FORENSIC PODIATRY |
GA Podiatrist is Forenic Expert in Murder Retrial
The convicted ‘”Stocking Strangler” is fighting for a new trial nearly 30 years after he was placed on death row for murdering and raping three Columbus women. The court heard from two expert witnesses who spoke about two pieces of evidence found at the crime scenes that defense attorneys say, don't match. One piece of evidence attorneys discussed on Thursday was a shoe print found at one of the crime scenes. The print was outside a window the intruder climbed through at the home of 71-year-old Ruth Schwob. The shoeprint was from a size 10 shoe, but the defense called podiatrist Dr. Charles Fenton III to the stand and Fenton testified that Gary has a size 13.5 shoe. Fenton said he got the measurements from Gary in prison and even tried to see if Gary could squeeze into a size 10 shoe.
|
Dr. Charles Fenton III |
“I wasn't able to get his foot into this shoe,” said Fenton. “Even if he curled his toes, he wouldn't be able to get into a size 10 shoe.” Fenton noted that even if Gary were able to squeeze into a size 10 shoe, it would be uncomfortable to the point where Gary would struggle to walk. In cross examination, the prosecution responded with a question that drew gasps from the gallery. “How uncomfortable would it be?” asked Senior Assistant Attorney General Sabrina Graham. “Would it be more uncomfortable to put your foot into a sized shoe that's too small? Or to be caught by the police by raping and murdering a series of women?” Fenton responded by saying it would be more uncomfortable to be caught by police.

INTERNATIONAL PODIATRISTS AND SPORTS MEDICINE |
Canadian Podiatrist Discourages Runners From Watching Nail Removals
All runners endure beat-up toenails. They’re marks of distinction, reminders of races run, and kilometres travelled. Permanent toenail removal is not for the faint-hearted. During the 45-minute procedure, the podiatrist anesthetizes the tip of the toe and applies a tourniquet at the base to limit bleeding. The podiatrist then uses a variety of scary-looking implements to soften the tissue and trim the nail down to the root until it can be pulled from its bed.
|
Dr. Joseph Stern |
“I’ve had it done three times,” says Earl Blewett, 47, a Canadian ultra-runner living in Oklahoma. “The first time, I made the mistake of looking at my foot while it was happening; I passed out. I made sure I never looked again.” “We try not to have patients watch,” echoes Dr. Joseph Stern, a sports medicine podiatrist in Vancouver. “There’s a rule: The bigger the guy, the faster he passes out.”

DIABETES RESEARCH |
6 weeks of ABX Therapy for Non-surgically Treated Diabetic Foot Osteomyelitis
Little is known about the optimal duration of antibiotic therapy for diabetic foot osteomyelitis (DFO). French researchers compared the effectiveness of 6 versus 12 weeks of antibiotic therapy in patients with DFO treated non-surgically (i.e., antibiotics alone). [Alina Tone. Six-week versus twelve-week antibiotic therapy for nonsurgically treated diabetic foot osteomyelitis: A multicenter open-label controlled randomized study. Diabetes Care, pub online before print November 20, 2014, doi: 10.2337/dc14-1514.]
This was a prospective randomized trial comparing 6- versus 12- week duration of antibiotic treatment. Remission of osteomyelitis during the monitoring period was defined as complete and persistent (>4 weeks) healing of the wound (if present initially), absence of recurrent infection at the initial site or that of adjacent rays, and no need for surgical bone resection or amputation at the end of a follow-up period of at least 12 months after completion of antibiotic treatment. The researchers concluded that the present multicenter prospective randomized study provided data suggesting that 6-week duration of antibiotic therapy may be sufficient in patients with DFO for whom non-surgical treatment is considered.

E-HEALTH NEWS |
Anthem Hacked in ‘Sophisticated’ Attack on Customer Data
Anthem Inc., the second biggest U.S. health insurer by market value, said hackers obtained data on tens of millions of current and former customers and employees in a sophisticated attack that has led to a Federal Bureau of Investigation probe. The information compromised includes names, birthdates, Social Security numbers, medical IDs, street and e-mail addresses and employee data, including income, Anthem said in an e-mail. The company will notify customers who were affected and provide credit and identify-theft monitoring services for free, Chief Executive Officer Joseph Swedish said in a letter to members.
There’s no evidence that credit-card numbers or medical information such as claims, test results, or diagnostic codes were compromised, Anthem said. The Anthem breach is the biggest in the healthcare industry since Chinese hackers stole Social Security numbers, names and addresses from 4.5 million patients of Community Health Systems Inc., the second-biggest for-profit hospital chain, last year. The attack is on a similar scale to hacks of customer data from Target Corp. and Home Depot Inc. last year in terms of the number of people affected.

QUERIES (NON-CLINICAL) |
Query: Office Relocation
I am currently leasing an office in the heart of the medical district on a major street in my small town. I am entertaining an offer to buy an office, but it is a solid mile outside the medical district. The new office is still on a big street, but is less traveled than my current office. Has anyone else had a similar experience who can offer guidance?

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1 |
RE: HBO Credentialling
From: Lee Rogers, DPM
Here's a position paper that counters the American College of Hyperbaric Medicine's (ACHM) position that was just released early by JAPMA (e-published ahead of print). It may help many podiatrists attempting to get privileges to supervise HBO in hospitals and also may help prevent podiatrists from getting denial by Medicare and private payers as a result of the anti-competitive ACHM position paper.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 |
From: Alan Bloch, DPM
I recently saw a patient who had a $3,000 deductible and has Blue Care Network (BCN). We explained to him that he had a $3,000 deductible, and we collected $100 upfront as a good faith payment for his office visit and injection. The patient reluctantly paid this amount on his credit card. When I saw him in the exam room, he was very upset about having to pay this. I explained to him that with the changes in insurances, we had to change our policies and now collect upfront. He told me that he most likely will not come back as he cannot afford to continue to see me. The next day, he called BCN and spoke to a representative who told him that it wasn't customary for providers to collect upfront for deductible charges. He then called his credit card company to dispute the charges. What are people thinking today?
I also wanted to ask how many people are having their patients sign ABNs for BCBS or Medicare patients with high deductibles, so if patients try to dispute their services, we have an ABN that they signed an acknowledgement that these charges may not be covered.

RESPONSES/COMMENTS (CODINGLINE CORNER) |
From: John Scholl, DPM
With traditional Medicare (not Medicare replacement policies or Medicare HMOs), the vast majority of the patients you are seeing this year have yet to meet their Medicare deductible. Medicare must process the claims and then send the claims to the secondary insurance, which takes time. You should be getting paid little by little from the secondary insurance companies until the patients meet their deductible. When deductibles are met, you should start receiving the Medicare payments.
