PODIATRY IN THE NEWS
Podiatry Ranked 12th Best High-Paying Job in America for 2016

When choosing a career, it's important to seek out jobs that are meaningful and challenging, but a high salary doesn't hurt, either. US News & World Report recently released its 2016 Best Jobs Rankings, which determines the best occupations in the country based on median salary, employment rate, growth, job prospects, stress level, and work-life balance.

Mean annual salary: $137,480
Projected growth (2014 to 2024): 14%

It's no easy task to become a podiatrist, however. After completing a bachelor's degree, individuals must attend medical school, complete a three-year residency program, and pass the American Podiatric Medical Licensing Exam.

PODIATRISTS IN THE NEWS
ID Podiatrist Discusses Dangers of Artificial Nails

“Application of artificial fingernails can harbor a large number of pathogens,” according to Dr. David Blackmer, a podiatrist at Idaho Footcare. “The acrylic in the artificial nails can give rise to the growth of gram-negative bacilli (pseudomonas) and yeast between the natural and artificial nail. This area is a perfect location for other pathogens to grow.”

Dr. David Blackmer

“Onychomycosis may lead to the development of secondary bacterial infections, ingrown toenails, and in the rare case, loss of limb,” Blackmer said. Losing a limb because you can’t clean your toenails is a Darwinian punishment. So what can you do? Over-the-counter medicines that are commonly used to treat yeast infections, such as Threelac, is the best way to stop fungi development on hands – as well as other parts of the body.

PODIATRISTS IN THE COMMUNITY
NY Podiatrist Takes Polar Bear Plunge for Make-A-Wish

Braving winter air temperatures, podiatrist  Dr. Andrew Shapiro,  from Lido Beach, NY, took the plunge on Super Bowl Sunday into the frigid Atlantic Ocean waves in Long Beach, NY. This was the 9th year in a row he participated in this event for Make-A-Wish Foundation. Cheered on by Long Beach residents, more than 5,000 people jumped into the Atlantic Ocean for the 19th annual Long Beach Polar Bear Club Dip. Organizers expect to raise well over a half a million dollars from this event.

Dr. Andrew Shapiro

The Super Bowl Sunday tradition started in 1998 with two friends who began collecting funds for Make-A-Wish Foundation to honor the memory of their friend's 4-year-old son, who died of cancer. To date, they've raised more than $4 million to grant wishes for kids with life-threatening medical conditions.

PODIATRISTS AND FOOTWEAR
MN Podiatrist Designs Luxury Shoes

I'm pretty sure one of the worst feelings in the world is splurging on a pair of fancy heels, only to not be able to wear them. Dr. Marion Parke, a podiatric surgeon, knew this feeling all too well (both personally and with her patients). So she took her medical expertise and translated it into high-end footwear. Her namesake label is crafted in the same Italian factory that produces pieces for Jimmy Choo, Miu Miu, and more. But there's something totally different about hers, and that has to do with how they're made.

Dr. Marion Parke

Though she swears they're not "doctor-approved," they are designed with a better understanding of how feet operate. Broken down into four basic parts — padded arch support, an insole that better holds the heel, medical-grade foam (she tells us that it's like adding another layer of soft tissue to the bottom of your foot), and more material near the outside of the shoe — Parke's pieces focus on wearability. They're also really damn chic.

CODINGLINE CORNER
Query: Surgical Preparation of Wound for Graft

Is there a time limit on how many days before applying a tissue graft (CPT 15275) that the preparation (CPT 15004) can be performed?

David Hauser, DPM,  Holland, OH

Response: There is no written policy I am aware of that sets, restricts, or limits the number of days prior to application of tissue graft or skin substitute material when you can "prepare" the recipient site. Having said that, "medically necessary and reasonable" as well as standard of care would ultimately dictate the reimbursement. The question you need to answer is how far out from the application of the graft material would it be reasonable and effective to prepare the site.

Note that CPT guidelines state: "Surgical preparation codes 15002-15005 for skin replacement surgery describe the initial services related to preparing a clean and viable wound surface for placement of an autograft, flap, skin substitute graft or for negative pressure wound therapy...Do not report 15002-15005 for removal of nonviable tissue/debris in a chronic wound (eg, venous or diabetic) when the wound is left to heal by secondary intention. See active wound management codes (97597, 97598) and debridement codes (11042-11047) for this service."

RESPONSES/COMMENTS (EHR)
RE: US Wound Registry
From: Mark Stempler, DPM

As we all know by now, for meaningful use 2016, we must sign up for two registries. I have signed up with the US Wound Registry, and they have gone far above and beyond the call of duty to help me fulfill this requirement; from responding to emails at all odd hours and to contacting my EHR, Practice Fusion, in order to figure out how to extract the necessary data to submit to the registry. In addition, their cost is reasonable and possibly less than other registries.

Disclosure: I have no financial connection with this organization.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS)
From: George Jacobson, DPM

What about the liability of practicing with an MD degree? Could PICA still really be PICA? Maybe it would become MICA. What would the new rates look like, with the new exposure? Trial attorneys would have a field day and discredit the defendant's degree, taking away from the facts of a case. They know how to impugn a witness. Imagine a jury listening to a line of questioning about the entire training of an MD. Imagine you had to answer "NO" to each carefully crafted question about your training. The last question could be "what year did you pass the medical boards?" One could be compared to a "quack". The profession would be set backwards a generation with this fake 6-month MD degree. This is not the same as earning a dual degree. I'll remain a podiatrist, DPM, thank you.

George Jacobson, DPM, Hollywood, FL

Editor's comment: It is not the MD degree that increases scope; it is a license as an MD. No DPM would be required to upgrade the degree, and there is no reason why older practitioners should do so. To characterize the degree as a "fake" shows a lack of understanding of the educational certifying process necessary to grant an MD degree. Licensing would require the additional completion of hospital rotations comparable to any foreign medical student wishing to obtain a license.
