Case Series Explores Umbilical Cord Tissue Allografts in Plantar Fascia Care
 
Regenative Labs, in collaboration with Babak Baravarian, DPM, Director of Foot & Ankle Research at University Foot & Ankle Institute, have announced the publication of a new observational case series examining the use of Wharton's jelly connective tissue allografts in patients with plantar fascia defects associated with plantar fasciopathy. "The single best method we've ever used in the care of plantar fasciopathy has been Wharton's jelly umbilical cord tissue allografts," said  Dr. Baravarian, senior author of the study.

The study, titled "Wharton's Jelly Tissue Allografts for Tearing in the Plantar Fascia: A Case Series," was published on September 24, 2025, in Biomedicines, a peer-reviewed, open-access medical journal. The case series reports outcomes from nine patients with plantar fasciopathy based defects who received a single ultrasound-guided application of a collagen-rich Wharton's jelly connective tissue allograft and were followed for 90 days.

Court Rules That Forensic Podiatric Evidence Unreliable
 
A Perry woman convicted of murder will get a new trial after Maine’s highest court ruled that key evidence used against her was scientifically unfounded. At trial, the prosecution used testimony from a podiatrist who compared bloody sock-marked footprints found at the crime scene to prints taken from Brackett while she was incarcerated. The podiatrist concluded that the footprints matched Brackett.
 
Brackett’s legal team challenged this evidence, arguing that forensic podiatry—the comparison of bare or sock-covered feet—is not an established or recognized forensic discipline and therefore should not be admissible. The footprint issue stems from the state calling Dr. Michael Nirenberg, a podiatrist in Indiana, to testify. He said there was a “moderate level of support” that Brackett left the footprints at the scene of the murder. Nirenberg compared photographs of bloody footprints to sock-clad footprints of Brackett once she was in police custody.

Key takeaways include the importance of providing additional information with samples, such as biopsy location, patient history, and prior treatment, along with clear instructions for the pathologist.
 
Ensuring quality specimens are preserved in appropriate solutions is paramount. For instance, gout samples should never be placed in formalin, as it causes degradation of gout crystals. Instead, they should be preserved in a cytolytic solution. Moreover, when conducting a punch biopsy on an ulcer, it's imperative to obtain samples from both central and peripheral areas. Peripheral samples assist in ruling out various types of vasculitis, while central biopsies aid in ruling out malignancies.
 
Additionally, when performing a nail biopsy, such as in cases involving a Hutchinson sign or a black streak extending from the nail matrix, it's crucial to take the biopsy beyond the lunula to where the black line begins. Furthermore, the biopsy should be full thickness to the periosteum to ensure proper Breslow scaling.

If you are being hired as a consultant to perform "claim review", then you should bill the insurance company your hourly rate. You do not need a code to bill them. Require them to pay a retainer upfront, and if you go over the number of hours you designated by the retainer, then notify them that there will be a bill for additional time. If they insist on a CPT code, then consider using CPT 99080-Special reports such as insurance forms, more than the information conveyed in the usual medical communications or standard reporting form.

RESPONSES/COMMENTS (THE FUTURE OF PODIATRY) - PART 1A

The subject of "unlimiting" podiatrists' limited licensure has been debated ever since I opened my first office more than 35 years ago. The old argument was to give podiatrists MD and DO degrees and allow them to choose whether to treat feet or go into some other medical specialty and leave the treatment of feet behind. Dr. Hyrwnak's approach is to broaden the DPM degree, just as DOs can do everything an MD can do, Dr. Hyrwnak's approach would seem to allow a person with a DPM degree to treat the entire body. If I am reading this line of thinking correctly, theoretically, a person with an unlimited DPM degree could do a GI fellowship, orthopedic residency, psychiatry, etc. 
 
The problem is how many doctors with expanded licensure would decide to treat people with foot problems, and doesn't "John J Public" deserve access to a doctor who will treat people with foot problems in a scenario where the majority of graduates might turn their backs on good old foot care? The only fair way for Dr. Hyrwnak's proposal to work would be to moderately but not fully expand the scope of a DPM and monitor each stage carefully before proceeding further.

I am compelled to again comment on this issue because, with all due respect to Dr. Hrywnak's well-versed and possibly well-intended post, to me, it falls into the category of isolationism and not of inclusive parity. 
 
If I may quote from one of his post's paragraphs, "Future-proofing the profession: the line between specialties is blurring. By expanding licensure, podiatrists can respond to emerging needs without outsourcing to other specialists..."
 
In other words, a plenary license will allow a podiatrist to diagnose, manage, and treat anything and everything ailing the patient that presents to their office or hospital's ER with a diabetic foot ulcer (which is the example given in his post). So many hats to choose from, right? In reality, "blurring" would be presenting to the public that the podiatry profession's members can be primary care, internal medicine, cardiology, and foot and ankle surgeons. Respectfully, I think only a fool would engage in such a practice without engaging in a team approach when treating a metabolically complicated patient.
