FORENSIC PODIATRY

OR Podiatrist’s Shoe Print Analysis is Blow to Prosecution in Murder Trial

Now, 23 years after Kathleen Marie Smith's murder — one of the highest-profile crimes in Tempe history — a footprint will loom large at the trial of her alleged killer, 47-year-old Robert Ortloff. A key piece of evidence is the fleeing man's "big foot." Witnesses Lisa Pickett and her grandmother directed authorities to this footprint, saying they saw the man make it in the wet flowerbed. It was if he'd stuck his hand into a freshly poured sidewalk at Grauman's Chinese Theatre in Hollywood.

Dr. John DiMaggio

In January 2006, Dr. John DiMaggio, a "forensic podiatrist" hired by the prosecution, took measurements of Robert Ortloff's feet at the Maricopa County Jail by court order. DiMaggio reported that Ortloff's shoe size is 12 1/2 to 13, which certainly qualifies as big. But, after analysis of the photograph in the flowerbed, the podiatrist concluded that the footprint had been made by size-9 1/2 Nike sneaker, maybe a 10.

That dealt a huge blow to the prosecution's case because it doesn't take an expert to explain what that means — it's not Ortloff's print. "If the size estimate is correct," DiMaggio wrote, "[Ortloff] could not have worn the shoe that made that scene shoe impression." O.J. Simpson's defense attorney , the late Johnnie Cochran, might have put it like this: "If the shoe don't fit . . . you must acquit."

AT THE COLLEGES

FL Podiatrist Receives Temple Gallery of Success Award

Temple University School of Podiatric Medicine has announced that Dr. Jose M. Concha was a recipient of the Temple University Gallery of Success Award. Born in Lima, Peru, Dr. Concha is a 1996 graduate of Temple University School of Podiatric Medicine and developed a unique way to treat fungus infections.

Dr. Jose M. Concha

During his post-graduate residency at The Medical Center of Delaware, Dr. Concha performed a standardized study which proved the efficacy of tea-tree oil against many pathological fungi. The studies won him various accolades, including a National Residency Research Award and the 1998 William J. Stickel Award. Concha extended the study and developed a useful product known as “FungaSoap.” This natural liquid soap is marketed nationwide and has proven to effectively help many patients with conditions related to topical fungal infections.

In addition to research and inventions, Dr. Concha participates in diabetic education and amputation prevention programs. He helped to form The Advanced Vein Center of North Florida which treats lower extremity venous pathology.

QUERIES (CLINICAL)

Query: Recurring Metatarsal Fractures in Healthy Adult

I have a patient who is a healthy man in his early 50s, ideal body weight, no medical history, non- smoker, takes no medications. For the past 15 years or so he has had metatarsal fractures with increasing frequency. Now it happens about once a year. He has had at least one fracture of every lesser metatarsal (total fractures 12 - 15). They heal within the expected time when immobilized. Rigid custom orthoses and good stable shoes have not eliminated the problem. His feet are mechanically normal and stable except for mild rearfoot varus causing ankle instability (mild enough that orthotics with 2 degree everted posts stabilize him well.) There is no history of fractures anywhere else in the body. Bone densitometry is mid-range normal for age. He is undergoing agent orange testing at the VA by his own request, as he was exposed repeatedly in Vietnam. I would appreciate any ideas for investigating etiologies.

RE: Tetanus Coding (Elliot Udell, DPM)
From: Lee Rogers, DPM

The tetanus toxoid vaccine is supplied in a number of preparations, but the Td is the most widely available (little "d" meaning that the adult diphtheria booster dose is less than the dose given as a child in the TD vaccine). The Advisory Committee on Immunization Practices (ACIP) of the CDC recommends adults are immunized with the Td vaccine every 10 years, or every 5 if a tetanus-prone wound presents (see Table 1). Furthermore, diphtheria could be a podiatric problem. There is a rare form called cutaneous diphtheria, causing skin ulcerations. Long-term diphtheria toxins can cause peripheral neuropathy. Given that the CDC recommends this vaccine for tetanus prophylaxis, I think we're well within our scope when using the Td.
