates, and political leaders have written letters to a federal judge in support of former Lake County official Tom Philpot seeking leniency as he faces sentencing on five counts of embezzlement. His sentencing hearing is scheduled for Feb. 21. He faces up to 20 years in prison. Philpot, who served multiple terms as Lake County clerk and coroner, was convicted Aug. 3, 2012, of embezzling $24,000 from county government by illegally giving himself salary bonuses between 2004 and 2009 while serving as clerk. He took the money from a federal fund earmarked to improve the collection of court-ordered child support payments.

I recommend looking into some form of rheumatological problem or some form of neurological problem. I did a Web search on spontaneous hallux varus and Pubmed came up with an abstract of 5 cases by Grandberry on "Idiopathic Adult Hallux Varus" in Foot Ankle Int. (1994 Apr; 15(4): 197–205). Jahss published two cases of spontaneous hallux varus in Foot and Ankle International, January 1983. In Jahss' article, one of the patients had a past history of polio. Look at these articles. They may assist you.

A lot of neurological conditions can be helped by foot surgery. A modified lateral wedge Reverdin-Isham-Akin osteotomy, along with angulated and/or lateral wedge proximal phalangeal base osteotomies for the second, third, and fourth toes would correct these deformities. This would allow the patient to wear normal shoes. The advantages of these procedures are that no fixation is necessary, and they can easily be performed under fluoroscopy using minimally invasive foot surgery.

Paul Kesselman, DPM is to be commended on his work on behalf of podiatric medicine and his assiduous attention to the plethora of regulations required by CMS. It would be good if we could clone Paul and place a version of him in each of our offices to assist in the adjudication of Medicare/DMERC claims and ensure compliance with the complex web of regulations forced upon us. The fact of the matter is that there is a cost associated with such efforts to comply, and when such costs are added to existing overhead at a time of declining reimbursements, one must decide if it is worthwhile to continue such services.

RE: CA Podiatrist Discusses Treatment of Plantar Tears

In 2004, Brian Fullem and I published a paper on plantar fascia tears in the orthopedic journal, American Journal of Sports Medicine (which has the highest impact factor in orthopedic journals): ncbi.nlm.nih.gov/pubmed/15090382. The average return to activity was 9 weeks, with no athletes (including professionals) requiring surgery when following our treatment regimen. This is an often referenced article in the orthopedic community.
 
I highly recommend that all "foot and ankle specialists" read this. Anecdotally, I know of one professional baseball player who tore his plantar fascia and had it repaired acutely. He eventually had it released. Incidentally, prior to that, his other foot also had a release for plantar fasciitis .
 
There is a recent chapter "Plantar Fascia Injuries" we just published in Foot and Ankle in Sports Medicine edited by David W. Altchek, M.D, Jonathan T. Deland, M.D., Rock Positano, DPM, MSc, MPH, and Joshua Dines, M.D.,(Lippincott, 2012) that highlights the current evidence-based treatments, including indications for surgery. Acute plantar fascial injuries are not one of them.
