WA Podiatrist Discusses His Battle With Gout
 
"As a 23-year-old avid athlete, I was far from an obvious candidate for a gout diagnosis. When I started experiencing flares of severe pain in my foot, I wrote it off as a sports injury, not thinking it could be something more serious. Little did I know at the time, I was one of the 9.2 million people in America who was affected by gout," says Huy D. Dang, DPM.

"It wasn’t until I was 27 years with my own podiatry practice that I finally realized that my chronic foot pain was likely to be due to gout and not a sports injury. I started the regular regimen of dietary changes and oral medications, including allopurinol, which reduced the frequency of the gout attacks," said the Dr. Dang. Despite that, the disease progressed. "That is when I decided to opt for the intravenous infusion therapy. In the first 3 weeks, I started experiencing relief from my symptoms, and by the end of the month, most of my joints were functioning better."

Dr. Facaros went to the New York College of Podiatric Medicine, where he graduated in 2007 as a Doctor of Podiatric Medicine. He completed a three-year residency training at Fountain Valley Regional Hospital and Medical Center in California.

Complaint Alleges NV Podiatrist in Kickback Scheme, FCA Violations
 
The United States government, on behalf of the Department of Defense and Department of Veterans Affairs, sued a number of entities over allegations they operated a kickback scheme to push the marketing, prescription, and acceptance of certain medications. Among these physicians were Amir Shalev, DPM and Ivan Lee Goldsmith, MD, sometimes receiving kickback payments via AS Enterprises Inc., a corporation in which Shalev was a director, the complaint alleges.

In December, the Centers for Medicare & Medicaid Services (CMS) released its 2021 Medicare physician fee schedule final rule. Within the final rule were some significant changes concerning remote patient/physiological monitoring (RPM) and the provision of other remote services. Here are the most significant takeaways from the final rule.
 
3.  Re-established Patient Requirement
 
Despite significant industry pushback, CMS finalized that it will reimplement the requirement that an established patient-physician relationship exists for the furnishing of RPM following the end of the COVID-19 public health emergency.

After months of working for call parity, podiatrists in Eugene, Oregon have finally officially succeeded! A group of podiatrists in Eugene now take ankle and foot trauma call at McKenzie Willamette Hospital and are being reimbursed for their work. The achievement is specifically impressive because of the battle they had to go through with a powerful local orthopedic group that wanted to limit their privileges.
 
OPMA, along with APMA, provided information and documentation to the hospital, making the case that podiatrists should be able to treat the entire range of foot problems, including ankle trauma. As of January 4, 2021, they now have this ability.

RE: CPME 320 Rewrite Doesn’t Require Pain Management

There is no documented requirement within the proposed CPME 320 document that directly addressed the escalating need for pain management from podiatric physicians. In fact, the proposal is to eliminate this essential rotation from the curriculum!  
 
The residents experiences in a behavioral health rotation should be expanded, NOT eliminated, to provide education in the treatment of pain. They should be educated not only in the risks associated with the prescription opioid medications for pain but, most importantly, in the need to identify the populations at the greatest risk for use and abuse. They should understand the role of counseling, behavioral modifications, and prevention of opioid use disorder. A wide breadth of knowledge of the current opioid crisis within the United States and the ability to proactively evaluate and treat our patients with long-term pain management issues is CRITICAL to our residency training.

I find it interesting that for the past many years, the question of “parity” keeps arising. In the meantime, it also appears to me that the vast majority of podiatrists are satisfied with their practices. It probably is a quiet majority - those who do not participate in writing articles or comment in these kinds of publications have busy practices and are earning what they feel is a satisfactory income with a healthy family lifestyle. I understand the concerns many of my colleagues presented. I also think that their frustration is not related just to podiatry, but is in fact with all medicine.
 
Limitations of practice are generic and part of how we treat medicine today. Ophthalmologists refer cardiac questions to cardiologists and do not think about trying to treat...

Aside from a few orthopedic foot and ankle surgeons (who we have some turf overlap with), I have never felt a lack of respect or parity from any of the MD or DO physicians that I have interacted with over the course of 30 years of practice. Disparity with payments makes little sense as our overhead expenses are the same as that of other doctors. I consider myself a podiatrist with foot and ankle surgery as a subspecialty so to speak. I believe the more podiatry residents train in hospitals with other MD and DO physicians, the more respect our profession will get in the future. I leave it to our associations to battle the payors for parity on that level. We should all support these efforts with our associations as there is strength in numbers.

RE: It's Time for Parity with MDs and DOs

Parity starts with the colleges of podiatric medicine teaching the same courses MDs and DOs are given. Case in point: For a number of years, the Illinois Podiatric Medical Association has tried, on behalf of the profession, to get the Scholl College of Podiatric Medicine (SCPM/RFU) at Rosalind Franklin University to teach gynecology, psychology, and pediatrics courses. These courses are the only ones missing for DPMs to be on par with MDs and DOs. These courses are taught to the students of the Chicago Medical School (CSM) at Rosalind Franklin University. How hard would it be to allow podiatry students to attend those same courses?
 
The administration of SCPM/RFU has, for all intents and purposes, deliberately blocked these courses from being taught to podiatric medical students. It is my opinion that what may be happening is the CMS is blocking it and RFU is going along. Four years at SCPM costs $170,000. The students at the SCPM are receiving a substandard education and deserve better.
 
In 1972, the Illinois College of Podiatric Medicine students walked out of school for two months, demanding a modern podiatric medical education. We were successful. Had we failed, there would not be a Scholl College of Podiatric Medicine! Time for change is painfully long overdue. Being nice has not worked.
