Multi-Disciplinary Team Ensures Optimal Care for Diabetics: MD Podiatrist

“In my opinion, neglecting to include any one discipline is a high predictor for undesirable outcomes,” Spector says. “As a vital member of the wound care team, podiatrists have multiple roles to play; they are usually the first physicians to recognize the significance of the ulcer. Podiatrists surgically debride necrotic tissue or drain infections when necessary – an often neglected, yet fundamental tenet of treatment. I make sure my diabetic patients have all the other pieces of the puzzle in place. We can’t achieve long-term success for these complex patients by ourselves.”

Podiatrists Return From 11th Mercy Mission to Nicaragua
 
Charles Morelli, DPM and Jeffrey Siegel, DPM recently returned from their 11th mission trip to Nicaragua. They again brought students and residents from around the country for training, and to allow them to assist in the training and education of the orthopedic residents at HEODRA hospital in Leon.

This year’s theme turned out to involve many post-trauma deformities with four Ilizarov frames, bone and soft tissue tumors, flat and cavus feet, bunions, etc. It was again a great mix of pathology for the residents, as well as the Nicaraguan residents and attendings. Sponsors for this mission included Bako Labs, ICS Software, MTF, Stryker, BME, and Mr. David Kidder.

96-Year-Old FL Podiatrist Still Doing Surgery
 
Would you trust a 96-year-old surgeon to cut you open? In Fort Lauderdale, there's no shortage of patients for Dr. Albert R. Brown, a podiatrist who has spent more than six decades helping people solve their foot problems. Often that involves filing down foot bones or the bony bumps — known as bunions — that form over bones. If Brown isn't the nation's oldest practicing surgeon, he's one of them. No such category exists in the Guinness World Records. "However, the category is active and open to applications," Sofia Rocher, public relations and digital coordinator for Guinness World Records North America Inc., wrote in an email.

Born in 1919 in Detroit, Brown became a pharmacist in 1940 and volunteered to help fight in World War II. He became a podiatrist in 1950 and quickly noticed how much pain resulted from traditional foot surgery methods. He says "little by little, I figured out what we now call minimal invasive surgeries," shifted his procedures away from hospitals, and since 1974, he has worked exclusively from his office. Brown was one of a handful of podiatrists to pioneer minimally invasive procedures, say alumni of the national organization founded by the Academy of Ambulatory Foot and Ankle Surgery.

WV Podiatrist Accused of Failing to Repay Loan
 
A couple in Mt. Gay claim they loaned a Logan podiatrist and her husband start-up funds for her medical practice, but have never been repaid. Judy Dingess and Matthew Lewis filed a lawsuit on Jan. 21 in Logan Circuit Court against Dr. Jamie Hall-Jasper and Scott Jasper, alleging breach of contract. 
 
According to the complaint, the Jaspers signed a promissory note from Digness and Lewis for $53,771.71, plus interest, agreeing to repay the amount through 25 percent of all incoming insurance payments. The plaintiffs seek a jury trial and recovery of the loan amount, plus interest and additional damages determined by the court.

BIOMECHANICS RESEARCH
U.K. Survey Aims to Discover Which Health Professions Prescribe Foot Orthotics
 
A team from the University of Salford, England, School of Health Sciences has launched a national survey to discover which health professions provide foot orthotics. The team also wants to know what type of patients and devices are involved. The survey is open until June. The aim of the survey is to learn about the variations between individual clinicians and professional groups in the use of foot orthotics, how skills in using orthotics are developed, and how research affects practice.

Chris Nester, BSc (Hon), PhD, a podiatrist and professor who heads the research program, said, “We are pretty convinced that foot orthoses are a good intervention, but the NHS [National Health Service] is struggling to understand where they are provided. While podiatry and orthotists have a strong voice in this field, other professions are not represented.”

Jeffrey Karr: I feel that the real issue is that there is not an FDA-approved method for treating osteomyelitis. As a result, industry is unable to support a lot of exposure of the percutaneous antibiotic delivery technique (PAD-T). I’m hopeful that this will change with a few of the new papers that are coming out soon, as the results clearly speak well for this technique.

RE: Companies Form New Alliance to Target Health-Care Costs

A recent Wall Street Journal article was titled "Companies Form New Alliance to Target Health-Care Costs: Twenty major employers band together in bid to hold down the cost of providing workers with benefits."
 
Does anyone know why this isn't considered collusion or why doctors and medical groups can't do this same thing? If podiatrists could share information about contracts with insurance companies, wouldn't that help in our negotiating power? No, I'm not pushing for a tower model where we share a tax ID; I'm asking about sharing data.

I recall two occasions when degree changes were granted in a very short period of time. In California in the 1960s, the CMA tried to eliminate DOs by having them change to MDs and then closed the DO schools. I think that about 85% of DOs turned in their degrees to obtain an MD certificate.
 
Then in the late '60s or early '70s, the DSC chiropodists were able to change to DPM degrees with evening and weekend courses at the colleges of podiatric medicine, specifically with courses in pharmacology and surgery. It can be done.

Kudos to Dr. Garoufalis for pointing out how important it is for patients with diabetes to be aware if they have dry cracking skin. Many diabetic patients with peripheral neuropathy and peripheral vascular disease have developed serious complications leading to amputations as a result of dry skin that lead to cracks and infections.
 
What compounds this problem is that because many diabetics do not have pain associated with this seemingly innocuous skin condition, they tend to be non-adherent when we advise them to use emollients. We can, however, do more for these people. There are many products that can be purchased and dispensed from our offices that do wonders for diabetics with dry skin. The advantage to going the route of in-office dispensing for this condition is that we can monitor adherence. If a patient returns 8 weeks later to have his nails debrided and still has dry cracking skin and says that he or she still has a full bottle of the emollient that was dispensed, it is time to have a conversation with the patient about what the ramifications are of his or her non-adherence. Having such a conversation could be limb-saving.
