Running in Worn Sneakers Can Cause Over-use Injuries: VA Podiatrist
 
Sneakers weren’t meant to last forever. As they wear down, the cushioning and support they provide diminishes, which could put you at higher risk of injury when you run or walk for exercise. Ayne Furman, DPM, writes for the American Academy of Podiatric Sports Medicine (AAPSM) that one situation in which shoes should be replaced is when the outer sole has worn through to the midsole.

“The midsole is intended to be shock absorbing and in some shoes serves to control excessive foot motion. After certain amount of repetitive load is placed on the midsole, it will compress, not rebound and absorb shock or control the foot as well as it did when new. In some cases, the midsole can deform and compress unevenly, which can create an alignment change of the foot. This can lead to over-use type injuries.” In general, though, how often you should replace your sneakers depends on how often you use them, how old they are, and what signs of wear they have, according to the AAPSM.

Brett Waverly, DPM recently joined the podiatry team at The Corvallis Clinic. Dr. Waverly received his podiatric medical degree from the Western University of Health Sciences in Pomona, CA. He then completed residency training at Regions Hospital in St Paul, MN, a level I trauma center and a teaching affiliate of the University of Minnesota.

Henry K. Stark, DPM has opened a new practice in Lake Park, FL. Dr. Stark received his podiatric medical degree (with honors) from the New York College of Podiatric Medicine in 1978 and completed two years of post-graduate training at Pittsburg hospitals.

Dr. Stark moved to Florida to begin his career in Lake Park as an associate. Six years later, he opened his own practice. For the next 35 years, Dr. Stark expanded his practice, hiring associates and forming a partnership, eventually encompassing four offices in the northern half of Palm Beach County. After selling his practice in 2015, Dr. Stark had retired but has now returned to his original location.

Prepare Your Feet for Winter Dangers: Canadian Podiatrist 
 
Whether it’s a day at the office in high-heels or hauling the sled up the kids’ favorite toboggan hill – over and over and over – your feet can take a beating in winter. The implications of that range from the uncomfortable to the downright dangerous, says Darren Woodruff, DPM of Central Alberta Podiatry.

While snow can be beautiful, ice and snow can be dangerous for feet and ankles. And if the temperature drops after a warm spell, remember that there’s likely ice under the white stuff. Cold temperatures also make you vulnerable to frostbite, which continues to be a concern each year, Dr. Woodruff says. If you’ll be outside for any length of time, layer on the socks and wear good, warm boots so you don’t risk losing toes.

2021 Remote Patient Monitoring Changes:  Key Takeaways - Part 5
 
In December, the Centers for Medicare & Medicaid Services (CMS) released its 2021 Medicare physician fee schedule final rule. Within the final rule were some significant changes concerning remote patient/physiological monitoring (RPM) and the provision of other remote services. Here are the most significant takeaways from the final rule.
 
5.  Approved Furnishing of Services by Auxiliary Personnel
 
CMS established a permanent policy that allows auxiliary personnel to furnish CPT 99453 and 99454 services under a physician's supervision. CMS has previously defined auxiliary personnel to mean "… any individual who is acting under the supervision of a physician, regardless of whether the individual is an employee, leased employee, or independent contractor of the physician, or of the legal entity that employs or contracts with the physician."

RE: Hospitalizations for Diabetes-Related Foot Ulcers on the Increase

Hospitalizations for diabetes-related foot ulcers (DFUs) are increasing. A study reported in Diabetes Care (Jan 2021, Hamilton, EJ, et al.) concluded that the incidence of diabetes foot ulcer (DFU) hospitalizations complicating type 2 diabetes increased, especially in younger participants and were more likely in those with peripheral sensory neuropathy, peripheral arterial disease, and suboptimal glycemic control.
 
Age of type 2 diabetes diagnosis (inverse), HbA1c, insulin use, height, urinary albumin/creatinine, absence of any foot pulse, previous peripheral revascularization, and peripheral sensory neuropathy were independent predictors of incident hospitalization for/with DFUs.

QUERIES (MEDICAL-LEGAL)
Query: When an Insurance Company Wrongly Sends a Check to a Patient
 
We evaluated and treated a patient for an auto claim with State Farm. The patient is on disability. State Farm issued payment for our services directly to the patient. He has refused to respond to messages, statements, etc. The insurance agent claims to have no recourse in getting us paid for this service. Can we pursue payment directly from the agent? What recourse do we have?
 
Editor's comment: PM News does not provide legal advice. Since the contract for services was between you and the insurance company, your first course of action is to commence legal action against the insurance company. If the amount is relatively small, your best bet may be small claims court. Filing fees are usually nominal. You should add the patient as a third-party defendant. When the patient receives the summons, you will likely be paid as courts generally do not look favorably on such unjust enrichment.

Dr. DiResta nailed it on so many aspects of this difficult, confusing topic; most importantly, on the best, simplest strategy for achieving true parity - changing the licensure of DPMs to a plenary, unrestricted license in all 50 states, and the need for podiatric medical students to take and pass a national medical licensing exam such as USMLE or COMLEX.
 
Although many readers are respected and treated as equals by their medical and osteopathic colleagues, there still exists many, many types of unfair, discriminatory codes and statutes that restrict our ability to practice to the full extent of our education and training. As a specialty, we may have achieved parity in our personal practice lives but we are still licensed far below our place in the house of medicine. Dr. DiResta is also spot on regarding the level of knowledge and training of recent graduates of 3-year residency programs. Podiatrists are the only group that completes a 4-4-3 model of education besides MD and DO. The final product of that model is indistinguishable from an MD and DO within their chosen field of specialty. 
 
And finally, MD=DO=DPM is achievable. The only way that a podiatrist will ever get an MD or DO degree is to graduate from an MD or DO medical school. Working to change our licensure as DPMs to a plenary, unrestricted license retains our unique identity and perspective as foot and ankle specialists, while allowing us to practice consistent with our education and training.

There was a time when I expected “a degree of consistency” for American medicine and licensing (pun intended). I was extraordinarily naïve. It took the irregularities of the pandemic to prove that to me. I actually EXPECTED the U.S. healthcare system to even-handedly handle Covid and allow allopathic physicians to prescribe and follow their own internal scientific guidance. Well-known medications were politicized and forbidden, no matter your license.
 
I just wrote three sentences and deleted them because the Editor can’t print them. That tells you everything you need to know. Consistency and loss of freedom of speech were the first casualties of the pandemic. Consistency in licensure no longer applies to MDs and DOs. Why would we expect it to exist for us?
