2 NJ Podiatrists Named to Bergen Magazine's Top Doctors List for 2017
Bergen County is home to some of the most prominent and sought-after physicians in the country. Bergen Magazine commissioned Professional Research Services (PRS) of Troy, Michigan to conduct a peer-review survey of thousands of practicing and active physicians across the county. Doctors were asked to select peers whom they deem are the best within their practice areas and whom they would seek out for particular procedures.

CA Podiatrist is Cellist and Soprano
Music from the Great American Songbook will be required fun listening Sunday at the Dixon United Methodist Church. The Beethoven and Friends Chamber Ensemble will perform selections from the famed composer’s rhythmically hard-to-resist songlist, including “I’ve Got Rhythm” and “Someone to Watch Over Me.” Beethoven and Friends features Lorraine Crozier, violin; Ed Lowry, clarinet; Dr. JoAnn Ross, cello and soprano; Linda Baron, piano; Elizabeth Kuehner, piano.

Ross started her musical career by taking cello lessons as a child. In junior high school, she learned she could sing. Through her years in school, she performed with several different groups: concert, dancing, and marching bands; a youth symphony; a church choir; a capella and madrigal singers. In college, Ross majored in voice and sang in choirs and operas. She trained as a podiatrist, but later studied landscape architecture. She teaches cello and singing at her studio in Carmichael.

Judge Blocks Anthem-Cigna Merger
A federal judge blocked the proposed $54 billion tie-up between national insurers Anthem and Cigna late Wednesday, saying the combination would harm competition in the national employer market. In a 12-page order, U.S. District Judge Amy Berman Jackson said the merger would eliminate the firms' head-to-head competition for the administrative services business of large employers and result in higher prices and reduced innovation in the market.
The judge also said Cigna, which has long shown lukewarm commitment to the merger, is “actively warning against it.” During the trial, Cigna officials presented “compelling testimony” that undermined the projections of future savings that would result from the merger. The two also have deep disagreements in strategy. Anthem attempted to cast these differences as a “side issue,” according to the order.

CMS Extends Deadline for 2016 PQRS EHR Submission
CMS is extending the submission deadline for 2016 Quality Reporting data submission. You now have until March 13, 2017 to submit 2016 EHR data via a registry. The original submission deadline was February 28, 2017.
Providers who do not satisfactorily report 2016 quality measure data to meet the PQRS requirements will be subject to a downward PQRS payment adjustment of up to 4% on all Medicare Part B Physician Fee Schedule (PFS) services rendered in 2018.

6 Signs It's Time to Fire a Staffer - Part 4
Here are six signs that it is time to fire an employee:
4. They take too many sick days. If they are truly sick, no problem: we are doctors and we want everyone to be healthy. However, we are also very intuitive when people are sick and when they are faking. And if you suspect one of you staff is faking sick to get off work, this is a sign they should go. For one thing, you need your employees to be doing their jobs. Small practices don't have an HR department and we can't just get a replacement out of the blue. Everyone in the office must make up the slack.

QUERIES (CLINICAL) |
Query: Diabetic Foot Infection
A 44 year old white male presented with an with ulcer sub 5th MTP with erythema and edema, lateral foot extending to the ankle. MRIs with and without contrast show increased bone marrow signal throughout the 5th metatarsal on T1 and T2. Abscess formation lateral 5th metatarsal is also present.
The patient was taken to surgery for an I&D. A total 5th ray resection was performed. I had hoped to leave some of the 5th met base but decided not to, after finding a large amount of pus around the 5th metatarsal base intra-operatively (and the MRI suggested osteomyelitis of the entire 5th metatarsal). The peroneal brevis was sutured to the cuboid periosteum and the wound was packed open. Bone from the 5th met head was sent for C&S. Remaining bone was sent to pathology.
The bone C&S grew out S. aureus and Enterococcus faecalis, both sensitive to vancomycin. Pathology, however, said there was no osteomyelitis in the bone specimen. I feel given the information provided, I treated this infection appropriately; however from a malpractice standpoint, the pathology report suggests otherwise. How does one reconcile the discrepancy? There should be histologic changes if osteomyelitis is present, right?
