Ankle Replacement By FL Podiatrist Allows Retired Soldier to Resume Walks

Daily walks were put on hold in 2009 when retired Army Sgt. Maj. Mike Mead could no longer suppress the pain in his left ankle. A total ankle replacement, one of the first done in Bay County in the past decade, has given the career soldier relief from the pain and a return to his walks. Dr. Shayne Jensen of Gulf Coast Podiatry Foot and Ankle Surgery Center, who performed the ankle replacement, said Mead’s pain was caused by a lack of cartilage in his ankle joint, likely caused by osteoarthritis. In July 2010, Mead was implanted with the INBONE Total Ankle Replacement from Wright Medical.

Jensen performs 10 to 15 ankle fusions a year, which he said is still the “gold standard” for treating ankle problems. An ankle fusion uses pins and plates to fuse the ankle joint, and patients lose range of motion in the ankle joint. The total ankle replacement uses an artificial prosthesis to offer patients better mobility.“It looks good, it healed well and it works,” Jensen said.

PA Podiatrist Named as Residency Genesis Coordinator

The William L. Goldfarb Foundation has announced that Dr. Lee J. Sanders, whose career has been directly involved in the creation, direction and expansion of residency programs for Doctors of Podiatric Medicine, has agreed to assume the position of Residency Genesis Coordinator under the Residency Genesis Project created by the Foundation.

The Residency Genesis Project was established in October, 2010 in response to the projected shortfall in residencies that has been identified by the Balance Committee of the American Association of Colleges of Podiatric Medicine. Within its first three months of operation, the Project has begun working with forty-eight different prospective residency directors privileged at hospitals located in twenty-nine states.

NC Podiatrist Appointed Chief of Surgery

The North Carolina Foot & Ankle Society has announced the appointment of Dekarlos Dial, DPM, as Chief of Surgery at Thomasville Medical Center in Thomasville, North Carolina. In addition to his private practice responsibilities, Dr. Dial serves the North Carolina Foot & Ankle Society as Seminar Chair.

Dial is a 2003 graduate of the Ohio College of Podiatric Medicine. He is Qualified in Foot Surgery and Reconstructive Rearfoot/Ankle by the American Board of Podiatric Surgery.

ONC to Track Nationwide Health IT Use

The Office of the National Coordinator for Health Information Technology at HHS has awarded two contracts totaling more than $3.9 million to monitor the nationwide use of health information exchanges and adoption of electronic health-record systems.

Surescripts, an Arlington, Va.-based vendor of electronic prescribing services and, more recently, an information exchange portal and an ONC-authorized health IT testing and certification body, was awarded a contract valued at more than $1.4 million. Surescripts is to collect, report and analyze "ongoing evidence of nationwide electronic exchange of clinical information, such as electronic prescribing," and submit reports at least quarterly, according to a contract description posted on a federal website.

IFAF Hosts Successful Seminar at Sea

The International Foot & Ankle Foundation (formerly the Northwest Podiatric Foundation) hosted a 7-day Eastern Caribbean Seminar at Sea aboard one of the World's largest cruse ships, the OASIS of the Seas, on January 15-22, 2011. The cruise out of Fort Lauderdale, had port stops in the Bahamas, St. Thomas, and St. Maartin, with three full days at sea at which time the 25 CME program was offered.

Speaker Faculty for the IFAF January Oasis Seminar at Sea Conference, 2011. Drs. Brian McInnes, Chris Reeves, Sean Grambart, Amber Shane, Rex Nilson, Mary Crawford, Wayne Bakotic, Ed Glaser, G Dock Dockery, and William Healey.

The dual screened, high-tech, two-sided conference room on the Oasis seated 50 attendees for the Seminar at Sea meeting. The IFAF offers several high-quality scientific seminars annually in great venues around the globe.

Query: Low-Cost Lasers?

I recently attended the SAM conference in Orlando, Florida in January and heard the last part of a lecture from Elliot Udell. He spoke about low-cost lasers that are in the works and could be available in the near future. I am presently looking at several lasers ranging from $25,000-$60,000. Has any other members heard any news of these low-cost lasers?

Before amputation or any surgery, this patient should be evaluated proximally for popliteal or ilio-femoral anatomic pathology or occlusive disease, whether by MRI angiogram or standard angiogram. That this hemangioma is a de nova genetically aberrant system should not be assumed. With such a normal right extremity, it seems unlikely it's purely systemic, and all eyes should be on a concomitant arterial problem causing a venous congestion, or a frank primary venous assault, possibly infective.

Also, consider referral to a practitioner familiar with "Avastin" treatment. Perhaps the VEGF inhibition may ameliorate the vasculature. This is being done in the eye for neovasculariztion and of course in cancer treatment. Naturally, amputation should be the last resort, and I would try all "Hail Mary" treatments to save this limb.

Despite Dr. Fisher's descriptions of H&P regulations, can we in good faith believe podiatrists have broad enough training to do a complete H&P? Without even OB/GYN training, or formal cardiac auscultation or ECG training, and I don't mean a podiatric rotation through the department as a podiatry resident. I wouldn't want a podiatrist evaluating my mother. The danger with podiatry in systemic evaluations is that many podiatrists don't know what they don't know. I am a podiatrist and if I were the patient, I wouldn't want a podiatrist determining my fitness systemically. I want an MD performing this. My life is more important to me than giving one up for the "Gipper."

In regard to Dr. Epstein's post, his comments ring true to those of us who graduated about 30 years ago. The profession is attempting to evolve into a surgical specialty, and contrary to what some may wish to believe, bestowing all with a 3-year residency trained status will probably not lead to MD status in the near future. My view is that there is plenty of work out there for a podiatrist not interested in doing hospital surgery. As to whether a 3-year hospital residency is needed for that, my viewpoint is that it is not needed.

The best method that I know of to prevent employees from opening up their own accounts to deposit your checks is to keep track of all adjustments made in the office. Once the charge is put into the computer, the only way to reconcile this amount to "get it off the books" is to either pay it, or adjust it. If adjustments are not looked at, your employees can take the money, adjust the amount so that it does not show up in accounts receivable.
 
Dr. Michaels' suggestion of using a receipt book is excellent.
