APMWA Announces 28th Annual Student Writing Competition
The American Podiatric Medical Writers Association has announced its 28th Annual Student Writing Competition.

All papers MUST be non-technical in nature. Appropriate subjects include practice management, ethics, law, education, or any topic that would be suitable for a lay publication.

There is no word limitation. Papers will be graded for content, style, grammar, neatness, and overall impact.

First prize will be one thousand dollars ($1,000), sponsored by an APMA Educational Foundation endowment from Dr. and Mrs. Steven Berlin, and recognition in the APMA NEWS and the APMWA Newsletter. Honorable Mention Certificates may also be awarded.

GA Podiatrist Bequests Gift to APF in Honor of CA Podiatrist
Mort Wittenberg, DPM, a long-time supporter of American Physicians and Friends for Medicine in Israel (APF) recently made a generous gift in recognition of the efforts of its immediate past president, Dr. Paul R. Scherer. Dr. Wittenberg's gift will help support the Fellowship Program that brings exceptional Israeli physicians to the United States for a year of education at a major University Hospital setting with the intent of the doctor returning to Israel to educate other physicians.

Dr. Wittenberg and his wife have been dedicated supporters of Israel and the APF for more than 40 years. Dr. Wittenberg took time out of his practice in Georgia each year to establish a small clinic at Kfar Menachem in Israel to treat patients before Israel had official podiatrists in the Ministry of Health. He was one of the driving forces, which ultimately resulted in the establishment of the foot clinic at Hadassah Ein Kerem in Jerusalem.

UK Podiatrist Gets Boost from MP
A podiatrist who is embroiled in a battle to highlight what he claims is a flaw in the regulation of his profession has received backing from his Member of Parliament (MP). Mark Russell took himself off the Health and Care Professions Council (HCPC) register in protest at the way it protects patients from chiropodists and podiatrists who have been struck off for malpractice.

Russell says only podiatrists and chiropodists are protected titles by HCPC standards, meaning anyone struck off from the register can still practice in the industry under other titles, like foot health specialists or podologists. Now he has won the backing of MP Mark Menzies, who has written to the health secretary, Jeremy Hunt, calling for a legislation review.

The three speakers described research from their teams that detailed everything from phone-based technologies that can prevent wounds and gangrene to cutting edge vascular technologies that can predict healing. "The future is enormously bright," posited Professor Boulton, who is also President of the European Association for the Study of Diabetes (EASD).

Bipartisan Plan Calls for SGR Repeal, 0.5% Increases to 2018; Then Value-based Care
Congressional lawmakers on Thursday unveiled a bicameral, bipartisan agreement to repeal Medicare's physician payment formula and replace it with a system that would provide stable payment updates for five years and shift Medicare to a system based on value versus volume of care. The deal is the product of three congressional committees—Senate Finance, House Ways and Means, and House Energy and Commerce—working to eliminate Medicare's sustainable growth-rate formula and offer incentives for Medicare-participating physicians to move to alternative payment models.
Called the SGR Repeal and Medicare Provider Payment Modernization Act, the legislation would permanently repeal the SGR and provide an annual update of 0.5% from 2014 through 2018. The 2018 payment rates would be maintained through 2023 so physicians have time to receive additional payments through a merit-based incentive payment system.

Response: CPT 28120 [partial excision (craterization, saucerization, sequestrectomy, or diaphysectomy) bone (e.g., osteomyelitis or bossing); talus or calcaneus] is the appropriate code to bill for an excision of talus bone. The os trigonum is considered part of the talus.

We at Eneslow find that when a patient has one foot significantly longer (a size and a half or more is the general rule, though this may vary depending on the patient's needs) than the other foot, the patient will be more comfortable if she fits each foot individually and purchases two pairs of the same style. If the patient does not wish to do this, she would do better in shoes with adjustable closures so that each foot can get a more specific fit.

I have seen many of these in the last 40 years of practice. It always did not seem to matter about the severity of the fracture or the plane of the break. Many treatment plans have been suggested, not the least of which is wiring the segments back. My experience has shown that the most effective and successful way is to meticulously excise the sesamoid, leaving all soft tissue as close to intact as possible. This solves the problem without fear of hallux deviation. A sharp 15 blade is the best solution to a chronic problem.
Put the patient in a BK cast for 3 to 4 weeks, then switch to a dancer's pad. If he is still symptomatic after three months, do a bone scan to check if there is delayed union. It won't hurt to try bone stimulation, but do this in a cast while the foot is immobilized. If still symptomatic, resection of the entire sesamoid from a plantar (vs. medial) approach may be more practical. I wouldn't worry about hallux deviation if you can stitch the capsule on that medial side, and get some scarring medially. But of course, make the patient aware that this could still happen.

Since the fracture is 18 months old, intra-articular, exhibits sclerosed, smooth ends with no evidence of periosteal remodeling, and considering its poor blood supply to begin with, this likely represents an atrophic non-union, or pseudoarthrosis. This can be confirmed by bone scan or MRI. However, if there is any residual activity (unlikely), then a bone stimulator is certainly worth a try, but would not be expected to work in the absence of non-viable bone (which is likely what the patient has).
Assuming symptoms warrant, and IF he fails conservative treatment (an orthotic with kinetic wedge and dancer pad accommodation is worth a 3-month trial), then TOTAL removal of the sesamoid makes the most sense since it is unlikely one would prevent the hallux drift from a partial sesamoidectomy anyway.
Post-op, I'd likely still place him in an accommodative orthotic with the same accommodations anyway, so it makes sense to try it pre-op. I think the KISS approach makes the most sense here.
